Abstract:
Ziel Identifikation der patientenbezogenen Einflussfaktoren auf den
AHB-Zugang Methodik Teilstandardisierte, schriftliche Befragung von Patientinnen und
Patienten zwischen 18 und 65 Jahren in 3 Akutkrankenhäusern der Region Hannover, die sich anhand ihrer Diagnose einer der folgenden sechs definierten
AHB-Indikationsgruppen zuordnen ließen: (1) operativ versorgte Knochenfrakturen, (2) Knie- und Hüftendoprothesen, (3) maligne
Erkrankungen aus der Viszeralchirurgie, (4) Herzerkrankungen ohne operative Intervention, (5) Herzerkrankungen mit operativer Intervention sowie (6)
Schlaganfall. Neben personenbezogenen Angaben (wie z. B. Alter und Geschlecht) wurden persönliche Rehabilitationsziele,
Rehabilitationsmotivation, Krankheitsverarbeitung, Funktionsfähigkeit und Gesundheitszustand sowie die Kenntnisse zum Wunsch- und Wahlrecht und der
persönliche Wunsch, eine AHB antreten zu wollen, erhoben. Ergebnisse Von den 1227 befragten Patientinnen und Patienten erhielten
insgesamt 42,5% eine Anschlussrehabilitation. Dabei variierten die prozentualen Anteile zwischen den Diagnosegruppen sehr stark und waren am
niedrigsten nach einer konservativ behandelten Herzerkrankung (3,2%) sowie am höchsten nach Knie- oder Hüftgelenksersatz
(98,1%). Unter Einbezug lediglich der Fälle mit tatsächlich AHB-relevanter Diagnose (n=1000) erhöhte
sich der Anteil der Personen mit AHB auf 51,7% (p 〈 0,001). Die
Gründe für eine nicht eingeleitete AHB lagen in seltenen Fällen in der Ablehnung durch den Kostenträger (0,7%),
der Ablehnung durch die Patientin/den Patienten selbst (1,6%) oder in der fehlenden Rehabilitationsfähigkeit (3,3%). In den
meisten Fällen blieb die Antragstellung ohne erkennbare Gründe einfach aus (28,5%). Als stärkster Einflussfaktor auf den
AHB-Zugang ließ sich klar die Diagnose identifizieren. Im Vergleich zu Knie- und Hüftendoprothese hatten die Patienten aus anderen
AHB-relevanten Diagnosegruppen eine deutliche verringerte Chance eine AHB zu erhalten (OR von 0,01 bis 0,07; p 〈 0,001). Der Wunsch des
Patienten/der Patientin, eine AHB antreten zu wollen, zeigte den zweithöchsten und ebenfalls signifikanten Einfluss auf das Antreten
einer AHB (OR=8,18; p 〈 0,001). Schlussfolgerung Da sich die AHB-relevante Diagnose als wichtigstes
Kriterium für den AHB-Zugang identifizieren ließ und keine messbaren Funktionsparameter starken Einfluss zu haben scheinen, kommt der
individuellen Arztentscheidung, einen AHB-Antrag zu stellen, eine besondere Bedeutung zu. Vor allem vor dem Hintergrund der geringen ärztlichen
Kenntnisse zum AHB-Indikationskatalog der DRV, des ärztlichen Handelns aus beruflicher Erfahrung heraus sowie dem fehlenden Fortbildungsangebot
(Gottschling-Lang, 2016) ist zu vermuten, dass die AHB-Beantragung wenig standardisiert und systematisch erfolgt. Um eine bedarfsgerechte
Patientenversorgung zu gewährleisten, sollten Assessmentverfahren etabliert und die Aus- bzw. Fortbildungen der Ärztinnen und
Ärzte um das Thema AHB ergänzt werden.
