    Abstract: In recent years, analysis of the second order statistics of ultrasound speckle has led to accurate prediction and measurements of the average speckle size in the transducer focal zone. In this paper, that work has been extended to the average speckle size as determined by the normalized autocovariance in the presence of transducer phase aberrations. In general, a phase aberration causes a narrowing of the main lobe of the normalized autocovariance in the lateral direction. However, the lateral speckle autocovariance also showed significant side lobes in the presence of phase aberrations, indicating that individual speckles in a region of interest are not independent but are correlated so that less information is present for the task of signal detection when a transducer phase aberration exists. The same evidence of correlated speckle was found in the near field of a transducer in the region of fine speckle texture. This explanation satisfies the quandary of poor detectability in the near field region where the speckle is fine but the lateral resolution is quite degraded. The axial speckle in the presence of phase aberrations showed a small increase in main lobe widths and no evidence of side lobes. Beginning in 1978, the analysis of the second order statistics of speckle images for the purpose of spatial compounding led to accurate measurement and prediction of the cross-correlation curve as a function of transducer aperture translation for purposes of spatial compounding. In this paper, that work has been extended to the presence of transducer phase aberrations. The existence of transducer phase aberrations causes significant increases in the rate of decorrelation of speckle interference patterns as a transducer is translated. This indicates that spatial compounding will result in quite significant improvements in area-wise SNR and low contrast lesion detection for the case of severe random aberrators or focal point errors.

    Abstract: Pulmonary embolism is a rare but life-threatening complication of cardiac catheterization. Underlying deep venous thrombosis (DVT) is often not detectable clinically. To determine the true incidence of DVT the authors prospectively studied 450 consecutive patients (29% women, 71% men, mean age: fifty-eight years) undergoing a diagnostic cardiac catheterization. Patients were examined clinically and by duplex sonography with a high- resolution (5 or 7.5 MHz) transducer before and twenty-four hours after catheterization before mobilization. Duplex sonography excluded complete proximal DVT in all patients. Only partial occluding thrombi (pDVT) were detected in 11 (2.4%) patients. The thrombi were always localized at the puncture site. In 2 patients a difference was found in the circumferences of the legs, but no other clinical signs of DVT were seen. With use of continuous wave (cw) Doppler sonography, only 3 of these 11 patients (27%) showed a spontaneous (s) sound. Phlebography was performed in 4/11 patients (36%). In 2 patients the diagnosis was confirmed; in 1 patient extravenous compression was assumed, and the other demonstrated a normal-appearing phlebography at the time of investigation. Logistic regression analysis yielded a 3.5 times higher risk for developing a pDVT if a venous puncture was performed in addition to arterial puncture. Furthermore a 9.8 times higher risk was found if more than one venous puncture was necessary. During the follow-up no patient developed clinical signs of pulmonary embolism. The results of this study demonstrate that DVT is a rare complication of cardiac catheteriza tion (0/450 patients), but pDVT occurred in 2.4%. Risk factors for pDVT are the venous puncture itself and multiple puncture attempts. Clinical relevance of pDVT remains to be determined.

    Abstract: The composition of mesenchymal stromal cells (MSCs) changes in the course of in vitro culture expansion. Little is known how these cell preparations are influenced by culture media, plating density, or passaging. In this study, we have isolated MSCs from human adipose tissue in culture medium supplemented with either fetal calf serum (FCS) or human platelet lysate (HPL). In addition, culture expansion was simultaneously performed at plating densities of 10 or 10,000 cells/cm 2 . The use of FCS resulted in larger cells, whereas HPL significantly enhanced proliferation. Notably, HPL also facilitated expansion for more population doublings than FCS (43 ± 3 vs. 22 ± 4 population doubling; p 〈 0.001), while plating density did not have a significant effect on long-term growth curves. To gain further insight into population dynamics, we conceived a cellular automaton model to simulate expansion of MSCS. It is based on the assumptions that the number of cell divisions is limited and that due to contact inhibition proliferation occurs only at the rim of colonies. The model predicts that low plating densities result in more heterogeneity with regard to cell division history, and favor subpopulations of higher migratory activity. In summary, HPL is a suitable serum supplement for isolation of MSC from adipose tissue and facilitates more population doublings than FCS. Cellular automaton computer simulations provided additional insights into how complex population dynamics during long-term expansion are affected by plating density and migration.

    Abstract: Familial hemiplegic migraine (FHM) is a rare, genetic form of migraine with aura. The severity of the aura imposes an effective prophylaxis that is currently based on standard anti-migraine drugs. To this concern, only short-term reports are currently available. Methods Eight patients from a multigenerational FHM type 1 family harbouring a T666M mutation in the CACNA1A gene were referred to our ataxia outpatient clinic. Medical history, general and neurological examination as well as therapeutic approaches were recorded regularly on a routine basis for an average period of 13 years (range 9–15 years). Brain imaging studies and EEG data were also collected. Results Our long-term follow-up revealed that ictal manifestations, which usually improve after the adolescence, may reoccur later in the adulthood. Permanent neurological signs as assessed by means of clinical evaluation as well as follow-up MRIs, EEGs and neuropsychological testing remained stable. Interval therapy with non-selective calcium antagonists reduced the burden of migraine attacks and was well tolerated in the long term.

