    Abstract: Nepal earthquake of 2015, a massive earthquake that struck near the city of Kathmandu (Gorkha) in central Nepal on April 25, 2015. Nearly 9,000 people were killed, many thousands more were injured, and more than 600,000 structures in Kathmandu and other nearby 31 districts were either damaged or destroyed. The earthquake was felt throughout central and eastern Nepal, much of the Ganges River plain in northern India, and northwestern Bangladesh, as well as in the southern parts of the Plateau of Tibet and western Bhutan.

    Abstract: Coronavirus disease 2019 (COVID-19) is a newly emerged disease that has become a global public health concern as it rapidly spread around the world. The etiologic agent responsible for this disease has been named as severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) by the International Committee on Taxonomy of Viruses as it shows similar genomic features to that of SARS-CoV which caused a pandemic in 2002. This disease first appeared in Hubei province of China and it follows human-to-human transmission but the path this virus took to set up human infection remains a mystery. By 17 April 2020, globally there have been 2,074,529 confirmed cases with 139,378 deaths because of COVID-19. SARS-CoV-2 shows several similarities with SARS?CoV, and Middle East Respiratory Syndrome Coronavirus (MERS-CoV) with its clinical presentations. This can vary from asymptomatic infection to severe disease and mortality. Real-time reverse-transcription polymerase chain reaction (rRT-PCR) screening is considered as the standard laboratory test for the diagnosis of COVID-19. There is no proven antiviral agent against SARS-CoV-2 so the treatment for COVID-19 is symptomatic, aiming for the management of the symptoms and prevention of the complications. The outbreak of COVID-19 has led to the implementation of extraordinary public health measures throughout the world. Numerous antiviral compounds used to treat other infections are being clinically researched to find possible treatment. Similarly, the traditional public health outbreak response strategy of isolation, quarantine, social distancing and community containment has been implemented in multiple countries and has played an important role in the prevention of new outbreaks.     Abstract: Background: On April 25 2015, a magnitude of 7.8 earthquake struck in central Nepal, causing a huge physical and social disturbances. Physical impacts comprised casualties with deaths and injuries and damage to infrastructure, cultural heritage and natural environment while social impacts are demographic, psychological and economic consequences. We report our experience in treating victims who were brought at our centre via different means.Objectives: to provide an overview the caseload and provide analysis of earthquake victims for improving the future strategies in similar scenarios.Methods: A retrospective study of the clinical characteristics and patterns of hospitalised patient after the 2015 earthquake was conducted. Demographic evaluation, surgical procedures and morbidities were reviewed. The patients were followed up for an average of 24 months, detail records were kept on their recovery and function.Results: A total of 85 patients were treated with subsequent follow-up. The proportion of males admitted was similar to that of females (49.4% and 50.5% respectively). The highest number of admitted age group ranges (17- 45) was about 37.64%. Most injured site was lower limbs (68.23%) where fracture tibia and fibula had the highest incidence (56.89%). Out of all, 14.11% of cases were open fractures. Trauma severity was assessed with injury severity score and most of them categorized as mild one (95.29%). The most common procedure performed was closed reduction and pinning (n=28), followed by open reduction and fixation (n=24). Overall, mortality rate was 2.35% (2 of 85). Total 25 implants were removed within three years of period and 28.6% of patients were not returned to date.Conclusions: The injury epidemiology reported in this study showed quite congruence with most other earthquake related studies. Analysis profiles of injuries and clinical features of earthquake victims will definitely impact rescue efforts and treatment of fracture injuries in possible future natural calamities.

