    Abstract: The ability to perform optimally under pressure is critical across many occupations, including the military, first responders, and competitive sport. Despite recognition that such performance depends on a range of cognitive factors, how common these factors are across performance domains remains unclear. The current study sought to integrate existing knowledge in the performance field in the form of a transdisciplinary expert consensus on the cognitive mechanisms that underlie performance under pressure. Methods International experts were recruited from four performance domains [(i) Defense; (ii) Competitive Sport; (iii) Civilian High-stakes; and (iv) Performance Neuroscience]. Experts rated constructs from the Research Domain Criteria (RDoC) framework (and several expert-suggested constructs) across successive rounds, until all constructs reached consensus for inclusion or were eliminated. Finally, included constructs were ranked for their relative importance. Results Sixty-eight experts completed the first Delphi round, with 94% of experts retained by the end of the Delphi process. The following 10 constructs reached consensus across all four panels (in order of overall ranking): (1) Attention; (2) Cognitive Control—Performance Monitoring; (3) Arousal and Regulatory Systems—Arousal; (4) Cognitive Control—Goal Selection, Updating, Representation, and Maintenance; (5) Cognitive Control—Response Selection and Inhibition/Suppression; (6) Working memory—Flexible Updating; (7) Working memory—Active Maintenance; (8) Perception and Understanding of Self—Self-knowledge; (9) Working memory—Interference Control, and (10) Expert-suggested—Shifting. Discussion Our results identify a set of transdisciplinary neuroscience-informed constructs, validated through expert consensus. This expert consensus is critical to standardizing cognitive assessment and informing mechanism-targeted interventions in the broader field of human performance optimization.

    Abstract: To constrain seismic anisotropy under and around the Alps in Europe, we study SKS shear wave splitting from the region densely covered by the AlpArray seismic network. We apply a technique based on measuring the splitting intensity, constraining well both the fast orientation and the splitting delay. Four years of teleseismic earthquake data were processed, from 723 temporary and permanent broad-band stations of the AlpArray deployment including ocean-bottom seismometers, providing a spatial coverage that is unprecedented. The technique is applied automatically (without human intervention), and it thus provides a reproducible image of anisotropic structure in and around the Alpine region. As in earlier studies, we observe a coherent rotation of fast axes in the western part of the Alpine chain, and a region of homogeneous fast orientation in the Central Alps. The spatial variation of splitting delay times is particularly interesting though. On one hand, there is a clear positive correlation with Alpine topography, suggesting that part of the seismic anisotropy (deformation) is caused by the Alpine orogeny. On the other hand, anisotropic strength around the mountain chain shows a distinct contrast between the Western and Eastern Alps. This difference is best explained by the more active mantle flow around the Western Alps. The new observational constraints, especially the splitting delay, provide new information on Alpine geodynamics.

    Abstract: Ein günstiger Zeitpunkt für die Einstellung eines potentiell verlagerten Eckzahnes liegt vor dem eigentlichen Durchbruchszeitpunkt, in dem bspw. mit einer Milchzahnextraktion bzw. Lückenöffnung für den bleibenden Zahn ein Pfad zur passiven Eruption geschaffen werden kann. Geschieht dies nicht oder bleibt der Zahn verlagert, so entsteht eine komplexe Aufgabe, die dem kieferorthopädischen Behandler eine genaue Planung abverlangt und in interdisziplinärer Zusammenarbeit mit einem oralchirurgisch tätigen Zahnarzt, Oral- oder Mund-Kiefer-Gesichtschirurgen erfolgen muss. Das moderne kieferorthopädische Behandlungsspektrum bietet vielerlei Möglichkeiten für die Einstellung verlagerter Zähne. Für den folgenden Patientenfall zeigen wir eine Möglichkeit zur Einstellung eines verlagerten Eckzahns mittels Aligner-Therapie in Kombination mit zusätzlichen adäquaten Hilfsmitteln (Auxiliaries).

    Abstract: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92] ; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events.     Abstract: In biomechanics, large finite element models with macroscopic representation of several bones or joints are necessary to analyze implant failure mechanisms. In order to handle large simulation models of human bone, it is crucial to homogenize the trabecular structure regarding the mechanical behavior without losing information about the realistic material properties. Accordingly, morphology and fabric measurements of 60 vertebral cancellous bone samples from three osteoporotic lumbar spines were performed on the basis of X-ray microtomography (μCT) images to determine anisotropic elastic parameters as a function of bone density in the area of pedicle screw anchorage. The fabric tensor was mapped in cubic bone volumes by a 3D mean-intercept-length method. Fabric measurements resulted in a high degree of anisotropy ( D A = 0.554). For the Young’s and shear moduli as a function of bone volume fraction (BV/TV, bone volume/total volume), an individually fit function was determined and high correlations were found (97.3 ≤ R 2 ≤ 99.1, p 〈  0.005). The results suggest that the mathematical formulation for the relationship between anisotropic elastic constants and BV/TV is applicable to current μCT data of cancellous bone in the osteoporotic lumbar spine. In combination with the obtained results and findings, the developed routine allows determination of elastic constants of osteoporotic lumbar spine. Based on this, the elastic constants determined using homogenization theory can enable efficient investigation of human bone using finite element analysis (FEA).

