Content: This book surveys experiences made with Polymethylmethacrylate (PMMA) dialysis membranes, ranging from a summary of the most recent experimental and clinical results to an overview of future potentials and new trends in research. Several articles treat the biophysical properties of the membrane, its chemical structure and manufacturing procedures aimed at obtaining hollow fibers suitable for medical purposes. Moreover, various experimental conditions are considered in order to identify the dialytic properties of the PMMA membrane and dialyzer series. In this context, particular emphasis is put on the last generation of PMMA membranes with higher porosity and ultrafiltration capacity. On the more practical side, an overview is given of various clinical conditions in which PMMA membranes are utilized at variable rates of ultrafiltration, blood and dialysate flows. The bio- or hemocompatibility of PMMA membranes is investigated, offering a summary of the blood-membrane interactions during hemodialysis. Furthermore, the importance of these aspects in relation to the chronic dialyzed and the acutely ill patients is highlighted, as well as long-term experiences made with different PMMA membranes. For those interested in the last developments and clinical results achieved with synthetic membranes, this book provides a substantial piece of information valuable either for further research or daily clinical practice

Content: In recent years, major concerns have been expressed about long-term derangements in hemodialysis patients and possible clinically relevant blood-membrane interactions. Preliminary studies employing biocompatible membranes and antioxidant agents such as vitamin E have demonstrated significant improvements in patient organ function and hemopoietic capacity with increased hematocrit and quality of life. Using a new technology, vitamin E has now been successfully bound to the inner surface of a hollow-fiber membrane, so that the patient’s blood is flowing in contact with a layer of antioxidant agent covering the inner surface of the membrane in the hemodialyzer. The opening chapters of this publication give an overview on oxidant stress and the role of vitamin E as antioxidant agent and elaborate on technical aspects, hydraulics and permeability characteristics of the membrane in question. The main part is dedicated to the hemocompatibility of the membrane, its effect on erythrocyte morphology and survival and the impact on white cell and platelet function. The metabolic effects and the antioxidant impact of the vitamin E-coated membrane are presented in the last part of the book.Intended as a reference tool for professionals, students and fellows involved in the care of end-stage renal disease patients, it is also hoped that this book serves as a starting point for further speculations and research in the field

Content: While continuous ambulatory peritoneal dialysis (CAPD) has been the standard peritoneal procedure since the seventies, different schedules of automated peritoneal dialysis (APD) have emerged during the eighties. Today, APD is considered a valuable tool in the management of ESRD patients, together with CAPD and hemodialysis. However, despite its frequent use, APD has § not yet been well assessed, and most pathophysiological and clinical studies on PD refer to CAPD. In this book, major experts in the field therefore discuss and evaluate the insights gained on APD up to now, presenting a comprehensive review of all experimental, technical and clinical aspects related to the various treatments grouped under the definition of APD. The recent developments presented are divided into four sections: membrane permeability, transport mechanisms and kinetic modeling applied to APD; prescription and adequacy of different APD treatment schedules; dialysis machines and solutions for APD, and, lastly, different clinical aspects such as the possibility to maintain APD program and residual renal function. Physicians involved in ESRD care, renal fellows and scientists both in the academic world and in the hospital setting will undoubtedly profit from this timely publication
