Inhalt: Positive deviance is an asset-based improvement approach. At its core is the belief that solutions to problems already exist within communities, and that identifying, understanding, and sharing these solutions enables improvements at scale. Originating in the field of international public health in the 1960s, positive deviance is now, with some adaptations, seeing growing application in healthcare. We present examples of how positive deviance has been used to support healthcare improvement. We draw on an emerging view of safety, known as Safety II, to explain why positive deviance has drawn the interest of researchers and improvers alike. In doing so, we identify a set of fundamental values associated with the positive deviance approach and consider how far they align with current use. Throughout, we consider the untapped potential of the approach, reflect on its limitations, and offer insights into the possible challenges of using it in practice. Inhalt: The EURACT Performance Agenda (EUPA) of the European Academy of Teachers in General Practice/Family Medicine (EURACT) is the third paper in a row following the European Definition of General Practice/Family Medicine (WONCA Europe) in 2002 which identified 6 core competencies and 11 abilities every general practitioner (GP) should master, and the EURACT Educational Agenda in 2005 which provided a framework to teach the core competencies by setting learning aims and monitoring their achievement. Performance (in contrast to competence) is understood as the level of actual performance in clinical care and communication with patients in daily practice. Small groups of EURACT Council members from 40 European countries have discussed and developed EUPA since 2007. EUPA is a general, uniform and basic agenda of performance elements every GP masters in daily practice, applicable and adaptable to different countries with different systems. It deals with the process and result of actual work in daily practice, not with a teaching/learning situation. EUPA discusses in depth the psychometrics and edumetrics of performance assessment. Case vignettes of abilities in GPs' daily practice illustrate performance and its assessment in every chapter. Examples of common assessment tools are workplace-based assessment by a peer, feedback from patients or staff and audit of medical records. EUPA can help to shape various performance assessment activities held locally in general practice/family medicine, e. g. in continuing professional development cycles, re-certification/re-accreditation/licensing procedures, peer hospitation programmes and practice audit programmes in quality management. It can give orientation for self-assessment for reflective practitioners in their continuing professional development. The EURACT Performance Agenda (EUPA) encourages general practitioners to initialize performance agendas adapted to their national health system to further strengthen the role of general practice/family medicine in their country.

Inhalt: Since the fall of communism, laissez-faire capitalism has experienced renewed popularity. Flush with victory, the United States has embraced a particularly narrow and single-minded definition of capitalism and aggressively exported it worldwide. The defining trait of this brand of capitalism is an unwavering reverence for the icons of the market. Although promoted as a laissez-faire form of capitalism, it actually reflects the very evils of selfishness and greed by entrepreneurs that concerned Adam Smith. Capitalism, however, can thrive without an extreme emphasis on efficiency and personal autonomy. Americans often forget that theirs is a rather peculiar form of capitalism, that other Western nations successfully maintain capitalistic systems that are fundamentally more balanced and nuanced in their effect on society. The unnecessarily inhumane aspects of American capitalism become apparent when compared to Canadian and Western European societies, with their more generous policies regarding affirmative action, accommodation for disabled persons, and family and medical leave for pregnant woman and their partners. In American Law in the Age of Hypercapitalism, Ruth Colker examines how American law purports to reflect--and actively promotes--a laissez-faire capitalism that disproportionately benefits the entrepreneurial class. Colker proposes that the quality of American life depends also on fairness and equality rather than simply the single-minded and formulaic pursuit of efficiency and utility.

Inhalt: The first successful open surgical repair of an abdominal aortic aneurysm was in 1951 by Dubost and represented a tremendous milestone in the care of this challenging disease. In addition to management of abdominal aneurysm, there is a chapter on treatment of aneurysms of the ascending aorta. Along with surgical treatment, one must also understand the molecular basis for how blood vessels remodel and thus, the role of cathepsins in aortic disease is elucidated.