    Abstract: This study presents a pool of AIDS-related items that researchers and clinicians could use to develop self-report AIDS questionnaires for children and adolescents. A total of 352AIDS-related itemsfrom 18AIDSsurvey studies involving children and adolescents, ranging in age from 10 to 21 years, were submitted to content analysis. Unique items with 85% agreement among raters, and items with the higher percentage of agreement (85% or above) among repeated items, were selected. On the basis of these criteria, 164 AIDS-related items were identified AIDS-related items were integrated intofwve mutually exclusive, theoretically defined groups: (a)factual knowledge, (b) misconceptions, (c) attitudes, (d) perceived susceptibility, and (e) perceived self-efficacy. Selection of itemsfrom these groups could lead to the development of a comprehensive and uniform self-report AIDS questionnaire for children and adolescents, and to meaningful comparisons of AIDS surveys by researchers and clinicians using similar items.

    Abstract: Allergic bronchopulmonary aspergillosis (ABPA) is a severe complication in patients with cystic fibrosis (CF), resulting in deterioration of lung function and impairment of overall prognosis. Standard therapy consists of high dosage, long-term corticosteroid treatment. This carries the risk of serious side effects such as immune suppression, diabetes and osteoporosis. Antifungal drugs such as itraconazole may cause interactions with other drugs and drug levels need to be monitored. Omalizumab treatment has been tried in several case studies. Methods: This was a retrospective study of six patients (four female, two male, age 4–33 years old) with CF and ABPA treated with omalizumab within an observation period of 7.5 years. Results: All patients showed clinical and laboratory stability or even an improvement within the treatment and post-treatment observation period, although omalizumab therapy was less effective in patients with progressed lung disease and long-term ABPA. Side effects of systemic steroids were reduced. Conclusion: Omalizumab has the potential to be an additional and solitary treatment option in patients with CF and ABPA. Early onset treatment may be beneficial and patients with early stage of lung disease seem to benefit more.

    Abstract: Asthma in childhood has a prevalence of 5–10% in Germany and severe asthma accounts for about 5% in this patient group. Positive predictive values for severe asthma are atopy, a positive family history and sensitizations against inhalative allergens. Alternaria is an important inhalative allergen and sensitization is suspected to correlate with severe and lethal asthma. We investigated the prevalence and impact of Alternaria sensitization in paediatric asthma. Methods: We reviewed paediatric patients with a diagnosis of low-grade, moderate and severe asthma. Data collection included concomitant atopic diseases, sensitization profiles, family history and prior hospitalization for asthma exacerbation. Results: A total of 207 paediatric patients (aged 1–17 years) were included in the study. Overall, 25% had low-grade asthma, 31% moderate and 44% severe asthma and 26% were formerly hospitalized. Alternaria sensitization was the most common in moulds, although without significant correlation with hospitalization and severe asthma. Alternaria sensitization increased with age and was significantly associated with co-sensitization against other moulds, grass pollen and cat epithelia. Allergic rhinitis was significantly correlated with hospitalization, independent of Alternaria sensitization. Conclusions: Alternaria sensitization was common and increased with age. No significant correlation was found between asthma degree, hospitalization rates and sensitization profiles. Alternaria sensitization demonstrated no isolated risk factor for severe asthma and hospitalization.

    Abstract: Serum supplementation is crucial in in vitro cell culture to provide all the essential nutrients needed for cellular processes. Fetal bovine serum (FBS) is considered the ‘gold standard’, but its production raises serious ethical concerns. Human-derived alternatives to FBS exist in the form of human platelet lysates (hPLs) or human AB serum (ABS). However, these serum products are usually pooled from several donors, in order to have a standardised product without patient-specific deviations. Nevertheless, the use of patient-specific serum in cell culture might be the key to successful transplantation of the cultured cells in medical applications, particularly as it avoids the transmission of infectious components or xenogenic proteins. In addition, the production of non-pooled hPL from single donors is likely to be a cost-effective and time-saving method. The current study used hPL units isolated from single donors and tested their performance as medium supplements for cell culture in comparison with FBS or ABS. This proof-of-concept study aimed to assess the potential of non-pooled hPL for personalised serum supplementation, and thus optimise in vitro models by making them more relevant to human physiology. We showed that A549, HepG2 and Caco-2 human cell lines were generally able to adapt to the new culture conditions and maintain viability, morphology and certain cell-specific characteristics. These results indicate that non-pooled, single patient-derived hPL could be a suitable alternative for in vitro serum supplementation.
