Abstract:
Multifokalni horoidopatski sindromi su grupa retkih poremecaja kod kojih se
primarni patoloski proces pojavljuje u nivou pigmentnog epitela retine, sa
zahvatanjem ili bez zahvatanja horiokapilarisa. Etiopatogeneza multifokalnih
horoidopatskih sindroma nije poznata. U radu prikazujemo slucaj pacijentkinje
starosti 54 godine, lecene na Ocnoj klinici u Nisu zbog smanjenja vidne
ostrine, osecaja bola u ocima pracenog metamorfopsijom, treperenjem,
svetlucanjem pred ocima i skotomima. U toku hospitalizacije uradjen je
kompletan oftalmoloski pregled koji je obuhvatio proveru vidne ostrine,
biomikroskopiju prednjeg i zadnjeg segmenta, kolor i crno beli fotofundus;
uradjena je fluoresceinska angiografija. Nacinjena su biohemijska ispitivanja
krvi i urina, virusoloska dijagnostika, imunoloska ispitivanja i tipizacija
humanog leukocitnog antigena. Fluoresceinska angiografija pokazala je ranu
blokadu fluorescence horoidee aktivnim upalnim ognjistima, kao i tipicno
kasno bojenje. Biohemijskim ispitivanjem dokazani su poviseni nivo glukoze u
krvi, C-reaktivni protein, holestrol i frakcije holesterola kao i
trigliceridi. Imunoloskim ispitivanjima dokazano je smanjenje C3 i C4
komponente komplementa, i porast cirkulisucih imunih kompleksa. ELIZA testom
su dokazani visoki titri imunoglobilina klase G antitela na citomegalovirus,
herpes simpleks virus i varicela zoster virus. Rezultati tipizacije humanog
leukocitnog antigena bili su A1:A2, A2: A24(9), B1:B44(12), B2:B60(40),
C1:Cw3 C2: Cw 5. Rendgengrafija pluca, seroloska isiitivanja na trepomemu,
toksokaru i toksoplazmozu kao i magnetna rezonancija endokranijuma i orbite
su takodje uradjeni. Povisen nivo cirkulisucih antitela, C3 i C4 komponente
komplementa, serumskog amiloida A, utvrdjeni imunoloskim analizama govore u
prilog autoimunom mehanizmu nastanka oboljenja. Aktivacija autoimunog procesa
najverovatnije je pokrenuta infektivnim tj. virusnim agensom, s obzirom na
visestruku virusnu afekciju kod nase pacijentkinje.
