    Abstract: The experience in treatment of 369 patients with stumps, shortenings and defects of finger phalanges and metacarpal bones is presented. Distraction method alone and distraction with combination of bone autoplasty were used. Thirty patients (8,1%) with amputated stumps, 77 patients (20,9%) with ununited and 93 patients (25,2%) with malunited fractures were treated using distraction only. In 42 patients (11,4%) with amputation stumps, 29 patients (7,9%) with pseudoarthroses, 31 patients (8,4%) with ununited, 28 patients (7,6%) with malunited fractures and in 39 patients (10,6%) with the defects of articular ends and metacarpal bones the distraction in combination with bone autoplasty was performed. Long term results were evaluated in 281 patients (76,1%). Good results were achieved in 163 patients (58%), satisfactory - in 111 (39,5%) and unsatisfactory - in 7 patients (2,5%).

    Abstract: There is an interesting tendency in the cinema of the 21st century first decade. We see the appearance of films based on the modification of the "film about film" theme which can be tentatively defined as "a film as an unfinished scriptn. The film's subject is the process of its creation. There are five particularly obvious examples: "Adaptation" (Spike Jonze, 2002), "A Movie about the Movies" (V. Rubinchik, 2002), "Reconstruction" (Chrisofler Вое, 2003), "Waiter" (Alex van Warmerdam, 2006) and "Stranger than Fiction" (M. Forster, 2006). The article investigates the peculiarities of these films' composition, the principles of the plot and attempts to understand the sources of this phenomenon in the cinema of the recent years.

    Abstract: Experience in surgical treatment of 112 patients with malunited fractures of distal radius epimetaphysis is presented. Indication to surgical intervention was fragments consolidation with displacement that was accompanied by marked wrist joint deformity and hand function disturbance. In all patients osteotomy and bone plasty were performed followed by application of distraction device. In the postoperative period dosed distraction of bone fragments up to their complete reposition and rehabilitation treatment was performed. Long term results were assessed for all 87 patients: good result was achieved in 67 (77%), satisfactory - in 17 (19.5%) and poor - in 3 (3.5%) patients.

    Abstract: Aim. To study the use of atomic-force microscopy for morphological diagnosis of a bone tissue and hard tooth tissue pathology. Methods. Dental occlusion, enamel crowns and bony tooth sockets shape and size, as well as embrasures’ size and their depth, and crown height were estimated by the means of atomic-force microscopy. 30 teeth and bony fragments removed due to blunt maxillofacial trauma with signs of connective tissue hypermobility and 27 removed teeth and bony fragments from the control group were analyzed. Results. Abnormal collagen structure in hypermobility syndrome was associated with the increase of inter-fiber space to 98 nm and decrease of mineral matrix content in the bone. Enamel crowns shape and size alterations [(5.5±0.3)×(5.4±0.1) μm] as well as inter-crown space increase to 1.5±0.1 μm were found out. At the bone tissue analysis, the main distinctive feature between the control group and the patients with hypermobility syndrome was the presence of cystic lesions in the hypermobility syndrome group, accompanied by change of bone mineral structures disposition, trabeculae formation and changes of bone mineral density. In patients with hypermobility syndrome low mineralization of hydroxyapatite crystal structure, changes in spatial crystal structure as well as organic matrix disposition, all leading to enamel structure malformation due to change of interrelation between enamel organic and mineral components which is not typical for that certain period of teeth ontogenesis were found at enamel examination. Thus, in patients with connective tissue hypermobility signs of bone and enamel mineralization and structure change were found. Conclusion. The use of atomic-force microscopy allows to study the native cell cultures, including bone and hard tooth tissues, that can be used as a basis to diagnose connective tissue hypermobility as well as for identification of personality.

    Abstract: Objective: to determine the capabilities and perspectives of island and perforator flaps for soft tissue reconstruction of the hand. Material and methods. Results of treatment of 41 patients with defects of the soft tissue of the hand, treated at the in the period from 2010 to 2013 and operated using the island flaps and perforator flaps (DAP flap). Results and conclusion. In all cases the authors have received positive functional and cosmetic results, which were evaluated using a questionnaire DASH, the average score was 12.3. Digital artery perforator flaps allow close defect covering tissue any surface fingers. The use of perforator flaps allows reconstructing soft tissue defect with the skin of the same anatomical region as identical morphologically lost. The second important advantage is that the DAP flap is not included in the magisterial arteries, allows its use as a recipient for subsequent microsurgical reconstructions.