    Abstract: Meta-analytical evidence confirms a range of interventions, including mindfulness, physical activity and sleep hygiene, can reduce psychological distress in university students. However, it is unclear which intervention is most effective. Artificial intelligence (AI)-driven adaptive trials may be an efficient method to determine what works best and for whom. The primary purpose of the study is to rank the effectiveness of mindfulness, physical activity, sleep hygiene and an active control on reducing distress, using a multiarm contextual bandit-based AI-adaptive trial method. Furthermore, the study will explore which interventions have the largest effect for students with different levels of baseline distress severity. Methods and analysis The Vibe Up study is a pragmatically oriented, decentralised AI-adaptive group sequential randomised controlled trial comparing the effectiveness of one of three brief, 2-week digital self-guided interventions (mindfulness, physical activity or sleep hygiene) or active control (ecological momentary assessment) in reducing self-reported psychological distress in Australian university students. The adaptive trial methodology involves up to 12 sequential mini-trials that allow for the optimisation of allocation ratios. The primary outcome is change in psychological distress (Depression, Anxiety and Stress Scale, 21-item version, DASS-21 total score) from preintervention to postintervention. Secondary outcomes include change in physical activity, sleep quality and mindfulness from preintervention to postintervention. Planned contrasts will compare the four groups (ie, the three intervention and control) using self-reported psychological distress at prespecified time points for interim analyses. The study aims to determine the best performing intervention, as well as ranking of other interventions. Ethics and dissemination Ethical approval was sought and obtained from the UNSW Sydney Human Research Ethics Committee (HREC A, HC200466). A trial protocol adhering to the requirements of the Guideline for Good Clinical Practice was prepared for and approved by the Sponsor, UNSW Sydney (Protocol number: HC200466_CTP). Trial registration number ACTRN12621001223820.

    Abstract: We take advantage of the new large AlpArray Seismic Network (AASN) as part of the AlpArray research initiative (www.alparray.ethz.ch), to establish a consistent seismicity-catalogue for the greater Alpine region (GAR) for the time period 2016 January 1–2019 December 31. We use data from 1103 stations including the AASN backbone composed of 352 permanent and 276 (including 30 OBS) temporary broad-band stations (network code Z3). Although characterized by a moderate seismic hazard, the European Alps and surrounding regions have a higher seismic risk due to the higher concentration of values and people. For these reasons, the GAR seismicity is monitored and routinely reported in catalogues by a 11 national and 2 regional seismic observatories. The heterogeneity of these data set limits the possibility of extracting consistent information by simply merging to investigate the GAR's seismicity as a whole. The uniformly spaced and dense AASN provides, for the first time, a unique opportunity to calculate high-precision hypocentre locations and consistent magnitude estimation with uniformity and equal uncertainty across the GAR. We present a new, multistep, semi-automatic method to process ∼50 TB of seismic signals, combining three different software. We used the SeisComP3 for the initial earthquake detection, a newly developed Python library ADAPT for high-quality re-picking, and the well-established VELEST algorithm both for filtering and final location purposes. Moreover, we computed new local magnitudes based on the final high-precision hypocentre locations and re-evaluation of the amplitude observations. The final catalogue contains 3293 seismic events and is complete down to local magnitude 2.4 and regionally consistent with the magnitude 3+ of national catalogues for the same time period. Despite covering only 4 yr of seismicity, our catalogue evidences the main fault systems and orogens’ front in the region, that are documented as seismically active by the EPOS-EMSC manually revised regional bulletin for the same time period. Additionally, we jointly inverted for a new regional minimum 1-D P-wave velocity model for the GAR and station delays for both permanent station networks and temporary arrays. These results provide the base for a future re-evaluation of the past decades of seismicity, and for the future seismicity, eventually improving seismic-hazard studies in the region. Moreover, we provide a unique, consistent seismic data set fundamental to further investigate this complex and seismically active area. The catalogue, the minimum 1-D P-wave velocity model, and station delays associated are openly shared and distributed with a permanent DOI listed in the data availability section.

    Abstract: Die Alignertherapie hat sich im Zuge der Digitalisierung in den vergangenen 20 Jahren zunehmend als nicht mehr wegzudenkende Behandlungsalternative in der Kieferorthopädie etabliert. Das Indikationsspektrum hat sich mit steigender Erfahrung und besserem Verständnis der Biomechanik vom Auflösen geringer Engstände und Schließen von Lücken zu komplexeren Fällen erweitert. Die Weiterentwicklung von kieferorthopädischer Computersoftware in Bezug auf die Verarbeitung von 3D-Daten macht es möglich, dass der Behandler heute entscheiden kann, ob der Workflow gänzlich in der Praxis bzw. im Eigenlabor stattfinden soll, oder ob Teile der Behandlungsplanung und Alignerherstellung an externe Dienstleister outgesourct werden. Der digitale Workflow für die In-House-Alignertherapie mithilfe der OnyxCeph³™ Software wird skizziert und an einem Beispiel verdeutlicht.

    Abstract: Okklusionsschienen sind definiert als intraorale Geräte zur Änderung der okklusalen Relation zwischen oberem und unterem Zahnbogen. Die Hauptindikation der Okklusionsschiene besteht in der Behandlung einer kraniomandibulären Dysfunktion (CMD) sowie zum Schutz von Zahnschmelz und Zahnhalteapparat. Aufgrund der beabsichtigten Änderung der Unterkieferpositionierung und der Neupositionierung der Kondylen bezeichnen wir die Okklusionsschiene als kraniomandibuläre orthopädische Positionierungsapparatur (COPA). Okklusionsschienen bestehen aus hartem Kunststoff. Nahezu immer verwenden wir eine COPA im Unterkiefer. Die Herstellung erfolgt im digitalen Workflow.
