    Kurzfassung: The purpose of this investigation is to describe the characteristics of professional and preprofessional learners who registered for and completed TF-CBTWeb, a modular, web-based training program designed to promote the dissemination of Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) and to demonstrate the feasibility of this method of dissemination. Between October 1, 2005, and October 1, 2012, a total of 123,848 learners registered for TF-CBTWeb, of whom 98,646 (79.7%) initiated the learning activities by beginning the first module pretest. Of those, 67,201 (68.1%) completed the full training. Registrants hailed from 130 countries worldwide, and they had varied educational backgrounds, professional identities (both professional and preprofessional), and a range of experience working with child trauma victims. Learners who were from the United States, students, those with master’s degrees, and those with fewer years of experience working with child trauma victims tended to have the highest course completion rates. Learners displayed significant increases in knowledge about each component of TF-CBT, based on module pretest and posttest scores. The advantages and limitations of this web-based training program evaluation are discussed, while important implications for the use of web-based trainings are reviewed.

    Kurzfassung: Male-to-female transgender women (TGW) experience high rates of substance use and HIV. A recent substance use trend is the use of prescription medication without a doctor’s consent. No research to date has examined the associations between this non-medical use of prescription drugs and HIV risk behaviour in TGW. In the present study, TGW recruited from community venues (N = 104) in the Mid-Atlantic region of the United States completed surveys assessing demographic information, non-medical use of prescription drugs, other substance use, injection practices and sexual risk behaviour. Twenty-four per cent of the sample reported lifetime non-medical use of prescription drugs across the following categories: analgesics (21.2%), anxiolytics (14.4%), stimulants (12.5%) and sedatives (8.7%). Participants reporting non-medical use of prescription drugs were more likely to report other substance use, needle use to inject drugs, injecting silicone and sharing needles. In multivariable analyses, non-medical use of prescription drugs was associated with unprotected sex, sex after engaging in substance use, and commercial sex work, after controlling for demographic factors. Self-esteem and social support from family served as protective factors for non-medical use of prescription drugs. HIV-prevention programmes focused on TGW in the United States may wish to expand their assessment of substance use to include the use of prescription medications without a physician’s consent.

    Kurzfassung: Why do parties offer broad or narrow policy agendas to voters? Taking up the call to focus more on ‘issue diversity’ in election campaigns, this article argues that agenda scope is informed by (1) parties’ experience with government participation and (2) their internal organizational structure. Non-governing challengers, which are losers in the current system, seek to change the political status quo by focusing on a few issues only, whereas mainstream parties have an incentive to reinforce existing patterns of competition and thus distribute their attention across a wide range of issues. However, the extent to which parties respond to these external stimuli depends on intraparty politics. Party leaders seek to satisfy vote- and office-seeking motivations and want to ‘appeal broadly’, whereas activists want the party to ‘speak to the base’ and narrow down its issue appeals. Analyses of party agendas in 18 European democracies (1950–2013) support these expectations.

    Kurzfassung: While transradial approach to conduct percutaneous coronary interventions offers multiple advantages, the procedure can cause radial artery damage and occlusion. Because radial artery is the preferred site for the creation of an arteriovenous fistula to provide dialysis, patients with chronic kidney disease are particularly dependent on radial artery for their long-term survival. Methods: In this retrospective study, we investigated the prevalence of chronic kidney disease in patients undergoing coronary interventions via radial artery. Stage of chronic kidney disease was based on estimated glomerular filtration rate and National Kidney Foundation - Kidney Disease Outcomes Quality Initiative guidelines. Results: A total of 497 patients undergoing transradial percutaneous coronary interventions were included. Over 70.4% (350/497) of the patients had chronic kidney disease. Stage II chronic kidney disease was observed in 243 (69%) patients (estimated glomerular filtration rate = 76.0 ± 8.4 mL/min). Stage III was observed in 93 (27%) patients (estimated glomerular filtration rate = 49 ± 7.5 mL/min). Stage IV chronic kidney disease was observed in 5 (1%) patients (estimated glomerular filtration rate = 25.6 ± 4.3 mL/min) and Stage V chronic kidney disease was observed in 9 (3%) patients (estimated glomerular filtration rate = 9.3 ± 3.5 mL/min). Overall, 107 of 350 patients (30%) had advanced chronic kidney disease, that is, stage III–V chronic kidney disease. Importantly, 14 of the 107 (13%) patients had either stage IV or V chronic kidney disease. Conclusion: This study finds that nearly one-third of the patients undergoing transradial percutaneous coronary interventions have advanced chronic kidney disease. Because many of these patients may require dialysis, the use of radial artery to conduct percutaneous coronary interventions must be carefully considered in chronic kidney disease population.

