Abstract:
L’ OMS recommande un test de charge virale ( CV ) du VIH six mois après l'instauration du traitement antirétroviral ( ART ) et tous les 12 mois par la suite, mais les coûts empêchent les tests en routine de la CV dans de nombreux pays en développement. Nous avons cherché à concevoir une approche ciblée du suivi systématique de la CV qui pourrait réduire les coûts et identifier ceux présentant un faible risque d’échec virologique ( EV ). Méthodes Nous avons analysé les données de dépistage d'un essai clinique sur des adultes sous ART au Malawi. Nous avons identifié des facteurs de risque associés à l’ EV et utilisé la méthode Knill‐Jones pour attribuer un score récapitulatif identifiant les personnes présentant un risque plus faible d’ EV . Résultats Parmi 957 adultes, la prévalence de l’ EV était de 9,4%. Les facteurs associés indépendamment à l’ EV comprenaient: l’âge 〈 38 ans ( OR = 3,44; IC 95%: 2,01–5,89), la durée de l’ ART 〉 2,5 ans ( OR = 2,98; IC 95%: 1,79–4,96), l'adhésion à l’ ART 〈 95% ( OR = 1,76; IC 95%: 1,06–2,94), des CD 4 〈 200 cellules/μL ( OR = 5,94; IC 95%: 3,27–10,78), l'hémoglobine 〈 13 g/dl ( OR = 2,76; IC 95%: 1,70 – 4,50) et des CD 8 〉 885 cellules/μL ( OR = 2,10; IC 95%: 1,28–3,44). Notre score récapitulatif de prédiction de l’ EV incluait tous les facteurs ci‐dessus à l'exception de la numération des CD 8 et était assez précis avec une aire sous la courbe ( AUC ) validée de 0,76. La mise en œuvre pourrait réduire les tests de CV de 65%. Conclusion Un score simple intégrant l’âge, la durée et l'adhésion de l’ ART , et la numération des CD 4 peut identifier avec précision les adultes à faible risque d’ EV dans un contexte subsaharien‐africain. Dans les régions où l'utilisation du traitement antirétroviral est élevée et où les capacités de dépistage de la CV sont limitées, une approche ciblée pourrait optimiser la surveillance de routine de la CV tout en identifiant les adultes ayant besoin de schémas alternatifs.
