Abstract:
Brojne studije su pokazale da je rana postinfarktna angina pektoris (angina pectoris) prediktor lose prognoze kod bolesnika s akutnim infarktom miokarda, mada nije bilo nijedne studije koja je proucavala ovu pojavu kod bolesnika starije zivotne dobi. Cilj naseg rada je bio da na osnovu petogodisnjeg nadgledanja bolesnika starije zivotne dobi s akutnim infarktom miokarda utvrdi da li rana postinfarktna angina pektoris kod starih osoba utice na mortalitet i pojavu novih koronarnih dogadjaja. Studijom je obuhvacen 51 bolesnik s akutnim infarktom miokarda, star 60 ili vise godina. Rana postinfarktna angina pektoris je zabelezena kod 31 bolesnika (grupa 1), dok kod 20 bolesnika (grupa 2) nije utvrdjena. Bolesnike smo nadgledali tokom pet godina i belezili smrt i nove koronarne dogadjaje. Analiza prezivljavanja je uradjena Kaplan-Mejerovim metodom. Analiza prezivljavanja nije pokazala razlike izmedju dve grupe bolesnika u verovatnoci nastanka: smrti (p=0,9459), srcane smrti (p=0,8253), ponovnog infarkta miokarda (p=0,7405), novih koronarnih dogadjaja (p=0,1708), nestabilne angine pektoris (p=0,1788), revaskularizacije miokarda (p=0,0691) i insuficijencije rada srca (p=0,7047). Rana postinfarktna angina pektoris kod bolesnika starije zivotne dobi nije povezana s losom dugorocnom prognozom, dok je kod mladih bolesnika ova veza potvrdjena u brojnim studijama.
