Content: This informative publication looks at the development and current state of otolaryngological research and practice in ASEAN countries. Written by leading experts in the field, it emphasizes a global approach to ear and hearing care, and documents and updates epidemiological aspects as well as educational programs for the deaf. Special attention is given to population-based epidemiological studies, which are amongst the most important tools in revealing the frequency of ear diseases and hearing disabilities in children and adults. These studies are of particular importance in cases where children are partially or completely deaf, and when early intervention is vital in order to preserve their ability to communicate and to guarantee their future prospects through appropriate education. Readers will also find an interesting discussion on the surgical treatment of carcinomas in the oral cavity and the management of nasal problems caused by anatomical variation, allergies or traumas. The book will be of immense value to those working in the field of ORL and related sciences

Content: Breaking new ground in terms of scientific analysis, this book addresses the question of who benefits most from treatment with recombinant human growth hormone. Outlined at the beginning of this book are the principles of evidence-based medicine along with a critical appraisal of the statistical issues that lie at the center of growth hormone trials. Each chapter reviews the current state of knowledge on the use of growth hormone in conditions ranging from Turner syndrome through other syndromes of intrauterine growth retardation to the short normal child, also highlighting issues that remain to be addressed in further research. Evaluating therapies in terms of efficacy and safety or the health benefit for the individual or society as a whole are rarely approached in pediatric endocrinology and for this reason a special chapter on health economic evaluation is included. This book is of interest and offers practical help to pediatricians and endocrinologists

Content: This book surveys experiences made with Polymethylmethacrylate (PMMA) dialysis membranes, ranging from a summary of the most recent experimental and clinical results to an overview of future potentials and new trends in research. Several articles treat the biophysical properties of the membrane, its chemical structure and manufacturing procedures aimed at obtaining hollow fibers suitable for medical purposes. Moreover, various experimental conditions are considered in order to identify the dialytic properties of the PMMA membrane and dialyzer series. In this context, particular emphasis is put on the last generation of PMMA membranes with higher porosity and ultrafiltration capacity. On the more practical side, an overview is given of various clinical conditions in which PMMA membranes are utilized at variable rates of ultrafiltration, blood and dialysate flows. The bio- or hemocompatibility of PMMA membranes is investigated, offering a summary of the blood-membrane interactions during hemodialysis. Furthermore, the importance of these aspects in relation to the chronic dialyzed and the acutely ill patients is highlighted, as well as long-term experiences made with different PMMA membranes. For those interested in the last developments and clinical results achieved with synthetic membranes, this book provides a substantial piece of information valuable either for further research or daily clinical practice

Content: In recent years, major concerns have been expressed about long-term derangements in hemodialysis patients and possible clinically relevant blood-membrane interactions. Preliminary studies employing biocompatible membranes and antioxidant agents such as vitamin E have demonstrated significant improvements in patient organ function and hemopoietic capacity with increased hematocrit and quality of life. Using a new technology, vitamin E has now been successfully bound to the inner surface of a hollow-fiber membrane, so that the patient’s blood is flowing in contact with a layer of antioxidant agent covering the inner surface of the membrane in the hemodialyzer. The opening chapters of this publication give an overview on oxidant stress and the role of vitamin E as antioxidant agent and elaborate on technical aspects, hydraulics and permeability characteristics of the membrane in question. The main part is dedicated to the hemocompatibility of the membrane, its effect on erythrocyte morphology and survival and the impact on white cell and platelet function. The metabolic effects and the antioxidant impact of the vitamin E-coated membrane are presented in the last part of the book.Intended as a reference tool for professionals, students and fellows involved in the care of end-stage renal disease patients, it is also hoped that this book serves as a starting point for further speculations and research in the field

Content: While continuous ambulatory peritoneal dialysis (CAPD) has been the standard peritoneal procedure since the seventies, different schedules of automated peritoneal dialysis (APD) have emerged during the eighties. Today, APD is considered a valuable tool in the management of ESRD patients, together with CAPD and hemodialysis. However, despite its frequent use, APD has § not yet been well assessed, and most pathophysiological and clinical studies on PD refer to CAPD. In this book, major experts in the field therefore discuss and evaluate the insights gained on APD up to now, presenting a comprehensive review of all experimental, technical and clinical aspects related to the various treatments grouped under the definition of APD. The recent developments presented are divided into four sections: membrane permeability, transport mechanisms and kinetic modeling applied to APD; prescription and adequacy of different APD treatment schedules; dialysis machines and solutions for APD, and, lastly, different clinical aspects such as the possibility to maintain APD program and residual renal function. Physicians involved in ESRD care, renal fellows and scientists both in the academic world and in the hospital setting will undoubtedly profit from this timely publication

