Content: Peritoneal dialysis (PD) is an invaluable tool in the treatment of patients with end-stage renal disease, which does not preclude a renal transplant, but incurs lower costs than other treatment options and represents an alternative when the vascular access is not feasible. Moreover, PD can be integrated in the armamentarium of different therapies and constitutes an important option when logistical or organizational problems are present. The publication on hand gives an account of the most recent studies on PD outcome and adequacy, exploring how different fluids, schedules and techniques may help to determine the most tolerated and effective dialysis for each patient. Novel systems including continuous flow PD are probing new limits of efficiency and performance. Biocompatibility and adequate correction of anemia are also among the issues discussed in this book, giving the reader the opportunity to refresh or expand his knowledge in the field. This publication definitely deserves a place on the desk of those who are engaged in the day-by-day activity of PD.

Content: This book surveys experiences made with Polymethylmethacrylate (PMMA) dialysis membranes, ranging from a summary of the most recent experimental and clinical results to an overview of future potentials and new trends in research. Several articles treat the biophysical properties of the membrane, its chemical structure and manufacturing procedures aimed at obtaining hollow fibers suitable for medical purposes. Moreover, various experimental conditions are considered in order to identify the dialytic properties of the PMMA membrane and dialyzer series. In this context, particular emphasis is put on the last generation of PMMA membranes with higher porosity and ultrafiltration capacity. On the more practical side, an overview is given of various clinical conditions in which PMMA membranes are utilized at variable rates of ultrafiltration, blood and dialysate flows. The bio- or hemocompatibility of PMMA membranes is investigated, offering a summary of the blood-membrane interactions during hemodialysis. Furthermore, the importance of these aspects in relation to the chronic dialyzed and the acutely ill patients is highlighted, as well as long-term experiences made with different PMMA membranes. For those interested in the last developments and clinical results achieved with synthetic membranes, this book provides a substantial piece of information valuable either for further research or daily clinical practice

Content: Even though peritoneal dialysis (PD) is by now well established and its advantages in terms of clinical efficacy, social impact and individual tolerance are acknowledged, it is still underutilized on a global scale. In view of this fact, the publication at hand has two objectives, namely to provide help to identify possible obstacles to a wider application of PD and to advance a project called USS PD: Understanding, Starting and Sustaining Peritoneal Dialysis (initiated by the Department of Nephrology, Dialysis and Transplantation of the St. Bortolo Hospital in Vicenza, Italy). The initial part of this book is thus dedicated to basic principles of PD, as understanding how this technique works is one of the prerequisites to improve the quality of its application and, ultimately, its outcomes. In the second part, in-depth reviews help the physician to identify the benefits and problems involved in a PD program, facilitating the initiation of a new program or the start of new patients on PD. The third part, finally, is dedicated to potential complications and technical solutions designed to solve the problems of the different techniques. This helps physicians to sustain the use of PD after having acquired the know how and the capacity of starting the program. Everyone interested in understanding and implementing PD will highly profit from the papers presented in this publication

Content: In recent years, major concerns have been expressed about long-term derangements in hemodialysis patients and possible clinically relevant blood-membrane interactions. Preliminary studies employing biocompatible membranes and antioxidant agents such as vitamin E have demonstrated significant improvements in patient organ function and hemopoietic capacity with increased hematocrit and quality of life. Using a new technology, vitamin E has now been successfully bound to the inner surface of a hollow-fiber membrane, so that the patient’s blood is flowing in contact with a layer of antioxidant agent covering the inner surface of the membrane in the hemodialyzer. The opening chapters of this publication give an overview on oxidant stress and the role of vitamin E as antioxidant agent and elaborate on technical aspects, hydraulics and permeability characteristics of the membrane in question. The main part is dedicated to the hemocompatibility of the membrane, its effect on erythrocyte morphology and survival and the impact on white cell and platelet function. The metabolic effects and the antioxidant impact of the vitamin E-coated membrane are presented in the last part of the book.Intended as a reference tool for professionals, students and fellows involved in the care of end-stage renal disease patients, it is also hoped that this book serves as a starting point for further speculations and research in the field

