    Abstract: In multiple myeloma (MM), a high number of focal lesions (FL) detected using positron emission tomography/computed tomography (PET/CT) was found to be associated with adverse prognosis. To design a new risk stratification system that combines the Revised International Staging System (R-ISS) with FL, we analyzed the data of 380 patients with newly diagnosed MM (NDMM) who underwent 18 F-fluorodeoxyglucose ( 18 F-FDG) PET/CT upon diagnosis. The K-adaptive partitioning algorithm was adopted to define subgroups with homogeneous survival. The combined R-ISS with PET/CT classified NDMM patients into four groups: R-ISS/PET stage I ( n  = 31; R-ISS I with FL ≤ 3), stage II ( n  = 156; R-ISS I with FL  〉  3 and R-ISS II with FL ≤ 3), stage III ( n  = 162; R-ISS II with FL  〉  3 and R-ISS III with FL ≤ 3), and stage IV ( n  = 31; R-ISS III with FL  〉  3). The 2-year overall survival rates for stages I, II, III, and IV were 96.7%, 89.8%, 74.7%, and 50.3%. The 2-year progression-free survival rates were 84.1%, 64.7%, 40.8%, and 17.1%, respectively. The new R-ISS/PET was successfully validated in an external cohort. This new system had a remarkable prognostic power for estimating the survival outcomes of patients with NDMM. This system helps discriminate patients with a good prognosis from those with a poor prognosis more precisely.

    Abstract: Although the number of vascular surgeries using vascular grafts is increasing, they are limited by vascular graft-related complications and size discrepancy. Current efforts to develop the ideal synthetic vascular graft for clinical application using tissue engineering or 3D printing are far from satisfactory. Therefore, we aimed to re-design the vascular graft with modified materials and 3D printing techniques and also demonstrated the improved applications of our new vascular graft clinically. We designed the 3D printed polyvinyl alcohol (PVA) templates according to the vessel size and shape, and these were dip-coated with salt-suspended thermoplastic polyurethane (TPU). Next, the core template was removed to obtain a customized porous TPU graft. The mechanical testing and cytotoxicity studies of the new synthetic 3D templated vascular grafts (3DT) were more appropriate compared with commercially available polytetrafluoroethylene (PTFE) grafts (ePTFE; standard graft, SG) for clinical use. Finally, we performed implantation of the 3DTs and SGs into the rat abdominal aorta as a patch technique. Four groups of the animal model (SG_7 days, SG_30 days, 3DT_7 days, and 3DT_30 days) were enrolled in this study. The abdominal aorta was surgically opened and sutured with SG or 3DT with 8/0 Prolene. The degree of endothelial cell activation, neovascularization, thrombus formation, calcification, inflammatory infiltrates, and fibrosis were analyzed histopathologically. There was significantly decreased thrombogenesis in the group treated with the 3DT for 30 days compared with the group treated with the SG for 7 and 30 days, and the 3DT for 7 days. In addition, the group treated with the 3DT for 30 days may also have shown increased postoperative endothelialization in the early stages. In conclusion, this study suggests the possibility of using the 3DT as an SG substitute in vascular surgery.

    Abstract: Dapagliflozin, developed as an SGLT-2 inhibitor, has a low melting point and high hygroscopicity, which needs extreme care during pharmaceutical production to keep the active pharmacological property. Various attempts have been made to overcome these problematic properties. Objectives: To develop dapagliflozin prodrugs that have similar pharmacological effects with improved hygroscopicity and thermal stability. Methods: The novel dapagliflozin ester prodrugs containing pharmaceutically acceptable moieties were synthesized and their pharmacokinetics (PK) and physical properties were compared with dapagliflozin propanediol hydrate (DPD, Farxiga®). The PK in dog and rat, in vitro stability, hygroscopicity, and physical property studies in accelerated conditions (40°C, 75% RH) were performed with prodrugs. Results and Discussion: Among the eight synthesized prodrugs, Cmax and AUC0-48h values of prodrug 8b (1.35 μg/ml and 14.78 μg·h/ml, respectively) were similar to those of DPD (1.67 μg/ml and 14.27 μg·h/ml, respectively). However, the rest of the prodrugs 8a, 8c, 8d, 8e, 8f, 8g and 8h showed significantly lower Cmax and AUC0-48h values than DPD. Prodrug 8b completely converted into parent drug in the body. Conclusion: The novel prodrug 8b exhibited comparative PK profile to that of DPD, but with low hygroscopic property and better thermal stability than DPD.