Content: Advances in medicine have led to the survival of critically ill patients who would previously have died. The physiological response of patients to critical illness is becoming more clearly defined and this book aims to describe the current state of knowledge of the endocrine aspects of that response. The opening chapter with a careful consideration of carbohydrate metabolism, discusses the changes seen in peripheral glucose uptake and hepatic glucose output in illness. In the next chapter body fluids, solutes, osmolality, the role of the kidneys, thirst and its regulation are considered. Chapter three details work which suggests that there is hypersecretion of parathyroid hormone from an early stage of illness, amongst other topics. Characteristic changes seen in thyroid hormone concentrations during illness and the underlying pathophysiological processes are mentioned in chapter four. With further chapters on the Pituitary Gonadal Axis, The Neuroendocrine Regulation of GH Secretion and GH/IGF-1 as Anabolic Therapy this book provides an overview of the current state of knowledge of endocrine and metabolic function in acute illness. This book will be of interest to physicians, anaesthetists and endocrinologists involved in the care of patients with critical illness

Content: Today it is a well-established fact that cigarette smoking is fundamentally a form of addiction to the drug nicotine. However, this does not mean that smoking can entirely be explained in terms of pharmacological factors – as with any drug addiction, social, economic, personal and political influences all play an important part in determining patterns of prevalence and cessation. This publication provides a comprehensive, up-to-date review of tobacco smoking. Written by internationally acknowledged experts, the 15 chapters of the book cover the history, economics, epidemiology, and psychology of tobacco use. The medical consequences of both active and passive smoking are discussed in detail, and the pharmacological and behavioral aspects of nicotine dependence are highlighted. Several chapters focus on smoking cessation, examining patterns and predictors of cessation in the general population and the role of GPs, nurses and pharmacists, and evaluating nicotine replacement therapy and other drugs, as well as the role of smokeless tobacco. Measures to prevent smoking such as smoke-free environments, advertising bans, and price increases are also considered. Finally, recommendations are given to change legislation concerning tobacco products in order to most effectively reduce the harm associated with nicotine use. This book is a must for anyone in the medical profession interested in smoking-related issues. Due to its easy readability and topicality, it will also appeal to nonmedical readers

Content: One of the most demanding aspects regarding the endocrinology of aging is to determine the extent to which hormones change with age, independent of age-related pathologies, and the clinical significance of these changes. The emphasis of the present volume is on function, as it is of particular interest to many nonspecialists under what circumstances endocrine therapy may be useful in the elderly. For this purpose, leading investigators in their respective fields review the most important hormonal systems affected by aging. Furthermore, to facilitate the utility of the reviews particularly for nonspecialists, the book emphasizes the critical analysis of specific issues: The extent to which the hormones considered really do change with age, especially in humans; the physiological significance and the causes of these changes; and the prospects for treatment.Additionally, this volume also includes reviews on subjects such as cytokines, the sympathetic nervous system and leptin, which have not been traditionally considered in reviews of endocrine systems. However, while the concept of hormone replacement in the elderly is promising, as the chapters in the present volume indicate, much more research is necessary to determine under what circumstances such therapy is appropriate rather than counterproductive

Content: Saliva is essential for oral health and influences all events in the mouth. A deficiency of saliva can have devastating consequences. Therefore, it is important to have a book about the basic tissue mechanisms involved in the secretion of saliva, based on an holistic approach. With such an aim in mind, this book contains chapters covering the histological basis for secretion, electrophysiological events, electrolyte and water secretion, protein synthesis and secretion, bloodflow, capillary dynamics, myoepithelial activity, glandular permeability, hormonal influences, including the effects of diabetes, and the synthesis and secretion of IgA in man. The chapters have been written by international experts in the field, who present balanced accounts of their subjects. The secretory mechanisms described in this book have a far-reaching impact beyond the salivary field and have applicability to all forms of exocrine secretion. Salivary glands offer many advantages for experimental study of these phenomena since they can be tested more precisely than most other glands, having ducts that can readily be cannulated for assessing the secretion. As a single-source of up-to-date data, this book will benefit doctors and dentists, and anyone interested in secretory phenomena, including physiologists, biochemists and cell biologists. It not only brings together all available data on this subject, as it stands at the turn of the century, but also lays down a sound platform of knowledge on which further investigations can be based
