    Abstract: HIV-infected individuals are susceptible to Mycobacterium tuberculosis ( M.tb ) infection and are at high risk of developing active tuberculosis (TB). Interferon-gamma release assays (IGRAs) are auxiliary tools in the diagnosis of TB. However, the performance of IGRAs in HIV-infected individuals is suboptimal, which limits clinical application. Interferon-inducible protein 10 (IP-10) is an alternative biomarker for identifying M.tb infection due to its high expression after stimulation with M.tb antigens. However, whether IP-10 mRNA constitutes a target for the diagnosis of TB in HIV-infected individuals is unknown. Thus, we prospectively enrolled HIV-infected patients with suspected active TB from five hospitals between May 2021 and May 2022, and performed the IGRA test (QFT-GIT) alongside the IP-10 mRNA release assay on peripheral blood. Of the 216 participants, 152 TB patients and 48 non-TB patients with a conclusive diagnosis were included in the final analysis. The number of indeterminate results of IP-10 mRNA release assay (13/200, 6.5%) was significantly lower than that of the QFT-GIT test (42/200, 21.0%) ( P = 0.000026). IP-10 mRNA release assay had a sensitivity of 65.3% (95%CI 55.9% – 73.8%) and a specificity of 74.2% (95%CI 55.4% – 88.1%), respectively; while the QFT-GIT test had a sensitivity of 43.2% (95%CI 34.1% – 52.7%) and a specificity of 87.1% (95%CI 70.2% – 96.4%), respectively. The sensitivity of the IP-10 mRNA release assay was significantly higher than that of QFT-GIT test ( P = 0.00062), while no significant difference was detected between the specificities of these two tests ( P = 0.198). The IP-10 mRNA release assay showed a lower dependence on CD4 + T cells than that of QFT-GIT test. This was evidenced by the fact that the QFT-GIT test had a higher number of indeterminate results and a lower sensitivity when the CD4 + T cells counts were decreased ( P & lt; 0.05), while no significant difference in the number of indeterminate results and sensitivity were observed for the IP-10 mRNA release assay among HIV-infected individuals with varied CD4 + T cells counts ( P & gt; 0.05). Therefore, our study suggested that M.tb specific IP-10 mRNA is a better biomarker for diagnosis of TB in HIV-infected individuals.

    Abstract: Subtype H9N2 avian influenza viruses (AIV), now the predominant avian influenza virus subtype in poultry in China, cause sporadic human infections manifesting as mild influenza-like illness. We isolated an H9N2 AIV from a critical case of respiratory illness in a 9-year-old with no underlying conditions. As this virus was associated with critical illness, we conducted a risk assessment to determine its mammalian pathogenicity. Clinical and laboratory data were collected from hospital records. A/Suzhou/GIRD01/2019 (H9N2) (GIRD01) was isolated from a throat swab and used in risk-assessment studies in comparison to prototypical and contemporary H9N2 AIVs and contemporary seasonal subtype H1N1 and H3N2 influenza viruses. The patient presented with fever, vomiting but rapidly declined to progressive wheezing followed by dyspnea. The patient was admitted to the intensive care unit and placed on mechanical ventilation. A diagnosis of pneumonia and type I respiratory failure was made. Viral RNA was detected in the bronchiolavelolar lavage and anal swab specimens, suggesting lower lung and extrapulmonary involvement. Respiratory syncytial virus was also detected by immunofluorescence in bronchiolavelolar lavage. Following an aggressive regimen of antiviral and antibacterial therapy, the patient recovered and was discharged from hospital after 13 days. GIRD01 was closely related to poultry-origin H9N2 AIVs in the area and contained several known markers of mammalian pathogenicity. GIRD01 also showed a strong affinity for mammalian-type over avian-type sialic acids. GIRD01 replicated more efficiently compared to older H9N2 viruses and contemporary seasonal viruses in vitro and produced asymptomatic infections in mice. In summary, GIRD01 was well-adapted to replication in in vitro and in vivo mammalian models but was not more pathogenic compared to similar contemporary strains of H9N2 AIVs. Therefore, these viruses may pose a risk of causing severe respiratory disease in humans.

