The history of the International Professional Network dedicated to Predictive, Preventive and Personalised Medicine (PPPM / 3PM) is rich in achievements. Facing the coronavirus COVID-19 pandemic it is getting evident globally that the predictive approach, targeted prevention and personalisation of medical services is the optimal paradigm in healthcare demonstrating the high potential to save lives and to benefit the society as a whole. The EPMA World Congress Supplement 2020 highlights advances in 3P medicine.

    Abstract: Purpose. To analyze the cases of ophthalmic hypertension in patients after vitreoretinal surgery with various tamponades of the vitreal cavity.  Material and methods. We analyzed 344 case histories of patients aged 30 to 80 who underwent vitreoretinal intervention with various tamponades of the vitreal cavity for macular rupture, epiretinal membrane, hemophthalmia, retinal detachment in 2019–2021. To detect ophthalmic hypertension, all patients underwent tonometry before surgery and every month after it during the entire 2-year follow-up period. Results. Of the 46 patients who were diagnosed with ophthalmic hypertension after vitreoretinal surgery, 18 patients achieved intraocular pressure compensation within the 2 year follow-up and they required no hypotensive therapy. 28 patients, including 4 patients who had glaucoma before vitreoretinal intervention were prescribed hypotensive therapy (carbonic anhydrase inhibitors, 2 adrenomimetics), 2 patients received an implantation of an Express anti-glaucomatous device. Conclusions. The analysis of ophthalmic hypertension in patients after vitreoretinal surgery showed that post-surgical IOP increase occurs both in cases of applying tamponade with silicon oil and those using sterile air or gasair mixture. Intraocular pressure compensation follows different patterns in every case, so that an individual approach is needed. Using one antihypertensive drug may be insufficient so in some cases a stronger antihypertensive therapy is required and sometimes anti-glaucomatous surgical interventions have to be resorted to.

    Abstract: The article provides a review of the scientific literature devoted to postcoid conditions. The concept of postcovid syndrome, its structure, and epidemiology are disclosed. The article contains the results of 2 meetings of experts (psychiatrists, psychotherapists, neurologists) from different regions of Russia who had experience in providing care to persons with postkovid syndrome. Particular attention is paid to postcovid depression, its clinical characteristics, and the experience of therapy. It was noted that the severity of the post-coV syndrome did not depend on the severity of the infectious disease suffered. The leading variants of depressive syndromes observed in SARS-CoV-2 patients were: astheno-depressive, apato-depressive and anxious-depressive syndromes. Anhedonia, anxiety, cognitive disorders, insomnia, and hypochondria occupied a significant place in the structure of postcovid mental disorders. It is emphasized that persons with postcovid syndrome, as a rule, initially found themselves in the field of vision of internists, therefore, their training in the diagnosis of postcovid mental disorders is relevant. Clinical experience has demonstrated the feasibility of antidepressant therapy for postcovid depression and the high efficacy and safety, including the risk of drug interactions, of using vortioxetine, including prescribing the drug to the elderly and the somatically impaired. The article is illustrated with clinical examples.
