    Abstract: The aim of this study was to determine levels of awareness of basic knowledge concerning control and prevention of chronic obstructive pulmonary disease (COPD) among general practitioners (GPs) in a Shanghai suburb and to evaluate adherence to GOLD (COPD) guidelines in primary care. All 593 practitioners, including rural and urban community GPs at 15 primary care centers in the Songjiang district of Shanghai, were questioned, comprising 331 from urban communities and 262 from rural. Only 115 GPs (19.4%) understood that COPD is categorized into acute exacerbation and stable stages, and 328 GPs (55.3%) recognized that treatment for COPD patients was still required during stable stages. A total of 235 GPs (39.6%) knew that oxygen therapy should be administered for 〉 15 h/d as regular home therapy. In 97 cases of stage III and IV COPD, only 1 patient had accepted long-term home oxygen therapy. Prescriptions that conformed with the GOLD recommendations occurred in approximately 8% of patients according to GOLD COPD severity staging. Basic knowledge of prevention and treatment of COPD was seriously lacking in GPs. Additional training is clearly needed.

    Abstract: Irritable bowel syndrome (IBS) related chronic visceral pain affects 20% of people worldwide. The treatment options are very limited. Although the scholarly reviews have appraised the potential effects of the intestinal microbiota on intestinal motility and sensation, the exact mechanism of intestinal microbiota in IBS-like chronic visceral pain remains largely unclear. The purpose of this study is to investigate whether Folic Acid (FA) attenuated visceral pain and its possible mechanisms. Chronic visceral hyperalgesia was induced in rats by neonatal colonic inflammation (NCI). 16S rDNA analysis of fecal samples from human subjects and rats was performed. Patch clamp recording was used to determine synaptic transmission of colonic-related spinal dorsal horn. Alpha diversity of intestinal flora was increased in patients with IBS, as well as the obviously increased abundance of Clostridiales order (a main bacteria producing hydrogen sulfide). The hydrogen sulfide content was positive correlation with visceral pain score in patients with IBS. Consistently, NCI increased Clostridiales frequency and hydrogen sulfide content in feces of adult rats. Notably, the concentration of FA was markedly decreased in peripheral blood of IBS patients compared with non-IBS human subjects. FA supplement alleviated chronic visceral pain and normalized the Clostridiales frequency in NCI rats. In addition, FA supplement significantly reduced the frequency of sEPSCs of neurons in the spinal dorsal horn of NCI rats. Folic Acid treatment attenuated chronic visceral pain of NCI rats through reducing hydrogen sulfide production from Clostridiales in intestine.

    Abstract: The recurrence rate of ischemic symptoms after coronary artery bypass grafting (CABG) is increasing in recent years. How to prevent and treat saphenous vein graft disease (SVGD [symptomatic ⩾50% stenosis in at least one Saphenous vein graft]) has been a clinical challenge to date. Different pathogenesis may exist in SVGD of different periods. There are currently few available scores for estimating the risk of SVGD after one year post CABG. Objective: We sought to develop and validate a simple predictive clinical risk score for SVGD with recurring ischemia after one year post CABG. Methods and Results: This was a cross-sectional study and the results were validated using bootstrap resampling on a separate cohort. A nomogram and risk scoring system were developed based on retrospective data from a training cohort of 606 consecutive patients with recurring ischemia 〉 1 year after CABG. Logistic regression model was used to find the predictive factors and to build a nomogram. To assess the generalization, models were validated using bootstrap resampling and an external cross-sectional study of 187 consecutive patients in four other hospitals. In multivariable analysis of the primary cohort, native lesion vessel number, SVG age, recurring ischemia type, very low-density lipoprotein level, and left ventricular end-diastolic diameter were independent predictors. A summary risk score was derived from nomogram, with a cut-off value of 15. In internal and external validation, the C-index was 0.86 and 0.82, indicating good discrimination. The calibration curve for probability of SVGD showed optimal agreement between actual observations and risk score prediction. Conclusion: A simple-to-use risk scoring system based on five easily variables was developed and validated to predict the risk of SVGD among patients who recurring ischemia after one year post CABG. This score may be useful for providing patients with individualized estimates of SVGD risk.

    Abstract: Recent literature has pointed out the seriously unmet treatment needs of delinquent youth with mental health problems in Hong Kong. This study pioneers to systematically develop and evaluate the effectiveness of a structured low-intensity cognitive behavioral intervention (LICBI) to concurrently address mental health issues and delinquent behaviors among Hong Kong Chinese youth. Method: A quasi-experimental controlled design was adopted with three assessments at baseline, postintervention, and 6-week follow-up. One hundred and eighty participants were recruited to the LICBI and 123 to the control group. Results: Multilevel modeling analyses showed that the LICBI might be effective in improving mental health–related factors (impulsivity, self-esteem, negative emotions, and loneliness) and delinquent behaviors. A mediation analysis suggested that LICBI might reduce participants’ delinquent behaviors through reducing their impulsivity and enhancing their self-esteem. Discussion: The results provide preliminary evidence for the effectiveness of LICBI for Chinese delinquent youth in a community social service setting.

