    Abstract: Viral encephalitis remains a significant public health problem world wide including Bangladesh.Its transmission is either from insect or environmental vectors or from human-to-humancontact. Cerebrospinal fluid analysis, viral studies, and various imaging techniques providesensitive and specific methods to detect many of these viral pathogens. Rapid diagnosis ofencephalitis allows early intervention in those conditions that are responsive to specificantiviral treatment, such as herpes simplex encephalitis. Specific treatment is available forcertain viral encephalitides, but for others only supportive therapy is available. Vaccines areavailable to prevent a few important causes of encephalitis, but prevention of encephalitisrevolves around minimizing exposure to pathogens, and insects which transmit many of them.Control must be based on rapid recognition of early cases, subsequent immunization ofpersons or animals at risk, or immunization of persons or animals with the potential to be atrisk, such as travelers, laboratory personnel, and attending clinicians.doi: 10.3329/taj.v16i1.3902TAJ June 2003; Vol.16(1): 32-38

    Abstract: An efficient protocol for in vitro propagation of Commelina benghalensis was developed. Nodal segments were showed the superb explants in performance for shoot proliferation than other explants. On the other hand, BAP with auxin was better combination. The maximum (90.52) number of explants response and the highest (23.25) number of shoots per plant were obtained from nodal segments on MS medium fortified with 3.0 mg/l BAP+0.1 mg/l IBA. The highest shoot length (17.25 cm) was achieved on MS medium containing 3.0mg/l BAP+0.5mg/l IBA. In vitro proliferated shoots were transferred to full and half strength of MS media where 1.5 mg/l IBA on full strength of MS media was the best to fit for the maximum number (12.69) of roots formation per micro-shoot. Well rooted plantlets were transferred to soil and successfully acclimatized with 97% survival rate. Three extracts i.e. methanol, ethanol, Petroleum ether of C. benghalensis L and four concentrations of each extracts were used against five gram (+ve) and five gram (-ve) bacterial species for the screening of antibacterial activity. Ethanol extracts was the superior in performance. The susceptibility of tested pathogenic bacterial species was increasing compare with increasing of extracts concentration with few exceptions. The highest zone of inhibition was obtained against S. aureus (17.50 mm) and P. aeruginosa (17.44 mm) at 800 mg/l dose level of extracts. It was also noticed that Gram (+ve) bacterial species are more susceptible to Benghal dayflower crude extracts than Gram (-ve) bacterial species.

    Abstract: Endoscopic findings help the clinical to give the treatment properly The purpose of the present study was to find out common findings of UGI endoscopy at a tertiary care hospital in Dhaka. This retrospective study was conducted in the Department of Gastroenterology at Holy Family Red Crescent Hospital, Dhaka from 14th October 2009 to 25th June 2013 among all the patients presented with GI symptoms. Endoscopies were documented on a computer-based datasheet. Under topical lidocaine, a Fujinon EG fiber optic Upper GI scope was passed through the mouth of a patient in left lateral position through the upper esophageal sphincter into the esophagus stomach and duodenum. Biopsies were collected and histopathology reports were recorded A total number of 2632 patients were recruited for this study and endoscopy was done of which 1406(53.4%) cases were reported as abnormal findings. Male (52.1%) was predominant than female (47.9%). Maximum patients were diagnosed as peptic ulcer disease (54.2%) followed by varices with or without gastropathy (20.04, gastric cancer (11.5%), esophageal cancer (9.6%) and gastritis with or without duodenitis which were 267cases, 154cases, 128cases and 63(4.7%) cases respectively. The most common cause of UGI bleeding was due to PUD (43.1%) followed by varices (34.7%), Gastric cancer (12.5%). The most common endoscopic findings are the PUD, varices with or without gastropathy, gastric cancer, esophageal cancer and gastritis with or without duodenitis.

    Abstract: A prospective randomized trial was performed to evaluate the combination of neoadjuvant chemotherapy in patient with oral squamous cell carcinoma. Prognosis of patient with squamous cell carcinoma in oral cavity is worth improving. Chemotherapy has been reported specially active in oral cavity tumour. It has been enrolling patients with stage T2, T3, T4 NO NI N2 MO untreated squamous cell carcinoma in oral cavity. Number of patients in this study was 25, suffering from squamous cell carcinoma. This study was a Prospective study conducted among the randomly selected 25 cases of histopathologically diagnosed Squamous cell carcinoma patients from the Department of Oral and Maxillofacial surgery Bangabandhu Sheikh Mujib Medical University and Department of Oncology Bangabandhu Shiekh Mujib Medical university . Both male and female were included and assigned for three cycles of cisplatin and 5fluorouracil followed by definitive treatment. The study was done between January 2007 to December 2008. 25 patients were included in this study, 21.7% patients showed complete response and 84.2% showed partial response. Out of 25, 20.0% patients mouth opening were reduced and 4.0% patients mouth opening were improved. After taking computed tomography all patient showed tumour progression. Histopathologically only 1 patient showed complete response among 25 which is not signcant. Neoadjuvant chemotherapy reduced the size of the lesion clinically. Results down staging of the tumour but histologically it was not given any significant changes. So probably neoadjuvant chemotherapy is unable to fulfill our desire expectation.