Content: Over the last decade, the availability of new devices and membranes has led to the development of modern hemodialysis techniques in which clinical tolerance and efficiency have significantly improved patient morbidity and mortality. Nevertheless, there are still limitations imposed by hemodialysis, manifested by the long-term complications occurring in patients treated for several years.Since most of these complications seem to be related to the chronic inflammation of ESRD patients and the progressive accumulation of middle-large molecules such as beta-2-microglobulin, GIP, leptin and others, a novel approach is needed, capable of adequately removing these substances from the body of the patient. New polymers with a biocompatible surface and high affinity for specific molecules have already been introduced to the market, and adsorption might in fact represent a new form of solute removal.Featuring articles by renowned experts, this volume provides the reader with a comprehensive summary of the most updated technology in the field of hemodialysis. New devices and techniques are described and discussed in detail, complemented by preliminary results; the rationale for adsorptive therapies completes the analysis. Residents and fellows, as well as staff physicians and members of the academic and scientific community involved in practice and research in the field of hemodialysis will find this book a valuable addition to their bookshelves

Content: Uric acid disorders are involved in both nephrological and hematological diseases. One of these crystal-associated diseases which has been known since antiquity is gout. More recently, tumor Lysis syndromes have been identified which affect patients with cancer, especially in the phase of cellular destruction after chemotherapy. The detection of these hyperuricemic syndromes, together with the improved understanding of urate handling by the kidney, have spurred new interest in the pathophysiology of hyperuricemic states, their clinical consequences and management. Moreover, the recent development of a recombinant form of urate oxidase transforming uric acid into allantoin (Rasburicase) has caused new interest in the pathophysiology of hyperuricemia and the potential applications of this new drug. The multidisciplinary approach of this book offers new insights into the metabolic syndromes in question by uniting authors from the fields of biochemistry, pharmacology, rheumatology, onco-hematology, and nephrology. The result is a compendium of the present knowledge in the field, which will also be very useful as a reference tool for professionals and students who want to expand their knowledge on this topic

Content: Transplantation is today firmly established as the therapy of choice for end-stage organ failure. However, despite recent developments, this therapy is still not without challenges and risks: The necessity to take immunosuppressive drugs for the rest of one’s life to prevent allograft rejection trades the morbidity and mortality of organ failure for the risks of infection and cancer as well as for an increased mortality from cardiovascular disease. Thus, there is an urgent need for optimizing the outcome of transplantation by achieving long-term, drug-free graft acceptance with normal organ function.Recently, numerous insights into the dynamic inter-relationship of host immune responses elicited by donor antigen presentation have substantially broadened our understanding of the cascade of events resulting in the acquisition of tolerance. With the pharmacopoeia of the transplant biologist continually expanding, the potential treatment combinations have become baffling and their impact on strategies to induce tolerance even more complex. This book presents novel insights into the pathways of acute rejection and their monitoring through molecular tests, new immunosuppressive agents currently under development as well as the most recent and promising approaches to induce tolerance that have emerged from experimental animal studies

Content: Since high-permeability membranes were first introduced, their performance has constantly improved. The publication at hand focuses on a new polymer, polyethersulfone, which was designed to provide better hydraulic permeability as well as high diffusive performance, thus allowing for fine tuning for different clinical applications. In the first part of the book, physico-chemical characteristics and ultrastructural features of the polyethersulfone membrane with regard to different clinical uses ranging from low to high permeability membranes are dealt with. The second part is devoted to clinical applications of the polyethersulfone membrane in ESRD. Its high biocompatibility, combined with the matching set up of the hemodialyzer, was associated with stable functional efficiency and permeability with regard to albumin, amino acids, beta-2-microglobulin and sepsis-associated cytokines in different hemodialytic modalities such as hemodiafiltration, high-flux hemodialysis, paired hemodiafiltration (PHF online), and low-flux hemodialysis. This publication is intended for students, clinicians and researchers alike. Descriptions of innovative technological and methodological approaches regarding the evaluation of membrane and dialyzer performances and biocompatibility make it a useful basis for those interested in future comparisons among different hemodialyzers