    Abstract: In order to promote sustainable economic development in the areas along the Belt and Road in China, it is of great necessity to reduce the negative impact of air pollutants resulting from industrialization and urbanization on the complex and fragile ecological environments of neighboring areas. First, this study estimated the total-factor air environmental efficiency (TFAEE) of 17 provinces along the Belt and Road in China from 2010 to 2017 using a slacks-based measure (SBM) model. Second, the global and local Moran indices were used to test the spatial correlations between TFAEEs. Finally, the spatial factors and spatial spillover effects influencing the TFAEEs were investigated using the spatial Durbin model with spatiotemporal double fixed effects. The results were shown as follows: (1) The total-factor TFAEEs of the areas along the Belt and Road were low and showed significant regional spatial differences during 2010–2017. (2) There was a positive spatial autocorrelation between the TFAEEs of the areas along the Belt and Road, and the spatial distribution generally clustered into High-High and Low-Low concentrations. (3) Economic development and technological innovation played significantly positive effects on TFAEEs of the areas in the Belt and Road, while energy consumption structure had negative effect on it. In addition, although industrial structure and environmental regulation were negatively correlated with TFAEEs, the coefficients were not significant. (4) The positive spatial spillover effect of the TFAEEs of the areas along the Belt and Road was mainly the result of significant environmental regulations and insignificant economic development factors, while the technological innovations, energy consumption structures and industrial structures showed insignificant negative spatial spillover effects.

    Abstract: The partial fiber breaking in the warp yarn of three-dimensional (3D) woven fabrics needs to be urgently reduced considering the large shedding displacement and repeated yarn frictions in the fabricating process. A kinematical model of the heald frame was established in order to analyze the impact and wear of the warp yarn in the shedding process of 3D weaving. The variation tendency of the friction force between the yarn and heddle eye was obtained and the variable acceleration trajectory of the heald frame based on minimizing the friction and impact load on the yarn was derived. The experimental results showed that the fiber breakage rate in one warp yarn using the derived curve trajectory reduced drastically. The research results are beneficial to the theoretical design of the shedding system of weaving machines for 3D woven fabric.

    Abstract: Total/ionized calcium ratio has been used as an indirect index of increased plasma citrate concentration during citrate anticoagulation. However, they were obtained without any adjustment of total calcium for albumin. Herein, we investigated whether the albumin-corrected total/ionized calcium ratio performed better than total/ionized calcium ratio. Methods 20 critically ill patients were treated with continuous venovenous hemofiltration (CVVH) using citrate anticoagulation. The systemic total, ionized calcium and albumin levels were dynamically measured, and correlations between different calcium parameters and plasma citrate levels were analyzed. Results The highest correlation was found between plasma citrate level and total/ionized calcium ratio at the third hour (R = 0.88, p 〈 0.001). The albumin-corrected total/ionized calcium ratio was not superior to total/ionized calcium ratio. Conclusions The total/ionized calcium ratio at the third hour after the start of citrate-CVVH is an early and simple indicator to reflect increased plasma citrate level. Adjustment of systemic total calcium for serum albumin is not necessary.

    Abstract: Here, we report a case of coexistence of a duplicate gallbladder and choledochal cyst, which was overlooked preoperatively and confirmed via intraoperative inspection and pathological examination. We concluded that a high index of suspicion is required to diagnose a double gallbladder, and special attention should be paid to preoperative radiological imaging when treating patients with choledochal cysts because these patients present a higher incidence of biliary anomalies.

    Abstract: To optimize a protocol for the steroid administration approach for idiopathic sudden sensorineural hearing loss (ISSNHL) in China. Methods: A questionnaire was distributed to otolaryngologists. The data on demographics, indications for first-line and salvage treatment, such as intratympanic administration of steroids (ITS) and postauricular steroids (PAS), and procedures were analyzed. Results: 74 respondents used oral steroids, 112 used intravenous injections, 10 used ITS and 6 used PAS as a monotherapy for first-line treatment, and 135 used ITS or PAS in conjunction with oral or intravenous injection as a first-line treatment. Of the 249 respondents who used ITS, 97.19% adopted it as salvage therapy. The most commonly used steroid was 0.5 ml dexamethasone at 5 mg/ml and the most common side effect was pain. Of the 174 respondents who used PAS, 94.25% used it as salvage therapy. The most commonly used steroid was 0.5 ml methylprednisolone mixed with 0.5 ml lidocaine. Conclusions: The obtained data suggested that intravenous injection of steroids was the most popular treatment for ISSNHL and that ITS or PAS were used as a salvage treatment in China.

    Abstract: In this paper, we propose a two-layer overtaking control framework of the autonomous electric ground vehicle, including trajectory planning and tracking control. In the upper layer, a stable overtaking trajectory is derived by considering the duration of the overtaking, driving comfort and vehicle stability simultaneously. Besides, the safety spacing between the overtaking and overtaken vehicles is analyzed to prevent the potential collision. To track the generated reference trajectory precisely, we design a robust model predictive controller adopting the input-to-state stability property in the lower layer to reduce the affection of disturbances and parameter uncertainties. Two driving strategies with different lateral accelerations, that is, mild and aggressive, are simulated to verify the effectiveness of the proposed control framework. By comparing to the driver-vehicle control system, the proposed control framework can not only achieve safe, smooth, and rapid overtaking but also realize the accurate state tracking in the presence of tire cornering stiffness uncertainty.
