    Abstract: Malignant syphilis is a rare skin manifestation of secondary syphilis, which has been reported primarily in the HIV-infected population. This study aimed to investigate the relationship between HIV infection, malignant syphilis, and neurosyphilis through a systematic chart review of 26 malignant syphilis patients seen at our hospital. We also performed a literature review of 83 reported malignant syphilis cases since 1987, when the first case of malignant syphilis co-infected with HIV was reported. We found there was no direct association between HIV infection and malignant syphilis or neurosyphilis. In contrast, we found that much higher proportion (30%) of malignant syphilis patients develop concurrent neurosyphilis compared to those with the common type of secondary syphilis (13.1%). Physicians should be aware of the fact that neurosyphilis is more common among patients with malignant syphilis and the importance of investigating for neurosyphilis in such patients.

    Abstract: Solid pseudopapillary tumor of the pancreas (SPTP) is a rare neoplasm with a low incidence and low rate of malignancy. We herein report a rare case of SPTP concurrent with regional portal hypertension (RPH) that was successfully treated by distal pancreatectomy and splenectomy. A 22-year-old woman presented with a left upper abdominal apophysis and normal liver function. She was diagnosed with an SPTP and RPH by abdominal ultrasound and computed tomography, and she subsequently underwent distal pancreatectomy and splenectomy. Noticeably, varicose vein plexus with wide range appeared on the upper edge of the pancreatic body and posterior gastric wall of the patient. Therefore, we created a path to avoid touching the varicose veins and took advantage of the endoscopic linear stapler to staple the veins. We herein report our surgical experience on SPTP assisted with the endoscopic linear stapler, which will be very realistic for the management of this rare clinical entity.

    Abstract: Mirizzi syndrome is a rare complication of chronic cholecystitis, usually caused by gallstones impacted in the cystic duct or the neck of the gallbladder. Mirizzi syndrome results in compression of the hepatic duct or fistula formation between the gallbladder and common bile duct (or hepatic duct, right hepatic duct, or even mutative right posterior hepatic duct). Clinical features include abdominal pain, fever, and obstructive jaundice. Severe inflammation and adhesion at Calot’s triangle are potentially very dangerous for patients with Mirizzi syndrome undergoing cholecystectomy. Case presentation: We report the case of a 68-year-old Asian woman who presented with abdominal pain and jaundice. She had a medical history of gallstones, but no fever. Magnetic resonance cholangiopancreatography revealed cholecystitis, cholelithiasis, common hepatic duct stones, and ascites. Findings at surgery included a porcelainized, atrophic gallbladder that was full of gallstones, fistula formation between the gallbladder and common hepatic duct, and left hepatic atrophy. The prominent feature was the left hepatic atrophy, but stones were not visible pre-operatively in the left liver by radiologic examination. Conclusions This patient exhibited what can be considered a special type II of Mirizzi syndrome with a fistula of the common hepatic duct as well as left hepatic atrophy.

    Abstract: The control strategy is established with position control as the inner loop and admittance control the outer loop to solve the smooth contact force problem in faucet polishing operation. To avoid the excessive polishing, the parameter sensitivity is also introduced. Firstly, the system model and the controller are presented, the sensitivity of the parameter is calculated, and the effect of the admittance parameters on the output of the system is analyzed. Secondly, through the simulation of position control and admittance control, the change of the contact force between the robot end actuator and the external environment is analyzed, which shows that the proposed control strategy improves the accuracy of position control by 3.0% and guarantees that the contact force between the manipulator and the environment is close to the expected value. Finally, the polishing experiment is carried out on an industrial robot. The experimental results show that the control strategy can well control the contact force in the polishing process of the faucet.

    Abstract: Intermedin (IMD) is a member of calcitonin/calcitonin gene-related peptide (CGRP) and involves in the regulation of cardiovascular function in both peripheral tissues and central nervous system (CNS). Paraventricular nucleus (PVN) of hypothalamus is an important site in the control of cardiac sympathetic afferent reflex (CSAR) which participates in sympathetic over-excitation of hypertension. The aim of this study is to investigate whether IMD in the PVN is involved in the inhibition of CSAR and its related mechanism in hypertension. Rats were subjected to two-kidney one-clip (2K1C) surgery to induce renovascular hypertension or sham-operation (Sham). Acute experiments were carried out four weeks later under anesthesia. The CSAR was evaluated with the renal sympathetic nerve activity (RSNA) and mean arterial pressure (MAP) responses to the epicardial application of capsaicin. The RSNA and MAP were recorded in sinoaortic-denervated, cervical-vagotomized and anesthetized rats. Bilateral PVN microinjection of IMD (25 pmol) caused greater decrease in the CSAR in 2K1C rats than in Sham rats, which was prevented by pretreatment with adrenomedullin (AM) receptor antagonist AM22-52, non-selective nitric oxide (NO) synthase (NOS) inhibitor l-NAME or γ-amino butyric acid (GABA) B receptor blocker CGP-35348. PVN pretreatment with CGRP receptor antagonist CGRP8-37 or GABA A receptor blocker gabazine had no significant effect on the CSAR response to IMD. AM22-52, l-NAME and CGP-35348 in the PVN could increase CSAR in Sham and 2K1C rats. These data indicate that IMD in the PVN inhibits CSAR via AM receptor, and both NO and GABA in the PVN involve in the effect of IMD on CSAR in Sham and renovascular hypertensive rats.