    Abstract: Solid-state electrolytes (SSEs) hold the key position in the progress of cutting-edge all-solid-state batteries (ASSBs). The ionic conductivity of solid-state electrolytes is linked to the presence of both amorphous and crystalline phases. This study employs the synthesis method of mechanochemical milling on binary x Li 2 S-(100- x )LiI system to investigate the effect of amorphization on its ionic conductivity. Powder X-ray diffraction (PXRD) shows that the stoichiometry of Li 2 S and LiI has a significant impact on the amorphization of x Li 2 S-(100- x )LiI system. Furthermore, the analysis of electrochemical impedance spectroscopy (EIS) indicates that the amorphization of x Li 2 S-(100- x )LiI system is strongly correlated with its ionic conductivity, which is primarily attributed to the effect of grain boundary resistance. These findings uncover the latent connections between amorphization, grain boundary resistance, and ionic conductivity, offering insight into the design of innovative amorphous SSEs.

    Abstract: Introduction: Irregular bone defects of the humerus are common in clinical practice, but there are fewer reported cases of irregular humeral defects accompanied by shoulder joint “locking” dislocation and reverse Hill-Sachs injury caused by an electric shock. The choice of treatment for such cases is closely related to the extent of shoulder joint function recovery. This is a case report of a 60-year-old male patient who suffered from a shoulder joint “locking” dislocation with accompanying reverse Hill-Sachs injury due to muscle contraction after being electrically shocked at work. The patient was treated with a 3D-printed custom humeral head prosthesis for the treatment of the shoulder joint “locking” dislocation and reverse Hill-Sachs injury. Case presentation: A 60-year-old male patient, working as a construction worker, presented to our emergency department with right shoulder pain and restricted movement for more than 30 min after an electric shock. Right humeral CT revealed a comminuted fracture of the right humeral head. D-dimer levels were significantly elevated at 3239.00 ng/mL, and oxygen partial pressure was slightly decreased at 68 mmHg. Treatment included emergency wound debridement and dressing for the electrical injury, cardioprotective measures, anticoagulation, and symptomatic management. After stabilizing the patient’s condition, the patient underwent 3D-printed custom prosthesis-assisted partial replacement of the right humeral head and rotator cuff repair in the orthopedic department. Postoperatively, the patient’s right shoulder joint wound healed well, and mobility was restored. Conclusion: This case report demonstrates that the use of a 3D-printed custom prosthesis for the treatment of irregular humeral bone defects caused by specific injury mechanisms, especially cases involving shoulder joint “locking” dislocation and reverse Hill-Sachs injury, can achieve precise bone defect repair, minimize surgical trauma, and provide superior outcomes in terms of postoperative functional rehabilitation.

    Abstract: Emerging evidence indicates that air pollutants contribute to the development and progression of gastrointestinal diseases. However, there is scarce evidence of an association with appendicitis in mainland China. Methods In this study, Linfen city, one of the most polluted cities in mainland China, was selected as the study site to explore whether air pollutants could affect appendicitis admissions and to identify susceptible populations. Daily data on appendicitis admissions and three principal air pollutants, including inhalable particulate matter (PM 10 ), nitrogen dioxide (NO 2 ), and sulfur dioxide (SO 2 ) were collected in Linfen, China. The impacts of air pollutants on appendicitis were studied by using a generalized additive model (GAM) combined with the quasi-Poisson function. Stratified analyses were also performed by sex, age, and season. Results We observed a positive association between air pollution and appendicitis admissions. For a 10 μg/m 3 increase in pollutants at lag01, the corresponding relative risks (RRs) and 95% confidence intervals (95% CIs) were 1.0179 (1.0129–1.0230) for PM 10 , 1.0236 (1.0184–1.0288) for SO 2 , and 1.0979 (1.0704–1.1262) for NO 2 . Males and people aged 21–39 years were more susceptible to air pollutants. Regarding seasons, the effects seemed to be stronger during the cold season, but there was no statistically significant difference between the seasonal groups. Conclusions Our findings indicated that short-term air pollution exposure was significantly correlated with appendicitis admissions, and active air pollution interventions should be implemented to reduce appendicitis hospitalizations, especially for males and people aged 21–39 years.