    Abstract: Background. Personalized approach to the issue of pelvic organ prolapse in women assumes a detailed analysis of the patient’s expectations from the surgical treatment ahead. Aim: detailing the treatment goals for patients with POP. Materials and methods: Four hundred eighty six patients (mean age 54.3 ± 9.5 years) participated in a survey designed based on content from verbal reports of women with POP seeking medical help. Results: Out of 9 presented options patients indicated 2–7 treatment goals. The most frequently specified main goals of the treatment were: cure of urinary incontinence (48.6%), correction of vaginal wall prolapse (27.1%), and improvement of quality of sexual life (10.3%). Conclusion. Patients with POP indicate numerous objectives of surgical treatment. Treatment of urinary incontinence is the most frequently sought out main goal.

    Abstract: Hypothesis/aims of study. An integrated evaluation of the effectiveness of pelvic organ prolapse (POP) treatment in women requires a consistent interpretation of the criteria of treatment satisfaction. The aim of this study was to specify surgical treatment satisfaction in patients with POP. Study design, materials, and methods. Surgical treatment was performed in 486 patients with POP stages II–IV. In 12 months, a patient survey (using a 1-5-point scale) evaluated the level of treatment satisfaction, reasons for lowered self-reported satisfaction, and respondents’ ambivalence regarding treatment results. Results. One-year post-surgical treatment of POP stages 0–I are observed in 80.9% of patients. Full satisfaction (5 points) was self-reported by 61.1% of patients, 28.6% of patients indicated partial satisfaction (3–4 points), and 10.3% of patients indicated treatment dissatisfaction (1–2 points). Post-surgery stage of POP is weakly correlated with self-reported treatment satisfaction (r = –0.39; p 〈 0.0001). Most common reasons for lower self-reported satisfaction were the emergence of new issues, fear of pathology recurrence, urinary incontinence, and pain. Five patients (1.0%) indicated that their condition “became much worse” after the surgical treatment. Conclusion. The treatment satisfaction range is variable. It is necessary to consider a variety of clinical, deontological, and psychological factors in the analysis of surgical POP treatment satisfaction spectrum.

    Abstract: Background. Assessment of the compensatory capabilities of the body of patients after cardiac surgery at the stationary stage of rehabilitation is of significant scientific and practical interest from the point of view of individualization of rehabilitation therapy for such patients. Aim. The analysis of clinical and functional status of patients with acute coronary syndrome after percutaneous coronary interventions at the stationary stage of rehabilitation was carried out. Methods. The algorithm for a comprehensive assessment of health status included general clinical research methods, clinical and somatotypic status; assessment of the degree of stress of adaptive compensatory-adaptive reactions, the state of immunity and physical performance; biochemical research methods in blood serum; six-minute walk test; psychological research methods. Results. There are revealed signs of violations on the part of carbohydrate, lipid metabolism, antioxidant protection, increase of immune system reactivity, reduction of reserve capabilities of non-specific protective mechanisms, reduction of parameters of full-fledged socio-psychological functioning, which should be taken into account in the preparation of individual rehabilitation programs. Conclusion. The revealed relationship between integral leukocyte indices and a number of clinical and functional indicators allows us to recommend their use for analyzing the state of adaptive processes in the body and assessing the rehabilitation potential in patients with acute coronary syndrome after cardiac surgery.

    Abstract: Bone regeneration of patients with osteoporosis takes significantly longer than the rehabilitation period of individuals without bone pathology. This is due to the predominance of destructive processes in the setting of osteoblastic reaction weakened by osteoporosis, persistent inflammatory reactions, as well as proliferating connective tissue.Aim. Тo study the nature and dynamics of processes involved in the destruction and regeneration of bone tissue under various conditions, taking forensic medical assessment into account.Material and methods. We studied the autopsy and biopsy material of bone tissue using clinical-instrumental and histological research methods.Results. Using atomic force microscopy, it was found that in individuals with osteoporosis the formation of endosteal callus exhibits features associated with the impaired interlacing of collagen fibres due to the expanded space between them, and, as a result, an impaired connection between osseous structures with a low level of mineralisation.Conclusion. In accordance with paragraph 24 of Order 194n On the Approval of Medical Criteria for Determining the Severity of Harm to Human Health verified systemic osteoporosis that was the setting for the fracture, which in fact is a pathological fracture, should not be considered as damage to health.