    Abstract: To reduce the demand on new asphalt and aggregates, more attention has been paid to increasing reclaimed asphalt pavement (RAP) contents used in asphalt paving mixtures. However, Chinese transportation departments are reluctant to use high RAP contents in practice, partially because of the performance of high RAP mixtures, its possible effectiveness, and potential rutting problems caused by asphalt rejuvenators. An investigation was done of the effect of styrene-butadiene rubber (SBR) latex and a warm-mix asphalt (WMA) agent on RAP mixes. The objective was to produce mixtures with high RAP content that demonstrate similar or better pavement performance than the conventional hot-mix asphalt. In this study, different laboratory tests were conducted on high RAP mixtures with a rejuvenator, SBR latex, and a WMA agent. The results illustrated that the asphalt rejuvenator partly recovers the RAP binder performance and has a negative effect on the rutting performance of mixtures, while SBR latex can improve both high-temperature and low-temperature performance with the rejuvenator. Using the WMA agent can satisfy the specifications and production tolerances of volumetric properties for high RAP mixes. Finally, it is found that the mixture with 50% RAP and SBR latex can supply a better fatigue performance, low-temperature performance, and moisture-resistant performance than the conventional hot-mix asphalt mixture.

    Abstract: Objective: To observe the expression of osteopontin (OPN) in hepatocytes of rats fed with corn baked by burning coal from fluorosis areas and a deficiency of calcium/protein intake following fluorosis. Methods: A total of 48 Wistar rats as objects were randomly assorted into four groups: dose-free fluorine group, which were mainly fed with fluorine-free corn (56% structurally), dose-free fluorine with biased dietary group, which were fed with lower contents of protein (119.41 g/kg) and calcium (0.68 g/kg), high-dose fluorine group (fluorine contents: 104.2 mg/kg), and high-dose fluorine with biased dietary group. After 180 days of cultivation, the contents of fluorine in the bones of rats were tested for the assessment of construction of fluorosis animal model. And the expression of OPN in hepatocytes of rats in different groups was detected with immunohistochemistry and reverse transcription polymerase chain reaction. Results: The present study validated the result that OPN was overexpressed in hepatocytes following fluorosis after oral intake of burning coal-baked corn. OPN was expressed most significantly in high fluorine with biased dietary group, and the high-fluorine group ranked the second most; and dose-free fluorine with biased dietary group ranked the third. The dose-free fluorine group expressed the least OPN. Conclusion: Overexpression of OPN in hepatocytes following fluorosis after excess fluorine intake was involved in liver damage process, which was enhanced by deficiency of calcium and protein intake. The results also demonstrated that the development of fluorosis in Guizhou province was correlated with local baking staple corn as a way of excess intake of fluorine and deficiency of calcium/protein intake.

    Abstract: To investigate the correlation between ApoE gene polymorphism and the risk of lower extremity arterial disease (LEAD) in T2DM patients of Han nationality in northern China. One hundred and fifty patients with T2DM and 150 patients with T2DM combined with LEAD were enrolled in the study and assigned to the control group and the case group, respectively. The results revealed that the number of epsilon 2 (ε2) genotype carriers in the case group was significantly lower than that in the control group (case vs control: 8.0% vs 15.4%), and the distribution of the ε2 allele frequency was similar to that of the genotype (case vs control: 4.0% vs 8.4%). A binary logistic regression analysis revealed that age, waist-to-hip ratio (WHR), and gender were risk factors for T2DM with LEAD and that the ApoE ε2 carriers was a protective factor (odds ratio [OR] 0.380; 95% confidence interval [CI] 0.162-0.892; P = .026). In male subjects, ε2 carriers were more common in the control group (case vs control:7.8% vs 24.1%), while epsilon 4 (ε4) carriers were more common in the case group (case vs control: 23.3% vs 8.6%); the distribution of frequency of ε2 and ε4 alleles was more common in the control group and the case group, respectively (case vs control: 3.9% vs 12.1%, 12.8% vs 5.2%). In male, age and WHR were risk factors for the disease, and being an ε2 carrier was a protective factor. Being an ApoE gene ε2 carrier is a protective factor for LEAD in male patients with T2DM that are of Han nationality in northern China.

    Abstract: The goal of this study was to assess the pathological and differential diagnoses of sinonasal teratocarcinosarcoma (SNTCS) in order to ultimately improve the diagnosis and treatment of this rare disease. Data from 2 cases of sinonasal teratocarcinosarcoma from the Wuxi People’s Hospital (China) were analyzed. The clinical presentation for these patients consisted of nasal obstruction, epistaxis, and headache. On further physical examination, the presence of a polypoid mass was identified and, despite surgery and radiotherapy, both cases experienced recurrence. Histologically, the tumors showed a heterogeneous mixture of components from the 3 germ layers, primitive neuroepithelial elements, diagnostic immature squamous cell nests (clear cell nests), and various epithelial and mesenchymal components. Staining of the different germ layers corresponded with the appropriate immune markers. In case 1, the postradiotherapy resection specimen was completely dominated by a mature teratoma, with a complete absence of the corresponding adenocarcinoma and fibrosarcoma components. To date, this is the first study describing this composition within an SNTCS recurrent tumor. In summary, SNTCS is a rare tumor characterized by the presence of benign and malignant epithelial, mesenchymal, and dysembryomal components. Owing to its heterogeneous histologic appearance, adequate sampling and recognition of all SNTCS components are needed for future diagnosis. Currently, surgical removal, postoperative radiotherapy, and a histology-specific multidrug chemotherapy appear to be the best therapeutic approach. Future individualized therapy may also hold promise.
