Cette thèse est fondée sur une recherche en cours dans le service de santé mentale pédiatrique de l'Hôpital Italien de Buenos Aires, en convention avec l'Université Paris-Descartes. La recherche s'inscrit dans une perspective médico-psychologique des liens, incluant le patient et la famille, à travers l'étude des processus psychiques impliqués dans les situations de groupe. Il s'agit d'une étude exploratoire. L'objectif principal consiste à étudier les effets intrapsychiques et intersubjectifs sur l'adolescent et le groupe familial confrontés à l'Insuffisance Rénale Chronique - IRC ; analyser si les liens intrafamiliaux et sociaux sont affectés par la chronicité dans la maladie rénale. La population d'étude est constituée par 15 patients âgés entre douze et vingt ans, et leurs familles. L'affection organique, l'âge, l'accord de participation à la recherche et l'appartenance à l'hôpital ont été les critères de sélection. Nous avons effectué cette recherche à travers un outil d'évaluation du fonctionnement des liens dans le groupe familial, au moyen d'entretiens, de productions graphiques, d'études de cas, d'analyses qualitatives et quantitatives, d'un questionnaire appliqué aux techniciens du service de dialyse, d'entretiens avec les professionnels, l'analyse de données médicales, etc. Nos hypothèses principales portaient sur le fait que la maladie rénale aurait de effets sur l'intrapsychique de l'adolescent, des conséquences sur l'appareil psychique familial et en particulier chez la mère. Nous avons vérifié selon nos hypothèses que, dans la majorité des cas étudiés, cette maladie chronique déstabilise la vie psychique de l'adolescent, affecte son développement et l'intégrité de sa famille; donnant lieu à une spécificité dans le lien parento-fïlial, affecté par la chronicité de la maladie rénale. C'est la mère qui, dans la grande majorité des cas, est l'aidant familial principal, et la plus vulnérable à la dépression dans le groupe familial

This thesis is founded on a research in progress at the Pediatric Department of Mental Health of the Italian Hospital in Buenos Aires, in convention with the University Paris Descartes. Research is part of a medical-psychological perspective of the bonds, including the patient and the family, through the study of the psychic processes involved in the group situations. It is an exploratory and descriptive study. The main aim consists in studying the intrapsychic and intersubjective effects of the teenager and his family group in front of chronic renal failure (CRF), to analyze if the intrafamily and social bonds are touched by what is chronic in the renal disease. The population of study consisted on 15 patients, between twelve and twenty years old, and their families according their ages, organic affections, the agreement of participation on the research and membership of the hospital; those were the selection criteria. The research has been made through a device group, clinical method, interviews, drawings, case studies, qualitative and quantitative analyses, a questionnaire applied to the dialysis service technicians, medical data, etc. Our principal hypothesis is related to the fact that the renal disease would have effects on the teenager's intrapsychic, consequences on the family psychic apparatus and particularly in the mother. We have checked according to our hypothesis that most of the studied cases; that are involved in this chronic disease destabilize the teenager's psychic life, the development of his adolescence and family integrity; giving place to a specificity in the parent-subsidiary bond, that is affected by the chronicle of renal disease. Mothers are the ones who mostly keep company and family support and they are also the most vulnerable to depression in the family group
