Abstract:
Zusammenfassung. Die chronisch rezidivierenden Entzündungsschübe führen zu einem progredienten fibrotischen Umbau des Pankreasgewebes. Leitsymptom sind Schmerzen! Mögliche Folgen sind Gangstrikturen, Pankreassteine, Pseudozysten, entzündliche Schwellung (Pseudotumor) des Pankreaskopfes und Hypertrophie der vegetativen Nerven. Ziel der Therapie ist, Symptome zu lindern und Komplikationen zu behandeln. Die konservative Therapie umfasst die Schmerztherapie nach WHO-Stufenschema, Ernährung, Enzym-Substitution und die Supplementierung fettlöslicher Vitamine. Dank Fortschritten der endoskopisch interventionellen Möglichkeiten werden viele Patienten nicht primär vom Chirurgen gesehen. Ein frühes interdisziplinäres Therapiekonzept ist aber entscheidend, denn trotz häufiger endoskopischer Re-Interventionen benötigt eine Mehrheit der Patienten eine chirurgische Therapie. Endoskopische Therapien kommen bei nicht operablen Patienten und als primäre Therapie von alleinigen Gangstenosen und Pseudozysten in Frage. Chirurgische Therapien sind eine gute Option bei Nicht-Ansprechen der endoskopischen Therapien nach spätestens einem Jahr, Gallengangs- und Duodenalstenosen, infizierten Pseudozysten mit soliden Anteilen, Pseudotumoren, Tumorverdacht und obstruierenden Verkalkungen im Pankreaskopf.
