    Abstract: Over-reliance on short-acting β 2 -agonists (SABAs) is associated with poor asthma outcomes. However, the extent of SABA use in Turkey is unclear owing to a lack of comprehensive healthcare databases. Here, we describe the demographics, disease characteristics and treatment patterns from the Turkish cohort of the SABA use IN Asthma (SABINA) III study. Methods This observational, cross-sectional study included patients aged ≥ 12 years with asthma from 24 centres across Turkey. Data on sociodemographics, disease characteristics and asthma treatments were collected using electronic case report forms. Patients were classified by investigator-defined asthma severity (guided by the 2017 Global Initiative for Asthma [GINA]) and practice type (primary/specialist care). The primary objective was to describe SABA prescription patterns in the 12 months prior to the study visit. Results Overall, 579 patients were included (mean age [standard deviation; SD]: 47.4 [16.1] years; 74.3% female), all of whom were treated by specialists. Most patients had moderate-to-severe asthma (82.7%, GINA steps 3–5), were overweight or obese (70.5%), had high school or university/post-graduate education (51.8%) and reported fully reimbursed healthcare (97.1%). The mean (SD) asthma duration was 12.0 (9.9) years. Asthma was partly controlled/uncontrolled in 56.3% of patients, and 46.5% experienced ≥ 1 severe exacerbation in the preceding 12 months. Overall, 23.9% of patients were prescribed ≥ 3 SABA canisters in the previous 12 months (considered over-prescription); 42.9% received no SABA prescriptions. As few patients had mild asthma, only 5.7% were prescribed SABA monotherapy. Therefore, most patients (61.5%) were prescribed SABA in addition to maintenance therapy, with 42.8% receiving ≥ 3 SABA canisters in the previous 12 months. Inhaled corticosteroids (ICS), ICS + a long-acting β-agonist fixed-dose combination and oral corticosteroids were prescribed to 14.5%, 88.3% and 28.5% of all patients, respectively. Additionally, 10.2% of patients purchased SABA over the counter, of whom 27.1% purchased ≥ 3 canisters in the preceding 12 months. Conclusions Despite all patients being treated by specialists and most receiving fully reimbursed healthcare, nearly a quarter of patients received prescriptions for ≥ 3 SABA canisters in the previous 12 months. This highlights a public health concern and emphasizes the need to align clinical practices with the latest evidence-based recommendations.

    Abstract: There is a direct relationship between foreign policy crisis and decision-making. In the decision making-process, access to information is critically important. Although information can be provided by various methods, it is common to obtain it from intelligence sources. In this respect, there is a strong connection between crisis management and intelligence, depending on the nature of the crisis. In fact, the function of intelligence is not only to obtain or produce information. Protecting information from enemies’ access (counter-intelligence), distort-impose knowledge (propaganda), to persuade their target audience by using it in combination with fear (psychological warfare), to carry out actions that will alter the conflict/contention environment in favour of the friendly sides with a secret hand (covert operations) are activities carried out by intelligence agencies. From this point of view, in foreign policy crises, intelligence appears to be a more active actor than previously thought. Examining intelligence functions through the perspective of a sample crisis is critical to understanding the relationship between crisis and intelligence. The role of intelligence functions in the Sanjak Crisis will be examined in this study to reveal the contribution of intelligence to crisis management.

    Abstract: 1936’da Fransa’nın Suriye’deki manda yönetimini bırakma kararıyla 1921 Ankara Antlaşması’nın ve dolayısıyla İskenderun Sancağı’nın durumu tartışmalı hale gelmiştir. Türkiye bu durumda Sancak’ın, Suriye’ye bırakılmasını uygun bulmamış ve Fransa’dan özerk bir yönetimde olmasını talep etmiştir. Bu talebin tetiklediği Sancak Krizi’nde Türkiye’nin askeri kuvvet kullanma tehdidiyle desteklediği diplomatik girişimleri, Milletler Cemiyeti’nin kararları ve İngiltere’nin arabuluculuğuyla beraber Sancak önce bağımsızlığını kazanmış ardından Türkiye’ye ilhak olmuştur. Sancak Krizi’nin yönetim süreci, İkinci Dünya Savaşı’na doğru ilerleyen uluslararası sistemdeki değişimden ve Cumhurbaşkanı Mustafa Kemal Atatürk’ün kriz yönetimindeki baskın lider profilinden etkilenmiştir. Karar birimi olarak lider, kriz yönetim sürecinde Fransa ile uluslararası zeminde müzakere ederken ulusal zeminde de TBMM’yle doğrudan iletişim içinde olmuştur. Baskın lider profilinin kriz yönetiminde ön planda olmasına rağmen, yasama organı olarak TBMM kimi zaman yürütmenin sorumlu olduğu bir üst denetim mekanizması kimi zaman izlenen siyasanın dillendirildiği etkin bir araç olmuştur. Böylece yasama organı kriz yönetim sürecine dâhil olmuştur. Krizin barışçıl çözüm sürecinde ve liderin kararlaştırma sürecinde; rakibin ikna olması için kırmızı çizginin açık ifadesi, kararın mutabık olunmuş ulusal çıkara dayandırılması, kriz yönetiminde bazı stratejik hamleler ve anayasal zorunluluk sebepleri ile yasama organı olarak TBMM krizde özel bir işleve sahiptir. TBMM’nin krizdeki etkinliği, Türkiye’nin kriz yönetim sürecindeki başarısını olumlu yönde etkileyerek destekleyici unsur olmuştur. Bu çalışma iki seviyeli oyun örneği olarak Sancak Krizi’nde; yasama organının karar alıcı ile ilişkisinin sebeplerini, niteliğini ve bu etkileşimin Türkiye’nin kriz yönetim ve karar alma sürecine etkisi boyutuyla sonuçlarını analiz edecektir.

