Abstract:
A insuficiência cardíaca grave permanece como um desafio clínico complexo e de grande impacto, representando uma síndrome multifatorial que resulta em alta morbimortalidade, mesmo com os avanços terapêuticos. Diante desse cenário, a busca por estratégias inovadoras e eficazes para o manejo desses pacientes é de extrema relevância. Nesse contexto, o suporte circulatório mecânico (SCM) tem emergido como uma abordagem potencialmente vital na unidade de terapia intensiva (UTI) para enfrentar os desafios da insuficiência cardíaca grave. Nesse contexto, o objetivo do presente estudo é avaliar a eficiência e os riscos associados ao uso de suporte circulatório mecânico na terapia intensiva para insuficiência cardíaca grave. Trata-se de uma revisão sistemática, de natureza quantitativa, a qual utilizou as bases de dados PubMed, Cochrane Library e SciELO para confecção do estudo. A pesquisa incluiu apenas estudos em inglês publicados entre 2013 e 2023 que avaliaram a eficiência e os riscos associados ao uso de suporte circulatório mecânico na terapia intensiva para insuficiência cardíaca grave. O estudo ressalta que o suporte circulatório mecânico demonstrou melhorar a sobrevida em pacientes com insuficiência cardíaca grave, sendo comparável à eficácia do transplante cardíaco precoce. Além disso, discute-se que, mesmo para pacientes não elegíveis para transplante, o suporte circulatório mecânico oferece opções terapêuticas para melhorar a qualidade de vida, reduzir hospitalizações e possivelmente aprimorar os desfechos de mortalidade. Os dispositivos de assistência ventricular (DAVEs) demonstraram ser eficazes na melhoria da sobrevida e da função cardíaca em pacientes com choque cardiogênico grave. Os balões intra-aórticos (BIAs), por outro lado, não melhoraram significativamente a mortalidade em comparação com os DAVEs e têm um risco maior de sangramento. A ECMO também levou ao aumento da sobrevida em pacientes com choque cardiogênico grave, no entanto, com risco maior de complicações, como sangramento e infecção. Em suma, o suporte circulatório mecânico surge como pilar central na terapia intensiva para insuficiência cardíaca avançada, demandando avaliação atenta de eficácia e riscos. Dispositivos como DAVEs, BIAs e ECMO mostram melhorias na sobrevida e função cardíaca, porém sua aplicação requer avaliação criteriosa e coordenação multidisciplinar. A eficácia promissora do SCM exige consideração equilibrada com os riscos inerentes. A seleção criteriosa de pacientes, escolha adequada de dispositivos e monitorização constante surgem como pilares para atenuar riscos e otimizar resultados.
