    Abstract: Alzheimer's disease (AD) is characterized neuropathologically by high densities of neurofibrillary tangles (NFT) and neuritic plaques (NP) in the cerebral cortex, in particular in neocortical association areas and in the hippocampal formation. We report here the case of an AD patient who developed signs of motor dysfunction early in the clinical evolution. A quantitative neuropathologic analysis revealed much higher densities of NFT and NP in the primary motor cortex than is usually observed in AD. This case, together with other reports, points to the existence of neurologically defined subgroups of AD with unusual clinical deficits that are correlated with the regional and laminar distribution of NP and NFT, and further supports the hypothesis that the symptomatology presented by AD patients results from the loss of specific neuronal populations leading to a syndrome of global cortical disconnection.     Abstract: Mental health services for people with mental retardation have been receiving greater attention over recent years following a long period of inactivity. The main policy factors which influenced the developments of services were in the USA, the emphasis on deinstitutionalisation and normalisation, and in the UK the focus on social care. Progress in the assessment of psychiatric disorders and improved diagnostic techniques have stimulated the development of a wide range of mental health services for people with mental retardation, compatible with current ideology of community care on both sides of the Atlantic. Specialist multi disciplinary, community based and easily accessible mental health services should be available for people with mental retardation. Economic cost, quality of care, and training of professionals and direct care staff, remain some of the challenges faced by purchasers and providers of mental retardation and mental health services in both countries.

    Abstract: The effects of establishing an ease of access 'walk-in' service on General Practitioner referrals to the ambulatory services of a catchment area psychiatric service are outlined. The characteristics of the patients referred are described. GPs made immediate and sustained use of the walk-in service and its creation expanded use of the ambulatory services by an overall 50%. Individual GPs were found to use the ambulatory services disproportionately and to discriminate between their use of the walk-in service and the coexisting traditionally run outpatient clinic. The walk-in ser vice also allowed patients to refer themselves; a policy which did not result in a flood of inappropriate referrals but enabled patients, already familiar with the service, to re-establish contact in times of distress. The findings are discussed in terms of the current growth of Community Mental Health Centres in the United Kingdom and their implications for GP/Psychiatric Service liaison.

    Abstract: The trend of smart products is now changing the world and transforming the life of customers. People tend to be more tied to smart, connected products seeking to ease and improve quality of life and find new ways to make things such as daily tasks differently. In this context, the world of clothing has noticed a big advance targeting various applications: sport, gaming, and health monitoring and rehabilitation. Wearable smart products are intelligent hybrid products made of garments, sensor networks, and applications, interacting with users and the environment capable of real-time data processing and storage, with capabilities to extend functionalities by communicating with other things. The multi-functionality property of smart products needs the integration of various electronic parts. High variety of parts with different interfaces makes the development of smart products complex than generic products and requires the involvement and close collaboration of multidisciplinary team (garment designers, sensor networks designers, and application developers). A high number of alternative designs can be considered at the beginning of the design process due to this variety and high availability of parts. The smart product development team needs then to explore multiple candidate solutions before finding the best smart product design that complies with the requirements. This work proposes the set-based concurrent engineering to deal efficiently with the alternatives. Therefore, this article provides a novel approach to shift from set-based concurrent engineering theoretical principles toward a pragmatic (practical) application to wearable smart products taking advantage of product lifecycle management system capabilities for the implementation. The use of product lifecycle management system to support the set-based concurrent engineering process will contribute for a better understanding of the approach avoiding confusions, simplify the architecture of the solution in view of integration with other systems, and enable the stakeholders to focus on the level of innovation of the smart product.

    Abstract: Two notions have been at the center of the Egyptian constitutional debates since 2011: the ‘civil state’ and the ‘religious party’. The Muslim Brothers have played on the ambiguity of the notion of a ‘civil state’ as being neither secular nor theocratic, just as their understanding of an Islamic state. The Salafi al-Nūr Party has long refused to embrace the notion. Nevertheless, in 2019 it obtained from the Parliament’s Speaker a definition close to the one defended by the Muslim Brothers and endorsed it as a victory against the secular interpretation of the term. The same ambiguity appears regarding the notion of a ‘religious party’. The al-Nūr Party tried to prevent the interdiction of such parties in the 2014 Constitution. At the same time, it distances itself from the notion, and abides by the law, including Christian members, presenting female candidates, and organically separating political and religious activities.

    Abstract: The aim of this study was to assess the quality of TURBT (transurethral resection of bladder tumor) using surrogate parameters and evaluate adherence to the guidelines regarding the management of bladder tumors. Materials and methods: A clinical audit of all new diagnosis of bladder cancer was undertaken from January 2016 to January 2017. A total of 101 new bladder cancer cases were included. Surrogates of TURBT quality including presence of detrusor in the specimen, rate of re-TUR, presence of carcinoma in situ, and 3-month recurrence rates were analyzed. Adherence to guidelines regarding management of non-muscle invasive bladder cancer including time to re-TUR and utilization of single instillation chemotherapy was evaluated. Results: Absence of detrusor muscle in the specimen of the initial TURBT was noted in 22.8% of the cases. The chance of including muscle in the specimen was almost four-fold for tumors larger than 3 cm. A single instillation of intravesical chemotherapy following TURBT was administered in only 40% of eligible patients; 54.3% of patients had a re-TUR, the majority (61.3%) for high-grade non-muscle invasive bladder cancer on initial TURBT. Re-TUR was done on average 10 weeks after initial TURBT. The 3-month recurrence rate was 36.0% with larger tumors ( 〉 3 cm) being more prone to early recurrences. Early recurrences were not affected by intravesical instillations with bacillus Calmette–Guérin or mitomycin C although there was a positive association between the presence of carcinoma in situ on initial resection and early recurrences. Discussion and conclusion: One in two patients will have a re-TUR, and approximately one in two patients will have tumor on re-TUR. Single immediate chemotherapy instillations after TURBT are underutilized. The presence of carcinoma in situ on initial TURBT and tumor size were predictors of early recurrences.

