Abstract:
Cuantificar el tiempo desde la entrada al programa de cuidados para el VIH hasta el comienzo del TAR e identificar los factores asociados con la iniciación del TAR en zonas rurales de KwaZulu‐Natal, Sudáfrica. Métodos Los adultos ≥16 años que entraron en el programa descentralizado de TAR de Hlabisa entre 2007‐2011 fueron seguidos hasta Junio 2013. Se calculó el tiempo medio de supervivencia hasta el momento de iniciar el TAR , desde la fecha en que ingresaron al programa y desde la fecha en que cumplieron criterios para iniciar TAR mediante métodos de Kaplan Meier. Los factores asociados se estudiaron con regresiones de Cox, con censura para muertes. Resultados De 37,749 adultos (71.6% mujeres), 17,638 (46.7%) comenzaron TAR . Casi la mitad (46.9%) cumplieron los criterios de CD 4 para ser elegibles para tratamiento en el momento de entrar en el programa. Entre los 20,039 individuos que no eran elegibles para TAR en el momento de entrar en el programa, solo un 62.5% fueron retenidos en los cuidados con al menos otra medición de CD 4, de los cuales 6,688 fueron subsecuentemente elegibles para TAR . En general, un 65.5% de los 24,398 individuos elegibles para TAR iniciaron el tratamiento durante el periodo de estudio. La iniciación del TAR era más probable entre mujeres (p 〈 0.0001), en individuos mayores (p 〈 0.0001) y en pacientes con un bajo conteo de CD 4 (p 〈 0.0001). Los pacientes que se volvían elegibles durante el seguimiento dentro del programa de atención para el VIH tenían una mayor probabilidad de iniciar TAR que aquellos que eran elegibles en el momento de entrar en el programa (72.6% vs 62.9%, cociente de riesgos instantáneos corregido=1.46; IC 95% [1.41‐1.51]), ajustando para sexo, edad, año y conteo de CD 4 en el momento de ser elegibles. Conclusiones En este programa rural, la continuidad de cuidados una vez que las personas han entrado en el programa de cuidados para VIH continúa siendo un reto, especialmente entre hombres y adultos jóvenes. El iniciar el TAR es más probable que se dé entre aquellos que ya están recibiendo atención para el VIH antes de ser elegibles, comparados con aquellos que ingresan en el programa de atención en una etapa tardía de la enfermedad