    Kurzfassung: This article examines the extent and types of clergy stress, the strategies used in coping with stressors, and the relationship between stressors and coping mechanisms in a sample of 261 Seventh-day Adventist pastors in North America. The results indicate that the most commonly reported stressors in order are: (1) lack of social support, (2) financial stress, and (3) time and workload stress. In terms of coping strategies, pastors sought relief most often through: (1) reflective growth/internal change; (2) social/emotional coping; (3) passive coping; and lastly (4) action-oriented coping. Significant correlational relationships existed between passive coping and financial stress, relocating stress, and congregational stress. In addition, we found significant inverse correlations between coping through reflective growth or internal change and relocating stress and congregational stress. There were no significant relationships with action coping or social/emotional coping and any stressor. Multi-regression analysis reveals that passive coping strategies were significantly related to financial stress. Thus, the greater the financial stress, the more likely pastors were to engage in passive coping strategies. Other coping strategies showed no significant relationships when included in multi-regression analysis. We conclude with recommendations for Church administrators to address structures and practices in place for pastors including an expansion of coping mechanisms to help pastors address their stress.

    Kurzfassung: Although the high prevalence of mental health issues among postsecondary students is well documented, comparatively little is known about the adequacy, accessibility, and adherence to best practices of mental health services (MHSs)/initiatives on postsecondary campuses. We evaluated existing mental health promotion, identification, and intervention initiatives at postsecondary institutions across Canada, expanding on our previous work in one Canadian province. Methods: A 54-question online survey was sent to potential respondents (mainly front-line workers dealing directly with students [e.g., psychologists/counsellors, medical professionals]) at Canada’s publicly funded postsecondary institutions. Data were analyzed overall and according to institutional size (small [ 〈 2000 students], medium [2000–10 000 students] , large [ 〉 10 000 students]). Results: In total, 168 out of 180 institutions were represented, and the response rate was high (96%; 274 respondents). Most institutions have some form of mental health promotion and outreach programs, although most respondents felt that these were not a good use of resources. Various social supports exist at most institutions, with large ones offering the greatest variety. Most institutions do not require incoming students to disclose mental health issues. While counselling services are typically available, staff do not reliably have a diverse complement (e.g., gender or race diversity). Counselling sessions are generally limited, and follow-up procedures are uncommon. Complete diagnostic assessments and the use of standardized diagnostic systems are rare. Conclusions: While integral MHSs are offered at most Canadian postsecondary institutions, the range and depth of available services are variable. These data can guide policy makers and stakeholders in developing comprehensive campus mental health strategies.

    Kurzfassung: The digging process using an excavator bucket was modelled, with the objective of introducing the interaction with the environment into the multi-body-simulation method. With today’s software tools, the dynamic behaviour of complex machinery can be effectively simulated. However, interaction with the environment, especially with soil is very challenging. This paper aims to contribute to meeting the challenges in this field of simulation. The given implementation makes use of analytical approaches originating from classical soil mechanics. After an analysis of the digging process, excavating tools, digging mechanisms and existing force calculation methods, approaches based on the ‘fundamental earthmoving equation’ and the ‘cavity expansion theory’ were used to model bucket filling, soil transport and bucket emptying along with accompanying forces throughout the described process. The simulation results are compared with experimental data available in publications. The main finding of this paper is a co-simulation, with a multi-body-simulation model capable of estimating the digging forces dynamically, with very high accuracy for low cohesion soil. Furthermore, the model also visualises the geometry of the soil surface, and the bucket contents, during the excavation cycle, in 2D, and can be extended for use in 3D.

    Kurzfassung: The steady increase in immigration in the United States over the past few decades has focused attention on the necessity for school districts to provide English language services in order for children with diverse language backgrounds to participate fully in public education. In this study, we utilize a large state-representative sample of elementary-aged students from culturally diverse backgrounds to examine gaps in the reading achievement of English learners versus their native English-speaking peers. Our inquiry examines the theoretical importance of micro–macro linkages to assess the relative impacts of school contexts and ethnic/cultural backgrounds on students’ reading achievement.

    Kurzfassung: Objective: To review the efficacy, safety, and place in therapy of ixekizumab for the treatment of moderate to severe plaque psoriasis. Data Sources: PubMed (1966 to July 2018) and clinicaltrials.gov were searched using the terms ixekizumab, LY2439821, interleukin-17, and psoriasis. Study Selection and Data Extraction: Human studies published in peer-reviewed medical journals in English were used. Data Synthesis: The efficacy and safety of ixekizumab has been primarily reported by 4 phase III trials (UNCOVER-1, UNCOVER-2, UNCOVER-3, and UNCOVER-J) and multiple post hoc analyses. The average proportions of patients achieving a 75%, 90%, and 100% reduction in their Psoriasis Area and Severity Index (PASI) were 89%, 70%, and 38%, respectively, after 12 weeks of therapy. PASI75 was maintained for up to 3 years in 80.5% of participants. Ixekizumab was statistically significantly more effective than ustekinumab, with 76.5%, compared with 59%, of patients achieving PASI90 in 52 weeks. The most common adverse events include nasopharyngitis (14.1%), upper respiratory tract infections (7.9%), and injection-site reactions (6.8%), which are similar to that for other biological agents. The risk of inflammatory bowel disease may be increased with ixekizumab. Relevance to Patient Care and Clinical Practice: This review summarizes and evaluates clinical data regarding the efficacy and safety of ixekizumab and discusses relevant differences compared with other biological agents used for the management of chronic plaque psoriasis. Conclusions: Ixekizumab is a highly efficacious and well-tolerated treatment option for patients with moderate to severe plaque psoriasis.