    Abstract: Majority of patients with 2019 novel coronavirus infection (COVID-19) exhibit mild symptoms. Identification of COVID-19 patients with mild symptoms who might develop into severe or critical illness is essential to save lives. We conducted an observational study in a dedicated make-shift hospital for adult male COVID-19 patients with mild symptoms between February and March 2020. Baseline characteristics, medical history, and clinical presentation were recorded. Laboratory tests and chest computed tomography were performed. Patients were observed until they were either transferred to a hospital for advanced care owing to disease exacerbation or were discharged after improvement. Patients were grouped based on their chest imaging findings or short-term outcomes. A total of 125 COVID-19 patients with mild symptoms were enrolled. Of these, 7 patients were transferred for advanced care while 118 patients were discharged after improvement and showed no disease recurrence during an additional 28-day follow-up period. Eighty-five patients (68.0%) had abnormal chest imaging findings. Patients with abnormal chest imaging findings were more likely to have disease deterioration and require advanced care as compared to those with normal chest imaging findings. Patients with deteriorated outcomes were more likely to have low peripheral blood oxygen saturation and moderately-elevated body temperature. There were no significant differences between patients with deteriorated or improved outcomes with respect to age, comorbidities, or other clinical symptoms (including nasal congestion, sore throat, cough, hemoptysis, sputum production, shortness of breath, fatigue, headache, nausea or vomiting, diarrhea). Abnormal chest imaging findings, low peripheral blood oxygen saturation, and elevated temperature were associated with disease deterioration in adult male COVID-19 patients with mild clinical symptoms.     Abstract: The neural activity of the left stellate ganglion (LSG) is closely related to the occurrence of ventricular arrhythmias (VAs). Bmal1 modulates genes associated with neural activity in the central nervous system. However, few studies indicated the role of Bmal1 in the LSG and the subsequent effect on the heart. Therefore, we aimed to investigate the influence of Bmal1 knockdown in the LSG on its neural activity and cardiac electrophysiology and to explore the mechanisms. Materials and methods We used adeno-associated virus (AAV) to knock down Bmal1 in the LSG. Male beagles were randomized into the Bmal1 knockdown group and the control group. After 4 weeks of injection, the LSG function, neural activity, left ventricular effective refractory period (ERP), and action potential duration (APD) were measured. Electrocardiography for 1 h was recorded for VAs analysis after myocardial ischemia. Nerve growth factor (NGF) and c-fos in the LSG were quantified by immunofluorescence. Transcriptomic analysis was performed to assess the gene expression in the LSG. Results Bmal1 was sufficiently knocked down by AAV. Compared with the control group, heart rate variability (HRV) in the knockdown group was altered. Bmal1 knockdown inhibited neural activity and function of LSG. It also prolonged ERP as well as APD90. Ischemia-induced VAs were significantly reduced. Nerve growth factor (NGF) and c-fos in the LSG were reduced. Bmal1 knockdown led to the expression changes of genes associated with neural activity in the LSG. Conclusion Bmal1 knockdown in the LSG suppresses neural activity and prevents ventricular arrhythmias after myocardial ischemia.

    Abstract: Golden pompano ( Trachinotus blochii ) is an economically important fish which exhibits sexual size dimorphism and is widely cultivated in the southern seas of China. To evaluate the nutritional composition of T. blochii of different mariculture areas, growth stages, and genders, the moisture, ash, amino acids, and fatty acids in the muscle were measured using national standard biochemical assay. The analysis found 16 kinds of amino acids in the muscle of T. blochii . The EAA contents of fish from Guangdong (GD) and Guangxi (GX) were significantly lower than those of Hainan (HN) and Fujian (FJ) ( p   & lt; 0.05). The unsaturated fatty acids were higher in T. blochii cultured in HN and FJ ( p   & lt; 0.05). Within the same sea area, the contents of TAA, EAA, DAA, and PUFA increased with growth in T. blochii , but the differences were not significant ( p   & gt; 0.05). EAA/TAA and EAA/NEAA conformed to the ideal FAO/WHO model. The AAS, CS, and EAAI scores of amino acids within groups gradually increased with growth. The TAA, EAA and PUFA contents in females were higher than in males ( p   & gt; 0.05). The slightly higher amounts of amino acids and fatty acids in female T. blochii indicated females had higher nutritional value. In conclusion, the HN and FJ groups, the later growth stages, and the female T. blochii had generally higher nutritional values than their respective counterparts. These results provide fundamental data supporting all-female T. blochii breeding and culture, and optimized marketing body size.