    Abstract: To investigate whether neurofilament light polypeptide (NfL) level in cerebrospinal fluid (CSF), currently a prognostic biomarker of neurodegeneration in patients with multiple sclerosis (MS), may be a potential biomarker of cognitive dysfunction in MS. Methods This observational case–control study included patients with MS. CSF levels of NfL were determined using enzyme-linked immunosorbent assay. Cognitive function was measured with the Brief International Cognitive Assessment for MS (BICAMS) battery and Paced Auditory Serial Addition Test (PASAT3), standardized to the Greek population. Results Of 39 patients enrolled (aged 42.7 ± 13.6 years), 36% were classified as cognitively impaired according to BICAMS z-scores (–0.34 ± 1.13). Relapsing MS was significantly better than progressive forms regarding BICAMS z-score (mean difference [MD] 1.39; 95% confidence interval [CI] 0.54, 2.24), Symbol Digit Modality Test score (MD 1.73; 95% CI 0.46, 3.0) and Greek Verbal Learning Test (MD 1.77; 95% CI 0.82, 2.72). An inversely proportional association between CSF NfL levels and BICAMS z-scores was found in progressive forms of MS (r p  = –0.944). Conclusions This study provides preliminary evidence for an association between CSF NfL levels and cognition in progressive forms of MS, which requires validation in larger samples.

    Abstract: The single photon emission computed tomography (SPECT) radiotracer [ 123 I]iomazenil is used to assess benzodiazepine receptor binding parameters. These measurements are relative indices of benzodiazepine receptor concentration (B′ max ). To evaluate the ability of such indices in accurately accessing the B′ max the authors compared them with absolute values of B′ max , measured using positron emission tomography (PET). The authors performed SPECT, PET, and magnetic resonance imaging (MRI) studies on a group composed of seven subjects. For SPECT studies, the authors administered a single injection of [ 123 I]iomazenil and estimated the total and specific distribution volumes (DV T SPECT , DV S SPECT ) and the binding potential (BP) using unconstrained (BP SPECT ) and constrained (BP C SPECT ) compartmental models. For PET studies, the authors used a multiinjection approach with [ 11 C]flumazenil and unlabeled flumazenil to estimate absolute values of receptor concentration, B′ max , and some other binding parameters. The authors studied the correlation of different binding parameters with B′ max . To study the robustness of the binding parameter measurements at the pixel level, the authors applied a wavelet-based filter to improve signal-to-noise ratio of time-concentration curves, and the calculated kinetic parameters were used to build up parametric images. For PET data, the B′ max and the DV PET were highly correlated ( r = 0.988). This confirms that it is possible to use the DV PET to access benzodiazepine receptor density. For SPECT data, the correlation between DV SPECT estimated using a two-and three-compartment model was also high ( r = 0.999). The DV T SPECT and BP C SPECT parameters estimated with a constrained three-compartment model or the DV T ″ SPECT parameter estimated with a two-compartment model were also highly correlated to the B′ max parameter estimated with PET. Finally, the robustness of the binding parameters allowed the authors to build pixel-by-pixel parametric images using SPECT data.

    Abstract: Researchers have paid increasing attention to mental health issues in adults with autism spectrum disorders (ASDs) over the last decades. However, little is known about how rates of clinical referrals, types of mental health diagnoses and treatment in adults with ASDs and intellectual disability have changed. We examined patterns of change in referral trends to specialist mental health services in south London from 1983 to 2000 ( N = 137). The majority of the cases (58.4%) did not have a diagnosable psychiatric disorder. Schizophrenia was the most frequent psychiatric diagnosis followed by depression, adjustment reaction and anxiety. There was a significant change in the rate of referrals, an increase in the diagnosable psychiatric disorders over time and a significant reduction of medication at time of referral. There were no significant changes in the use of other therapeutic interventions. The proportion of participants living independently increased. Implications for services and future research are discussed.

    Abstract: Studies have shown that preterm birth significantly influences mothers’ psychological health. This study aimed to identify factors associated with preterm birth and assess postnatal depression and anxiety symptoms in mothers of preterm infants ( n = 75) compared to mothers who delivered at term ( n = 125) in a Greek sample. Multiple pregnancies, assisted reproduction technology, caesarean section, non-Greek ethnicity and smoking during pregnancy were associated with preterm delivery. Moreover, preterm infants’ mothers had higher depression, state anxiety and trait anxiety scores. These findings suggest that addressing preventable causes of preterm delivery is crucial, while mothers of preterm infants should receive postnatal support.