    Abstract: Abstract 5024 Introduction The non-Hodgkin's lymphoma (NHL) with breast involvement is an extremely rare extranodal presentation. The aim of this study is to analyze the clinical features and treatment outcomes of NHL with breast involvement, and to investigate whether the arbitrary classification between primary breast lymphoma (PBL) and secondary breast lymphoma (SBL) has any clinical relevance. Patients and Methods We retrospectively analyzed 98 patients newly diagnosed as NHL with breast involvement from 16 hospitals in Korea between January 1994 and June 2009. The eligibility criteria included: (1) histological confirmation by pathologist should be made, (2) documentation of one or both breasts involvement by histology or imaging modalities was needed. The PBL was defined as disease localized to one or both breasts ± regional lymph nodes (ipsilateral axillary, supraclavicular and internal mammary lymph nodes), and SBL defined as disease with systemic lymph nodes and/or other extranodal organ involvement as well as one or both breasts involvement. Mediastinal and cervical lymph nodes were not regarded as regional lymph nodes. Recurrent lymphomas in the breast following prior treatment were not included in this analysis. Results The median age at diagnosis was 45 (range, 17-83) years, and median follow-up duration was 39.2 (range, 0.5-186.0) months. The two most common histologic subtypes included were diffuse large B-cell (68 patients, 69.4%) and mucosa-associated lymphoid tissue (8 patients, 8.2%) histology. Other 7 histologic subtypes were identified. Among 98 patients, 89 (91%) were treated at least 1 cycles of systemic chemotherapy, 82 (84%) treated with anthracycline-based regimens, 44 (44.9%) treated with combination of chemotherapy and rituximab. Any surgery or any radiotherapy to the breasts was performed in 27 (27.6%) patients, respectively. According to the definition, PBL and SBL group were 58 (59.2%) and 40 (41.8%) patients, respectively. The estimated 5-year progression-free survival (PFS) and overall survival (OS) was 51.2% ± 6.8 and 61.3% ± 6.0, respectively. Overall response rate (ORR) of 93 patients who were evaluable was 91.4% (CR, 76.3%; PR, 15.1%). Compared the baseline characteristics of PBL with those of SBL, PBL group showed more favorable clinical factors as 0 or 1 of Eastern Cooperative Oncology Group performance status (p 〈 0.001), normal LDH level (p=0.003), absence of B symptom (p=0.001) and low or low-intermediate international prognostic index (p 〈 0.001). 14 (24%) of PBL group were treated with abbreviated course (£4 cycles) of systemic chemotherapy and local therapy, 11 (28%) of SBL group were treated 4 or less than 4 cycles of systemic chemotherapy because of mainly disease progression or early death. ORR was significantly higher in PBL group (96.6% vs 72.5%, p=0.005), and estimated 5-year PFS (63.6% ± 8.7 vs 35.2% ± 8.9, p 〈 0.001) and OS (71.8% ± 7.2 vs 45.3% ± 9.9, p=0.004) was also significantly longer in PBL group compared with SBL group. Eight (8.2%) patients had central nervous system (CNS) relapse or progression in the course of disease, and PBL group had a significantly higher rates of CNS relapse or progression compared with SBL group (6 [10.3%] vs 2 [5%] , p=0.046). In multivariate analysis for PFS, 4 or less than 4 cycles of systemic chemotherapy regardless of any local treatments (hazard ratio [HR], 5.14; 95% confidence interval [CI] , 2.18-12.12) and more than 2 of extranodal organ involvement (HR, 10.64; 95% CI, 4.75-23.83) were independent prognostic factors for shorter PFS. And, for OS, 4 or less than 4 cycles of systemic chemotherapy regardless of any local treatments (HR, 4.03; 95% CI, 1.90-8.54) was the only independent prognostic factor for shorter OS. Conclusion Although criteria for PBL and SBL, we traditionally used, did not consider tumor biology and an arbitrary definition, we confirmed that the patients with NHL involved breasts could be classified into two different groups; traditional PBL and SBL according to the clinical characteristics, treatment outcomes and patterns of failure in this analysis. Continuation ( 〉 4 cycles) of active systemic chemotherapy was the only prognostic factor for OS, regardless of any local treatment. Thus, standard systemic chemotherapy should be the mainstay of treatment for NHL involved breasts. And, newer treatment strategy adapted on the poor prognosis should also be warranted.     Abstract: Allium sativum ., commonly known as garlic, is a species in the onion genus ( Allium ), which is a large and diverse one containing over 1,250 species. Its close relatives include chives, onion, leek and shallot. Garlic has been used throughout recorded history for culinary, medicinal use and health benefits. Currently, the interest in garlic is highly increasing due to nutritional and pharmaceutical value including high blood pressure and cholesterol, atherosclerosis and cancer. For all that, there are no comprehensive databases available for Expressed Sequence Tags(EST) of garlic for gene discovery and future efforts of genome annotation. That is why we developed a new garlic database and applications to enable comprehensive analysis of garlic gene expression. Description GarlicESTdb is an integrated database and mining tool for large-scale garlic ( Allium sativum ) EST sequencing. A total of 21,595 ESTs collected from an in-house cDNA library were used to construct the database. The analysis pipeline is an automated system written in JAVA and consists of the following components: automatic preprocessing of EST reads, assembly of raw sequences, annotation of the assembled sequences, storage of the analyzed information into MySQL databases, and graphic display of all processed data. A web application was implemented with the latest J2EE (Java 2 Platform Enterprise Edition) software technology (JSP/EJB/JavaServlet) for browsing and querying the database, for creation of dynamic web pages on the client side, and for mapping annotated enzymes to KEGG pathways, the AJAX framework was also used partially. The online resources, such as putative annotation, single nucleotide polymorphisms (SNP) and tandem repeat data sets, can be searched by text, explored on the website, searched using BLAST, and downloaded. To archive more significant BLAST results, a curation system was introduced with which biologists can easily edit best-hit annotation information for others to view. The GarlicESTdb web application is freely available at http://garlicdb.kribb.re.kr . Conclusion GarlicESTdb is the first incorporated online information database of EST sequences isolated from garlic that can be freely accessed and downloaded. It has many useful features for interactive mining of EST contigs and datasets from each library, including curation of annotated information, expression profiling, information retrieval, and summary of statistics of functional annotation. Consequently, the development of GarlicESTdb will provide a crucial contribution to biologists for data-mining and more efficient experimental studies.

