    Content: This paper analyzes the status of public debt management performance in 17 small states through the findings of the Debt Management Performance Assessment reports. Empirical evidence indicates that the higher the quality of a country's policies and institutions, the better is its capacity to carry debt and withstand exogenous shocks. Borrowing for productive purposes can be an important element in boosting growth of gross domestic product but, conversely, excessive borrowing or poorly structured debt in terms of maturity, currency, or interest rate composition can quickly offset the positive impact, deter new foreign and domestic investment, compromise reform programs, depress growth of gross domestic product, exacerbate the challenge of meeting debt service obligations, and may induce or propagate economic crises. Arguments in favor of sound debt management are especially compelling for small states that must mitigate the particular risks to which their economies are exposed. Against this backdrop, the paper identifies aspects of debt management where small states do relatively well and those where they perform poorly, relative to other developing countries, and examines the underlying factors at play. It elaborates on some of the successful measures taken by small states to enhance debt management performance and considers how these may be applied more broadly in other small states. The paper offers a number of practical suggestions to strengthen debt management performance.

    Content: In 1999 the U.S. Agency for International Development introduced performance-based contracting in an effort to improve the effectiveness of some of Haiti's nongovernmental organizations in providing basic health services, such as immunization and prenatal and maternal care. These providers has been operating under a payment system that reimbursed their expenses up to a ceiling. The new systems set performance targets and withheld a portion of their historical budget, allowing them to earn back the withheld payment plus a bonus if they met the targets. A one-year pilot involving three providers showed some marked improvements in performance.

    Content: In September 2002, RAND contracted with the U.S. Agency for Healthcare Research and Quality (AHRQ) to serve as the evaluation center for its national patient safety initiative. The evaluation center is responsible for performing a longitudinal evaluation of AHRQ's patient safety activities and for providing regular feedback to support the continuing improvement of this initiative over a four-year project period. This report presents findings on the history leading to the AHRQ patient safety initiative, the start-up of the initiative, and early activities through September 2003. It focuses on assessing the context and goals that were the foundation for the initiative and documents the baseline status of the activities being undertaken. The evaluation found the agency has done an impressive job in starting the patient safety initiative, despite unreasonable high expectations and insufficient funding. The evaluators identify four priorities for AHRQ that they believe will have the strongest positive impact on the future of the patient safety initiative: designing interim objectives to pull the health care system toward the long-term goal of reducing errors by 50 percent; developing a national patient safety data repository; participating in active public-private partnerships and supporting health care organizations in their implementation activities; and balancing research and adoption activities