    Abstract: Hypertension is a very common chronic disease world wide, and stroke is one of it's complications if not treated properly. In Bangladesh, women have traditionally tended to overlook their own disease states when a male member of the family is already afflicted. The cross sectional observational study was carried out to explore the knowledge, attitude and perception hypertensive females have towards stroke between July 2012 and December 2012 at HFRCMCH. Admitted and outdoor female hypertensive patients were randomly selected; with their consent they were asked to respond to a prestructured questionnaire. We found that 52% of our cohort identified the brain as being the organ involved in stroke; however 35% still believed it to be a disease of the heart. Around 80% of the patients did understand the complications and impact stroke would have on their daily lives; however, there were some conceptual misunderstandings with regards to the risk factors and warning signs. Hypertensive women need to have a clearer concept regarding their own disease, understand the warning signs and the risk factor of stroke and in turn create a greater awareness of the link between hypertension and stroke.

    Abstract: Cirrhosis of liver is one of the commonest hepatobiliary problems in Bangladesh as well as in global perspective. The purpose of the present study was to find out the clinico-demographic characteristics of liver cirrhosis patients admitted at a tertiary care hospital. Patients who are clinically suspected to have cirrhosis of liver, admitted in medicine units of Holy Family Red Crescent Medical College Hospital, Dhaka have been analyzed regarding clinical presentation and complications profile. Total 100 patients were included of which 84% were cirrhosis of liver, 7% were chronic active hepatitis, 3% were chronic persistent hepatitis, 2% were Wilson's disease with cirrhosis and 4% were hepatocellular carcinoma (HCC) on the top of cirrhosis of liver. The peak age incidence was between 31-50 years with male-female ratio of 4.88:1. History of jaundice (42%), parenteral medications (14%), infusion (6%) and abdominal surgery (4.0%) were important findings from patient's past history. History of alcohol intake was found in only 4% cases. Overall, weakness (78%), anorexia (72%), abdominal swelling (74%), weight loss (64%), ankle swelling (42%), low grade fever (35%) and yellowish discolouration of eyes, skin and urine (37%) were the most common symptoms. Common signs, revealed by clinical examination as a whole, were anaemia (55%), ascites (68%), oedema (52%), jaundice (34%), palpable spleen (55%) and hepatic facies (37%). Cirrhosis of liver most commonly occurs in middle aged male patients. Weakness, weight loss, anorexia, abdominal discomfort/dyspepsia and swelling of the abdomen are the most prevalent symptoms.

    Abstract: Sorption of meso-tetrakis(4-sulfonatophenyl)porphyrin ( H 2 tpps ) onto chitosan has been investigated in aqueous medium. Kinetic and isotherm studies were carried out by considering the effects of various parameters, such as pH, initial concentration of H 2 tpps solution, and temperature. The kinetic data obtained from different batch experiments were analyzed using pseudo first-, second-order, intraparticle, and film diffusion kinetic models. The equilibrium sorption data was analyzed by using Tempkin, Langmuir and Freundlich models. The best results were achieved with the pseudo second-order kinetic, Langmuir and Freundlich isotherm models. The intraparticle diffusion and film diffusion are the rate limiting steps. The amount of sorbate adsorbed at equilibrium (q e ) increased with increasing the initial concentration of H 2 tpps solution, showing maximum sorption capacity of 445.21 μmol.g -1 . The activation energy (E a ) of sorption kinetics was found to be 19.47 kJ.mol -1 . Thermodynamic parameters such as change in free energy (ΔG), enthalpy (ΔH) and entropy (ΔS) were evaluated by applying the Van't Hoff equation. Thermodynamic activation parameters such as change in enthalpy of activation (ΔH ‡ ), entropy of activation (ΔS ‡ ), and free energy of activation (ΔG ‡ ) were also calculated. The thermodynamics of H 2 tpps sorption onto chitosan in aqueous medium indicates its spontaneous and endothermic nature.

    Abstract: We report on the latest results of a Parkes multibeam survey for pulsars and dispersed radio bursts in the Large Magellanic Cloud (LMC). We conducted both periodicity and single-pulse searches at a much larger range of trial dispersion measures (DMs) than previously searched. We detected 229 single pulses with signal-to-noise ratio (S/N) 〉 7 that were classified by the deep-learning network FETCH as being real (with 〉 90% likelihood), of which nine were from the known giant-pulse-emitting pulsar PSR B0540−69. Two possibly repeating sources were detected with DMs suggesting that they lie within the LMC, but these require confirmation. Only three of the 220 unknown pulses had S/N 〉 8, and the DM distribution for these detected pulses follows an exponential falloff with increasing DM and does not show any excess of signals at DM values expected for the LMC. These features suggest that the detected pulses are not likely to be real, although they are visually compelling. We also report the discovery of a new pulsar (PSR J0556−67) in our periodicity search. This pulsar has a spin period of 791 ms, a DM of 71 cm −3 pc, an estimated 1400 MHz flux density of ∼0.12 mJy, and shows no evidence of binary motion. Future observations may be able to confirm whether any of the weak but promising candidates in our single-pulse and periodicity searches, including our two possible repeaters, are real or not.