    Abstract: Peripheral T cell lymphoma (PTCL) is a heterogenous group of aggressive lymphoma, which T-cell phenotype itself associated with unfavorable prognostic factors when compare to patients with B-cell phenotype lymphoma. High-dose chemotherapy (HDT) followed by autologous stem cell transplantation (ASCT) has represented a rescue option from poor prognosis. We retrospectively investigated the clinical outcomes of ASCT in 139 Korean patients with PTCL between December 1994 and June 2007. The overall survival (OS) and progression-free survival (PFS) were measured from the date of transplantation and were estimated using the Kaplan-Meier method. The median age at transplantation was 41 years. The histological subtypes had 45.3% PTCL-unspecified, 13.7% anaplastic large cell (ALK +/−: 15.7/57.8%), 10.8% angioimmunoblastic T cell, 21.6% extranodal NK/T cell, and 8.6% others. Disease status at transplantation consisted of 54 patients (38.8%) with first complete response (CR)/partial response (PR), 58 patients (41.7%) with chemosensitive-relapsed CR/PR, 15 patients (10.8%) with refractory CR/PR and 12 patients (8.6%) with progressive or refractory. At a median follow-up of 29.3 months, the 5-year probability of OS and PFS was 42.3±4.9% and 42.2±5.9%, respectively. The 5-year OS and PFS rates in patients with 1st CR/PR were 64.9±7.2% and 61.1±7.6%, compared to those in patients with chemosensitive-relapsed and refractory CR/PR were 33.5±6.8% and 33.2±8.3%, respectively. Early transplant-related mortality was 10.1%. Histological subtypes, bulky (≥10cm), prognostic index for PTCL (PIT) at diagnosis (0–2 vs 3–4), age-adjusted IPI at diagnosis, disease status at transplantation, the transplant timing (front-line vs second-line) and conditioning regimen were significant differences of survivals in univariate analysis. PIT, the transplant timing and the achievement of CR before transplantation were the variable prognostic for OS and PIT, bulky, and the achievement of CR provided significant independent factors for PFS in multivariate analysis. Our data showed that patients with PTCL who had low PIT and achieved CR before ASCT improved the OS and PFS regardless of the transplant timing and front-line ASCT of primary chemosensitive patients was effective in the OS.     Abstract: The breast is a rare extranodal site of non-Hodgkin lymphoma, and primary breast lymphoma (PBL) has been arbitrarily defined as disease localized to one or both breasts with or without regional lymph nodes involvement. The aim of this study was to evaluate the clinical outcomes in patients with diffuse large B cell lymphoma (DLBCL) and breast involvement, and to find the criteria of PBL reflecting the outcome and prognosis. Methods We retrospectively analyzed data from 68 patients, newly diagnosed with DLBCL and breast involvement at 16 Korean institutions between January 1994 and June 2009. Results Median age at diagnosis was 48 years (range, 20-83 years). Forty-three (63.2%) patients were PBL according to previous arbitrary criteria, sixteen (23.5%) patients were high-intermediate to high risk of international prognostic index. The patients with one extranodal disease in the breast (OED) with or without nodal disease were 49 (72.1%), and those with multiple extranodal disease (MED) were 19 (27.9%). During median follow-up of 41.5 months (range, 2.4-186.0 months), estimated 5-year progression-free survival (PFS) was 53.7 ± 7.6%, and overall survival (OS) was 60.3 ± 7.2%. The 5-year PFS and OS was significantly higher for patients with the OED group than those with the MED group (5-year PFS, 64.9 ± 8.9% vs. 27.5 ± 11.4%, p = 0.001; 5-year OS, 74.3 ± 7.6% vs. 24.5 ± 13.0%, p 〈 0.001). In multivariate analysis, MED (hazard ratio [HR], 3.61; 95% confidence interval [CI] , 1.07-12.2) and fewer than four cycles of systemic chemotherapy with or without local treatments (HR, 4.47; 95% CI, 1.54-12.96) were independent prognostic factors for worse OS. Twenty-five (36.8%) patients experienced progression, and the cumulative incidence of progression in multiple extranodal sites or other than breasts and central nervous system was significantly different between the OED group and the MED group (5-year cumulative incidence, 9.7 ± 5.4% vs. 49.0 ± 15.1%, p = 0.001). Conclusions Our results show that the patients included in OED group, reflecting different treatment outcome, prognosis and pattern of progression, should be considered as PBL in the future trial. Further studies are warranted to validate our suggested criteria.
