    Abstract: Adults with systemic right ventricle have a significant risk for long-term complications such as arrhythmias or heart failure. Methods: A nationwide retrospective study based on the German National Register for Congenital Heart Disease was performed. Patients with transposition of the great arteries after atrial switch operation or congenitally corrected TGA were included. Results: Two hundred and eight-five patients with transposition of the great arteries after atrial switch operation and 95 patients with congenitally corrected transposition of the great arteries were included (mean age 33 years). Systolic function of the systemic ventricle was moderately or severely reduced in 25.5 % after atrial switch operation and in 35.1% in patients with congenitally corrected transposition. Regurgitation of the systemic atrioventricular valve was present in 39.5% and 43.2% of the cases, respectively. A significant percentage of patients also had a history for supraventricular or ventricular arrhythmias. However, polypharmacy of cardiovascular drugs was rare (4.5%) and 38.5 % of the patients did not take any cardiovascular medication. The amount of cardiovascular drugs taken was associated with NYHA class as well as systemic right ventricular dysfunction. Patients with congenitally corrected transposition were more likely to receive pharmacological treatment than patients after atrial switch operation. Conclusion: A significant portion of patients with systemic right ventricle suffer from a relevant systemic ventricular dysfunction, systemic atrioventricular valve regurgitation, and arrhythmias. Despite this, medication for heart failure treatment is not universally used in this cohort. This emphasises the need for randomised trials in patient with systemic right ventricle.

    Abstract: Most patients born with CHD nowadays reach adulthood, and thus quality of life, life situation, and state of medical care aspects are gaining importance in the current era. The present study aimed to investigate whether patients’ assessment depends on their means of occupation. The findings are expected to be helpful in optimising care and for developing individual treatment plans. Methods The present study was based on an online survey conducted in cooperation with patient organisations. Participants were recruited from the database of the German National Register for Congenital Heart Defects. In total, 1828 individuals (777 males, 1051 females) took part. Participants were asked to rate aspects such their state of health on a six-tier scale (1=worst specification). Response behaviour was measured against the background of occupational details. Results Training for or pursuing a profession was found to be significantly associated with participants’ rating of five of the six examined aspects (p 〈 0.05). Sex seemed to play an important part in four of the six aspects. Conclusions An optimal treatment plan for adults with CHD should always consider aspects such as sex and employment status. To work out such an optimal and individual treatment plan for each adult CHD patient, an objective tool to measure patients’ actual CHD-specific knowledge precluding socially accepted response bias would be very useful.

    Abstract: We estimated the abundance of Antillean manatees ( Trichechus manatus manatus ) through a large-scale project conducted in 2010 in north-eastern Brazil and evaluated the efficacy of an aerial survey for conservation purposes. Two observers conducted the survey via flights that maintained an altitude of 150 m and an air speed of 140 km h −1 , covering over 2590.2 km 2 of the coastline. Strip transects were flown in a zigzag pattern. A total of 67 manatees (on- and off-effort) were recorded in 55 sightings. Historical published records of occurrence were formally incorporated using a Bayesian approach. We estimated the manatee population for north-eastern Brazil in the form of a posterior distribution with a mean of 1104 individuals and a 95% posterior probability interval ranging from 485 to 2221 individuals, which indicates high uncertainty. More large-scale studies in the region are warranted to understand temporal trends, in addition to further studies in hotspot areas, with smaller spatial scales, to reduce the coefficient of variation and to allow the use of improved techniques for monitoring the manatees. A greater emphasis on species-specific characteristics and methods to enhance detection probability (e.g. dual observers) are also recommended. The conditions prevailing along the study area were not conducive to aerial surveillance; thus, the results are not a precise estimate of the manatee population. However, these highlight the importance of conservation efforts for the Antillean manatee, considered the most endangered aquatic mammal in Brazil.

