    Content: In the mid-2000s, India began rolling out large-scale, publicly-financed health insurance schemes mostly targeting the poor. This paper describes and analyzes Andhra Pradesh's Aarogyasri scheme, which covers against the costs of around 900 high-cost procedures delivered in secondary and tertiary hospitals. Using a new household survey, the authors find that 80 percent of families are eligible, equal to about 68 million people, and 85 percent of these families know they are covered; only one-quarter, however, know that the benefit package is limited. The study finds that, contrary to the rules of the program, patients incur quite large out-of-pocket payments during inpatient episodes thought to be covered by Aarogyasri. In the absence of data and program design features that would allow for a rigorous impact evaluation, a comparison is made between Andhra Pradesh and neighboring Maharashtra over an eight-year period spanning the scheme's introduction. During this period, Maharashtra did not introduce any at-scale health initiative that was not also introduced in Andhra Pradesh. Andhra Pradesh other health initiatives were considerably less ambitious and costly than Aarogyasri. The paper finds that Andhra Pradesh recorded faster growth than Maharashtra (even after adjusting for confounders) in inpatient admissions per capita (for all income groups) and in surgery admissions (among the poor only), slower growth in out-of-pocket payments for inpatient care (in total and per admission, but only among the better off), and slower growth in transport and outpatient out-of-pocket costs. The paper argues that these results are consistent with Aarogyasri having the intended effects, but also with minor health initiatives in Andhra Pradesh (especially the ambulance program) playing a role

    Content: Although a large literature discusses the productivity effects of land fragmentation, measurement and potential endogeneity issues are often overlooked. This paper uses several measures of fragmentation and controls for endogeneity and crop choice by looking at inherited paddy and wheat plots to show that these issues matter empirically. While crop choice can mitigate effects, fragmentation as measured by the Simpson index increases production cost and fosters substitution of labor for machinery, especially for small and medium farmers. Greater distances between fragments have a smaller effect. Creating opportunities for market-based consolidation could be one step to limit fragmentation-induced cost increases

    Content: This paper explores the conditions under which public spending could minimize violent conflict related to oil wealth. Previous work suggests that oil can lead to violent conflict because it increases the value of the state as a prize or because it undermines the state' bureaucratic penetration. Yet, little has been said on how oil wealth could be used to prevent the onset of violent conflict through public spending by buying off citizens and elites, increasing state legitimacy by providing basic services, or strengthening the military and security apparatus. The empirical analysis (148 countries over 1960-2009) shows that higher levels of military spending are associated with lower risk of small- and large-scale conflict onset in countries rich in oil and gas. By contrast, in economies with little natural resources, increases in military spending are associated with a higher risk of conflict. Welfare expenditure is associated with lower risk of small-scale conflict, irrespective of the level of oil revenue. However, general government spending does not appear to have any robust mitigating effects

    Content: The term energy access has various connotations to energy development specialists. For this review, we define energy access as relating both to physical proximity to energy infrastructure and to the policies and frameworks supporting the transition to better, reliable, and more efficient use of electricity and modern fuels. This viewpoint frames energy access as a development process sometimes referred to as the energy transition that starts with reliance on low-quality energy sources (straw, dung, candles) and finishes when high-quality energy sources, such as commercial fuels or electricity, are available. Access to these higher-quality energy sources allow for services (lighting, communication, cooling, pumping), which are not available at lower rungs of the energy ladder. This report focuses on the World Bank's portfolio of energy access-related projects approved during most of the past decade (FY2000-08). The objectives of the review were to compile an up-to-date data base on energy access-related assistance commitments and review current trends and patterns of energy access-related assistance. The authors also wanted to examine to the greatest extent possible the lessons that could be learned across regions, focusing on policy and project design recommendations. Finally, it was important to establish a solid methodology for measuring energy access in order to provide a baseline for future reviews of the investment portfolio. This study focuses on the World Bank's role in energy access investments for the period between fiscal years 2000 and 2008. Developing and transition countries face huge investments in energy access in order to meet their commitments to achieving the Millennium Development Goals (MDGs)
