    Abstract: Hintergrund Die Erhaltung einer arbeitshygienisch und mikrobiologisch einwandfreien Raumluft, insbesondere in Operationssälen, soll durch die Verwendung von raumlufttechnischen Anlagen (RLTA) gewährleistet werden. Diese sollen u. a. die Zahl an Partikeln und Keimen in der Luft unter empfohlenen Grenzwerten halten. Um die regelrechte Funktion sicherzustellen, werden in festgelegten Abständen Kontrolluntersuchungen unter Ruhebedingungen, frühmorgens vor offiziellem OP-Beginn durchgeführt. Ziel dieser Studie ist es, den Effekt von verschiedenen Belüftungsarten auf die Raumluftqualität in einer medizinischen Einrichtung anhand der Untersuchungsergebnisse aufzuzeigen. Methodik In der Studie wurden Messprotokolle von 34 Operationssälen aus einem Zeitraum von 10 Jahren ausgewertet. 29 Operationssäle wurden mittels turbulenzarmer Verdrängungsströmung belüftet, 5 mittels turbulenter Mischströmung. In allen 34 Operationssälen wurde die Luftreinheit bez. der Höhe an Luftpartikelzahlen und Luftkeimzahlen untersucht. Gemessen wurde an Messpunkten 10 mm unterhalb der Zuluftdecke, im Bereich des OP-Tisches und außerhalb der Zuluftdecke. Ergebnisse Luftpartikelzahlen an der Zuluftdecke lagen zwischen 0/m³ Luft und 4441/m³ und überschritten den empfohlenen Grenzwert nie. Luftkeimzahlen hingegen lagen bei 0 KBE/m³ bis 200 KBE/m³. In 10,9% der Messungen wurde hier der Grenzwert überschritten. Die Werte in Raummitte waren generell höher als an der Zuluftdecke. Es lagen signifikante Unterschiede (p  〈  0,001) zwischen den Werten an der Zuluftdecke oder am Bereich des OP-Tisches und Werten außerhalb der Zuluftdecke vor. Der Kontaminationsgrad im Bereich des OP-Tisches war je nach Belüftungsartsystem unterschiedlich (p  〈  0,001). Schlussfolgerungen Die Auswertungen verdeutlichen, dass durch die Verwendung von RLT-Anlagen i. d. R. eine partikel- und keimarme Raumluft gewährleistet werden kann. Die Verwendung turbulenzarmer Verdrängungsströmung scheint dabei bessere Ergebnisse zu liefern als die turbulente Mischströmung. Eine ausreichend große Zuluftdecke kann das OP-Gebiet, den Instrumententisch, das operierende Personal und den Patienten vor Kontamination schützen. Von einer Auswirkung der Luftkontamination auf postoperative Wundinfektionen ist jedoch laut aktuellem Kenntnisstand nicht auszugehen. Die Messmethodik sollte hinsichtlich Optimierungsmöglichkeiten überdacht werden.

    Abstract: In der Chirurgie gibt es zahlreiche Gesundheitsgefährdungen für das Personal. Ziel des Artikels ist, die Risiken für die Gesundheit und die Arbeitsfähigkeit des chirurgischen Personals und die Möglichkeiten für die Verbesserung des Arbeitsschutzes im Sinne der Verhaltens- und Verhältnisprävention sowie der Sekundär- und Tertiärprävention vor allem im Hinblick auf die Zusammenarbeit mit dem Betriebsarzt am Beispiel des Arbeitsplatzes OP-Saal aufzuzeigen. Die narrative Kurzübersicht basiert auf eigenen arbeitsmedizinischen, chirurgischen und interdisziplinären Erfahrungen aus der täglichen Praxis und selektiven Referenzen der aktuellen medizinisch-wissenschaftlichen Literatur. Die mögliche tätigkeitsbedingte Gesundheitsgefährdung und die Risiken für die Arbeitsfähigkeit des medizinischen Personals sowie die Möglichkeiten für den Arbeitsschutz im Sinne der Verhaltens- und Verhältnisprävention sind eine bedeutsame, nicht zu unterschätzende (da teils auch gesetzlich vorgeschriebene) Angelegenheit im interdisziplinären Status der Chirurgie. Vor allem den Berührungspunkten zur Arbeitsmedizin muss sich diese Disziplin (neben eigenem Kenntniserwerb von chirurgischer Seite) verantwortungsvoll widmen.

    Abstract: The aim of this study was to investigate the interactions between mental health, effort-reward imbalance, intrinsic overcommitment and burnout in occupational groups that face mental stress. Methods: Three hundred forty-six people aged 43.8 ± 9.9 years in professions characterized by mental strain (medical assistants, nurses, bank employees, teachers) were surveyed. The Effort-Reward Imbalance Questionnaire, overcommitment, General Health Questionnaire and Maslach Burnout Inventory were used. Results: There were occupational group-specific differences. Nurses showed the highest effort-reward imbalance, the highest impaired mental health and risk of burnout. The intrinsic overcommitment was highest among teachers. Gender and work experience had no influence, but there were some differences in age. Conclusions: The occupational groups we studied experience stress differently. They would benefit from the establishment of general conditions to improve the management of stress in the workplace. The focus should be on improving mental health.