    Abstract: Injuries on alpine ski slopes have been described in cohorts of a reasonable sample size, but constant improvements in safety gear, increased use of airborne rescue, and safety measures during the coronavirus disease 2019 (COVID-19) pandemic mandate re-evaluation. Therefore, the purpose of this study was to evaluate skiing and snowboarding injuries, effectiveness of airborne rescue, and impact of the COVID-19 pandemic on a large sample size. Methods: Data on alpine injuries were prospectively collected from the state emergency services dispatch center in the state of Tyrol (Austria). A total of 10,143 patients were identified, with an average age of 33.5 years (SD = 20.36). The ski patrol was involved in 8,606 cases (84.9%) and some patients (n = 1,536; 15.1%) required helicopter rescue. Results: A total of 10,143 patients were identified from the dataset of the emergency dispatch center. The most frequently injured region was the knee (30.2%), and it was followed by the shoulder (12.9%), the lower leg (9.5%), and the head/skull (9.5%). Conclusion: The present findings indicate that the most frequent site of injuries on alpine slopes is the knee, and life-threatening injuries are rare. Airborne rescue is very time-effective, however clinical studies with patient follow-up should be emphasized to determine the impact of airborne rescue on patient outcome. The present findings indicate that the duration of all rescue operations has been prolonged as a result of the introduction of safety measures during the COVID-19 pandemic.

    Abstract: Atrial septostomy is performed in patients with severe pulmonary arterial hypertension, and has been shown to improve symptoms, quality of life and survival. Despite recognized clinical benefits, the underlying pathophysiologic mechanisms are poorly understood. We aimed to assess the effects of right-to-left shunting on arterial delivery of oxygen, mixed venous content of oxygen, and systemic cardiac output in patients with pulmonary arterial hypertension and a fixed flow of blood to the lungs. We formulated equations defining the mandatory relationship between physiologic variables and delivery of oxygen in patients with right-to-left shunting. Using calculus and computer modelling, we considered the simultaneous effects of right-to-left shunting on physiologies with different pulmonary flows, total metabolic rates, and capacities for carrying oxygen. Our study indicates that, when the flow of blood to the lungs is fixed, increasing right-to-left shunting improves systemic cardiac output, arterial blood pressure, and arterial delivery of oxygen. In contrast, the mixed venous content of oxygen, which mirrors the average state of tissue oxygenation, remains unchanged. Our model suggests that increasing the volume of right-to-left shunting cannot compensate for right ventricular failure. Atrial septostomy in the setting of pulmonary arterial hypertension, therefore, increases the arterial delivery of oxygen, but the mixed systemic saturation of oxygen, arguably the most important index of tissue oxygenation, stays constant. Our data suggest that the clinically observed beneficial effects of atrial septostomy are the result of improved flow of blood rather than augmented tissue oxygenation, provided that right ventricular function is adequate.

    Abstract: Two new species of Ordovician echinoderms are described: Pleurocystites mitratus from the Kiln Mudstones (Caradoc) at Craighead, and Homocystites beggi from the Starfish Bed (Ashgill) of Threave Glen. Description of the former necessitated a review of the composition and status of the family Pleurocystitidae. The genus Dipleurocystis Jaekel 1918 and the subgenera Pleurocystites s.s. and Pleurocystella Paul , 1967 are rejected. The number and type of pectinirhombs; size of the periproct, composition of its border and nature of its covering plates (periproctals), and the ornament of the thecal plates and proximal columnals, are the most useful taxonomic criteria within the Pleurocystitidae. Pleurocystites mitratus is characterized by three conjunct pectinirhombs, a symmetrical theca without a rectal lobe and small periproctals each with a central pit. Homocystites beggi is distinguished from the closely related H. constrictus by strong granular ornament on the thecal plates. Other echinoderm remains include plates of a cheirocrinid cystoid from the Kiln Mudstones.