    Abstract: Introduction: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare disease which is characterized with complement mediated intravascular hemolysis, bone marrow failure and thrombosis. The prevelance of PNH is estimated 1 to 16 cases per million in USA. X-linked somatic mutation of phosphatidylinositol glycan‐A (PIGA) gene in hematopoetic stem cells causes an impairment of glycosylphosphatidylinositol (GPI) anchored proteins. The absence of GPI‐dependent molecules that are CD55 (decay accelerating factor; DAF) and CD59 (membrane inhibitor of reactive lysis; MIRL), normally protect the cell from complement‐mediated hemolysis by preventing the formation of the membrane attack complex. The diagnosis is based on flow cytometry which can detect the deficiency of these two complement regulary proteins. We report a retrospective analysis of demographic and clinical characteristics of PNH patients from different centers. Material and methods: We conducted a retrospective analysis of the patients' recorded data. Patients' demographics, medical and treatment history, comorbid conditions, PNH clone size, disease characteristics and outcomes, symptoms, PNH-specific treatments, PNH-related events, morbidity (including myeloproliferative disease, other malignancies, and infections), mortality. Clinical data captured include lactate dehydrogenase (LDH) levels, PNH clone size, hemoglobin levels, thrombotic events, renal functional tests at the time of diagnosis, and other laboratory data. Specific information collected for eculizumab-treated patients includes dosage and dose adjustments and blood cell counts, reticulocyte count and LDH level after eculizumab treatment. Results: 138 patients were included from 28 different centers. All patients were diagnosed by flow cytometry for GPI-linked antigens on red cells and neutrophils. The number of male (69/138) and female (69/138) patient was equal and the median age was 41 years. Median hemoglobin (hb) level was 8.75±2.13 gr/dL; Platelet (plt) level was 131× 109/L at the time of diagnosis. Overall, 49(35,5%) of the patients had been diagnosed with bone marrow failure, including aplastic anemia or hypoplastic anemia (n=31; 22,5%), myelodysplastic syndromes (n=18; 13%). A history of any prior thrombotic event was reported in 45 patients (32,6%). At the time of analysis, 12 (8,7%) patients had pulmonary hypertension. The median granulocyte and monocyte clone size was 63,6% (±32.26) and 66.76 ±28.75 respectively. Fatigue (58%) is the most commonly reported symptom and abdominal pain was seen in 8% of patients. After the eculizumab therapy, the median time for normalization of Hb and LDH level were 7 and 14,6 months, respectively. There was no correlation between thrombosis and clone size, hb, plt, LDH level at the time of diagnosis. LDH level was higher in fatigue patients compared with the patients who were not fatigue (p=0.021). Discussion: PNH is a clonal but non malignant disease that is very rare and knowledge on large case series is really limited. The clinical findings and symptoms could be variable and unfortunately it takes very long time to diagnose because of unawareness of physicians. Eculizumab is a good option in treatment of PNH for improving symptoms of intravascular hemolysis but we still need better understanding of thrombosis mechanism of PNH to better management. In the future, novel inhibitors of the alternative pathway of complement will be used to improve survival and quality of life for PNH patients.     Abstract: Objectives. To further define the clinical spectrum of the disease for pediatric and metabolic specialists, and to suggest that the general pediatrician and pediatric neurologist consider succinic semialdehyde dehydrogenase (SSADH) deficiency in the differential diagnosis of patients with (idiopathic) mental retardation and emphasize the need for accurate, quantitative organic acid analysis in such patients. Patients. The clinical features of 23 patients (20 families) with SSADH deficiency (4-hydroxybutyric aciduria) are presented. The age at diagnosis ranged from 3 months to 25 years in the 11 male and 12 female patients; consanguinity was noted in 39% of families. Outcome Measurements. The following abnormalities were observed (frequency in 23 patients): motor delay, including fine-motor skills, 78%; language delay, 78%; hypotonia, 74%; mental delay, 74%; seizures, 48%; decreased or absent reflexes, 39%; ataxia, 30%; behavioral problems, 30%; hyperkinesis, 30%; neonatal problems, 26%; and electroencephalographic abnormalities, 26%. Associated findings included psychoses, cranial magnetic resonance or computed tomographic abnormalities, and ocular problems in 22% or less of patients. Therapy with vigabatrin proved beneficial to varying degrees in 35% of the patients. Normal early development was noted in 30% of patients. Conclusions. Our data imply that two groups of patients with SSADH deficiency exist, differentiated by the course of early development. Our recommendation would be that accurate, quantitative organic acid analysis in an appropriate specialist laboratory be requested for any patients presenting with two or more features of mental, motor, or language delay and hypotonia of unknown cause. Such analyses are the only definitive way to diagnose SSADH deficiency; the diagnosis can be confirmed by determination of enzyme activity in white cells from whole blood. We think that increased use of organic acid determination will lead to increased diagnosis of SSADH deficiency and a more accurate representation of disease frequency. As additional patients are identified, we should have a better understanding of both the metabolic and clinical profiles of SSADH deficiency.