    Abstract: The aim of this study was to identify work-related experience and behavior patterns among rescue workers in different urban and rural organizational structures. Methods: The subjects included 276 paramedics from Germany (average age 39.3 ± 8.04 years) in three groups (professional fire department, aid organization in urban or rural regions). Work-related behavior and experience patterns (AVEM) were examined. Influences of age, gender, professional qualifications, frequency of operation, and organization were analyzed. Results: AVEM risk patterns were detectable in 44.3% of professional fire department personnel and in 20% of both aid organization groups. There were differences in the various AVEM dimensions. Age had a strong influence on the AVEM dimension work-related ambition. Conclusions: There is a need for workplace health promotion and prevention measures, which are especially useful for employees in professional fire departments. Aid organizations in urban and rural regions should also attend to these measures.

    Abstract: Workloads are increasing and could cause mental stress, e.g., in the form of subjective effort reward imbalance (ERI) and overcommitment (OC). The heart rate variability (HRV) is a valid method for objective monitoring of workload. The aim of this project is to systematically evaluate the literature on HRV as an objective indicator for mental stress in individuals with different levels of ERI or OC. Methods A systematic literature review examining HRV of employees in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement for reporting systematic reviews was performed. Electronic databases used were PubMed, Ovid, Cochrane Libary, Scopus and Web of Science, PsyInfo, Psyndex, and Livio. Only articles from 2005 to 2021 were included. Inclusion criteria were case-control studies, intervention studies, cross-sectional studies, or longitudinal studies with different levels of ERI and/or OC, 〉 10 participants in each group, measurement of 24h HRV by using Holter ECG or chest belt, and full-text in English or German language. The methodological quality was evaluated by using a modified STARD for HRV. Results Five studies matched the inclusion criteria by using HRV (24-h ECG) with a different HRV analysis at day and night. It showed an adaptation of HRV with higher ERI or OC with reduced parasympathetic HRV parameters, but the studies were not comparable. Conclusions There is a need for occupational health studies that examine strains and stress of different employees with predominantly mental stress. The well-established parasympathetic mediated HRV parameters seem to be suitable parameters to objectify the stress.

    Abstract: This systematic review was conducted in accordance with the PRISMA guidelines to summarize the existing literature on the effects of different exercise interventions on cardiac autonomic control and secondary health factors. Resting heart rate variability (HRV) was used as indicator of cardiac autonomic control. Secondary factors were related to factors that contribute to cardiovascular health. Studies examining the effects of endurance, resistance, multimodal, or coordinative training interventions in healthy participants aged between 45 and 60 years old on average were considered. The methodological quality of the studies was examined using two assessment scales (TESTEX and STARDHRV). PROSPERO registration number: CRD42020206606. The literature review retrieved eight studies fulfilling all inclusion criteria. Cardiac autonomic control and cardiovascular health improved after endurance and multimodal interventions. Resistance training had no significant impact on HRV or any secondary health factor. Coordinative exercise interventions showed inconclusive results regarding HRV but showed significant improvements in secondary health factors. The quality assessment tools revealed some methodological and reporting deficits. Despite the small number of studies, we suggest endurance and multimodal interventions including aerobic exercises for the enhancement of cardiac autonomic control and the reduction of cardiovascular risk in middle-aged adults. Further studies need to be conducted to examine the long-term effects of exercise in the midlife period.

    Abstract: Safety footwear is part of personal protective equipment to reduce the risk of accidents in the workplace and is mandatory for many activities in various occupational groups. Wearing standardized work shoes can lead to increased stress on the worker due to their special characteristics (fit and weight). Additional optimization of work shoes, with the aim of symmetrizing the gait and thus reducing the stress, under the special conditions of occupational health and safety should therefore be sought. The purpose of this study was to examine whether the fitting of a gait adjustment aid with the aim of symmetrizing the gait has an influence on the general strain of shift workers. Methods For this purpose, heart rate variability (HRV) was continuously recorded in 29 male subjects (intervention group n  = 15, control group n  = 14) at 2 measurement time points (2 weeks in between) during the late shift. Results After wearing the gait adjustment aid for 2 weeks, the intervention group showed a significant increase in time-related HRV parameters. The frequency-related parameters total power and power in the high frequency band (power HF band) showed a decrease in the control group from the first to the second measurement time. The results indicate that the gait adjustment aid shows positive effects on HRV and can thus positively influence the general stress situation at the workplace in this sample. Discussion Further studies to verify an effect with larger numbers of cases and taking further grouping possibilities into account (e.g. parameters of gait analysis, physical performance and previous illnesses) are, however, required.