Menstrual disorders such as amenorrhea, abnormal/excessive uterine bleeding, dysmenorrhea, and premenstrual syndrome are particularly common in adolescentgirls. Although menstrual irregularities may be normal during the early postmenarchal years, pathologicalconditions require proper and prompt management.     Abstract: Introduction: Multicenter surveillance has been carried out on maternal near-miss in the hospitals with sentinel units. Near-miss is recognized as the predictor of level of care and maternal death. Reducing maternal mortality ratio is one of the challenges to achieve Millennium Development Goal. Objective was to determine the frequency and the nature of near-miss (severe acute maternal morbidity) events and analysis of near-miss morbidities among pregnant women.Methods: Prospective surveillance was done for a year in 2012 in nine hospitals in Kathmandu valley. Cases eligible by definition recorded as a census based on WHO near-miss guideline. Similar questionnaire and dummy tables were used to present the result by non-inferential statistics. Results: Out of 157 cases identified with near-miss rate of 3.8, severe complications were PPH (40%) and preeclampsia-eclampsia (17%). Blood transfusion (65%), ICU admission (54%) and surgery (32%) were the common critical intervention. Oxytocin was the main uterotonic used both prophylactically (86%) and therapeutically (76%), and 19% arrived health facility after delivery or abortion. MgSO4 was used in all cases of eclampsia. All of the laparotomies were performed within 3 hours of arrival. Near-miss to mortality ratio was 6:1 and MMR 62.Conclusions: Study result yields similar pattern amongst developing countries and same near-miss conditions as the causes of maternal death reported by national statistics. Process indicators qualify the recommended standard of care. The near-miss event can be used as a surrogate marker of maternal death and a window for system level intervention.  Keywords: abortion, eclampsia, hemorrhage, near-miss, surveillance

    Abstract: Introduction: Cervical cancer is the leading gynaecological cancer in Nepal. Most of the time, it is diagnosed in the late stage with its associated morbidity and mortality. This study aimed to find out the prevalence of late-stage presentation of cervical cancer among confirmed cases of cervical cancer in a tertiary care centre. Methods: A descriptive cross-sectional study was conducted at a tertiary care centre of Nepal from March 2021 to May 2021 after obtaining ethical clearance from the Institutional Review Board (Reference no.805). A convenient sampling method was used. A descriptive analysis was done of all cases of cervical cancer who were diagnosed within the last 36 months and attended hospital during the study period. The information was collected by interview and hospital record was checked. Analysis was done using Statistical Package for Social Sciences version 20. Point estimate at 95% Confidence Interval was calculated along with frequency and proportion for binary data. Results: Among the 142 confirmed cervical cancer cases, the prevalence of late-stage presentation of cervical cancer was 93 (65.5%) (57.7-73.3 at 95% Confidence Interval). The mean age at diagnosis was 50.6±10.9 years. More than two-thirds of the women were from outside Kathmandu valley 102 (71.8%) and came from 〉 50km distance. The majority of the women 83 (58.5%) were illiterate. Conclusions: The study showed that two-thirds of the women presented in advanced stage and the factors leading to the late stage. This highlights the fact, that the focus should be on the provision of organized screening programs and early diagnosis and treatment of cervical cancer.

    Abstract: ABSTRACTThe study is done to know the causes for the abnormal uterine bleeding in perimenopausal women atChitwan valley and to compare the histopathological findings between the pre-menopausal andpostmenopausal women. Retrospective analysis of patients who have had abnormal bleeding during a periodof one and a half years' from January 2000 to June 2001 in Department of Gynaecological Oncology of BPKoirala Memorial Cancer Hospital (BPKMCH), Chitwan, Nepal. There were total sixty women (aged 45years and above) having abnormal bleeding managed at BPKMCH. A total of sixty patients were studied.Majority (57%) of them was postmenopausal ladies presented with abnormal vaginal bleeding. The agerange was 45 to 81 years. Only 37% had pathological bleeding including malignancies and only about 17%had malignancies. Malignant diseases were found in 7.7% cases in the pre-menopausal ladies whereas itwas 23.4%, almost three times higher, in the post-menopausal ladies. Carcinoma of the cervix and of theendometrium occurred with increasing frequency with increasing age in this study. So, a thorough work-upis needed for the elderly women presenting with an abnormal vaginal bleeding especially to rule outmalignancies.Key Words: endometrial biopsy, endometrial cancer, postmenopausal bleeding.