    Abstract: Les inventaires de détection basés sur les phéromones des papillons lymantriidés exotiques (Lepidoptera : Noctuidae : Lymantriidae) nécessitent un espacement suffisant entre les pièges garnis d'appâts à phéromones spécifiques particulières pour éviter les effets antagonistes des hormones hétérospécifiques sur l'attraction des appâts. Dans des expériences de terrain avec la spongieuse japonaise, Lymantria dispar japonica Motschulsky, dans le nord du Honshû (Japon), nous avons d'abord déterminé les composantes congénériques qui ont de forts effets antagonistes sur l'attraction des papillons mâles à la phéromone conspécifique (7 R ,8 S )- cis -7,8-époxy-2-méthyloctadécane ((+)-disparlure). Comme les composés les plus antagonistes sont la phéromone (composantes volatiles) de la nonne, L. monacha (L.), une espèce sympatrique, nous avons monté des expériences avec des pièges appariés garnis ou bien de l'appât phéromonal de L. dispar (L.) ((+)-disparlure (50 µg)) ou alors de l'appât phéromonal de L. monacha (un mélange de (7 R ,8 S )- cis -7,8-époxyoctadécane ((+)-monachalure (50 µg)) de (7 Z )-2-méthyloctadécène (5 µg) et de (+)-disparlure (50 µg). L'effet antagoniste de l'appât de L. monacha sur l'attraction de l'appât de L. dispar diminue en fonction de la distance entre les pièges appariés (0, 0,5, 2, 7,5, 15, 30 m) pour finalement disparaître. Nous recommandons un espacement de 15 m entre les pièges pour les inventaires de détection basés sur les phéromones d'espèces multiples de papillons lymantriidés exotiques en Amérique du Nord.     Abstract: Dans une étude de piégeage réalisée dans la forêt expérimentale de recherche du Centre de recherche de Tohoku (Morioka, Honshu, Japon), nous avons examiné les effets de la phéromone hétérospécifique sur l’attraction phéromonale des mâles de la spongieuse japonaise, Lymantria dispar japonica (Motschulsky) et de la spongieuse rose, L. mathura Moore (Lepidoptera: Noctuidae: Lymantriinae). Les pièges garnis de phéromone synthétique de L. d. japonica (( 7R ,8 S )- cis -7,8-époxy-2-méthyloctadécane = (+)-disparlure (100 µg)) ou de L. mathura ((9 R ,10 S ,3 Z ,6 Z )- cis -9,10-époxynonadécadiène = (+)-mathuralure (20 µg) et ( 9S,10R,3Z,6Z )- cis - 9,10-époxynonadécadiène = (−)-mathuralure (80 µg)) attirent respectivement les mâles de L. d. japonica et ceux de L. mathura . Les pièges garnis des phéromones synthétiques des deux espèces capturent significativement moins de mâles de L. mathura que les pièges garnis seulement de la phéromone synthétique de L. mathura . Les captures de L. d. japonica dans les pièges munis de (+)-disparlure ne sont pas affectées par l’addition de la phéromone de L. mathura . Le (+)-disparlure est un antagoniste comportemental de l’attraction phéromonale des mâles de L. mathura , alors que les mâles de L. d. japonica restent indifférents à la présence de la phéromone synthétique de L. mathura .     Abstract: La tuberculosis (TB) aparece cada vez más en una forma resistente a múltiples fármacos (TB-RMF) también en Europa. El tratamiento es difícil debido a los diversos efectos secundarios de los regímenes multifármaco. La ciprofloxacina es muy utilizada como uno de los pocos fármacos de TB de segunda línea. Informamos sobre el curso de una psicosis aguda inducida por ciprofloxacina en una paciente con TB-RMF (isoniacida, estreptomicina) que se resolvió después de la interrupción del tratamiento de ciprofloxacina y la introducción de una nueva oxazolidona. Se recomiendan consideraciones de tratamiento cuidadosas, particularmente en los pacientes con factores de predisposición adicional a los síntomas neuropsiquiátricos, en la TB-RMF potencialmente peligrosa, creando así un enorme reto terapéutico.

    Abstract: Hypophosphataemia is frequently encountered in dairy cows during early lactation. Although supplementation of P is generally recommended, controversy exists over the suitability of oral P supplementation in animals with decreased or absent rumen motility. Since the effects of transruminal P absorption and the reticular groove reflex on the absorption kinetics of P are not well understood, it is unclear in how far treatment efficacy of oral P supplementation is affected by decreased rumen motility. Phosphate absorption was studied in six phosphate-depleted dairy cows fitted with rumen cannulas and treated with test solutions containing either NaH 2 PO 4 or CaHPO 4 with acetaminophen. Each animal was treated orally, intraruminally and intra-abomasally in randomised order. Absorption kinetics of P were studied and compared with the absorption kinetics of acetaminophen, a marker substance only absorbed from the small intestine. Intra-abomasal treatment with NaH 2 PO 4 resulted in the most rapid and highest peaks in plasma inorganic P (P i ) concentration. Oral and intraruminal administration of NaH 2 PO 4 resulted in similar increases in plasma P i concentration from 4 to 7 h in both groups. Treatment with NaH 2 PO 4 caused more pronounced peaks in plasma P i concentration compared with CaHPO 4 . Neither transruminal P absorption nor the reticular groove reflex affected P absorption kinetics as determined by comparing plasma concentration–time curves of P and acetaminophen after administration of 1 m -phosphate salt solutions. It is concluded that oral treatment with NaH 2 PO 4 but not CaHPO 4 is effective in supplementing P in hypophosphataemic cows with adequate rumen motility. Decreased rumen motility is likely to hamper the efficacy of oral phosphate treatment.
