    Abstract: Amaç: Bu çalışmada sezaryen (CS) hastalarında 6, 12 ve 24 saat sonra çıkarılan idrar sondalarının ameliyat sonrası sonuçlarını ve üriner retansiyonlarını karşılaştırmayı amaçladık. Gereç ve Yöntemler: Bu prospektif çalışmaya önceden sezaryen ameliyatı olan ve bu endikasyon nedeniyle spinal anestezi altında elektif sezaryen uygulanan gebeler dahil edildi. Hastalar 1:1:1 oranında üç gruba ayrıldı ve 6,12 ve 24 saat sonra hastaların sondası çekildi. Rezidüel idrar miktarı ultrasonografi ile ölçüldü. Ayrıca ambulasyona kadar geçen süre, rekateterizasyon oranı, idrar yolu enfeksiyonu (İYE) semptomları ve hastanede kalış süreleri kaydedildi. Bulgular: 6 saat sonra kateteri çıkarılan grupta üriner retansiyonu oranı (13.9 %) anlamlı olarak daha yüksekti. İlk ambulasyona kadar geçen süre kateterin süresinden etkilenmişti ve kateteri 24 saat sonra çıkarılan grupta anlamlı olarak daha yüksekti (p=0.038). Ek olarak, 24 saat sonra kateteri çıkarılan grupta dizüri ve İYE anlamlı olarak daha yüksek izlendi. Ancak hastanede kalış süresi üç grup arasında benzerdi. Sonuç: Yarar-zarar dengesi göz önüne alındığında, CS’den 12 saat sonra kateterin çıkarılması önerilebilir. Ancak, yönetim bireyselleştirilmelidir.

    Abstract: OBJECTIVE: In this study, we aimed to investigate the knowledge and attitudes of obstetricians about probiotics, prebiotics, and microbiota and evaluate the differences by years in the profession. STUDY DESIGN: This cross-sectional study was conducted on obstetricians working in a tertiary center. A non-validated questionnaire consisting of 18 questions was collected face-to-face. The first part of the questionnaire evaluated the sociodemographic data and the duration of the profession, the other parts assessed the knowledge of prebiotic, probiotic, and microbiota of obstetricians and their attitudes toward prescribing or sug gesting these supplements. Furthermore, the obstetricians were divided into two groups: those with a profession less than 〈 12 years and ≥12 years according to the median value of the whole study group to assess the impact of the experience. RESULTS: There was a statistically significant difference between obstetricians with a profession of 〈 12 years and ≥12 years in their self-knowledge assessment of probiotics, prebiotics, and microbiota. However, the correct definitions were chosen at similar rates. Only 40% of obstetricians with a 〈 12 years of profession thought the use of probiotics during pregnancy to be safe, while more than two-thirds of the obstetricians with a profession of ≥12 years found it safe. Additionally, significant differences were found between the groups’ sources of information, and the purpose of recommendation. CONCLUSIONS: The knowledge and attitude of obstetricians towards probiotics, prebiotics, and microbiota may be affected by the duration of the profession.

    Abstract: Purpose: An accurate assessment of a potential lymph node metastasis (LNM) is an important issue for the individual treatment of cervical cancer. The goal of the current retrospective study was to identify predictive factors of LNM in patients with cervical cancer. Materials and Methods: In this retrospective study, from 2007 to 2019, patients with cervical cancer of the International Federation of Gynecology and Obstetrics (FIGO) stage IB1-IIA2 were included. Incidences of LNM were described, and the predictive value of the risk factors for pelvic and paraaortic LNM were determined in univariate and multivariate models. Results: Among 212 patients, the number of patients with pelvic LNM was 81(38.2%), the number of patients with paraaortic LNM was 17(8%). Multivariate analysis revealed that the parametrial involvement (hazard ratio [HR]: 2.75, 95% confidence interval [CI] , 1.18-6.40, p=0.019) and LVSI ([HR]: 3.83, 95% [CI] , 1.76-8.38, p=0.001) were independent risk factors for pelvic LNM with cervical cancer patients. For paraaortic LNM parametrial involvement was independent risk factor on multivariate analysis ([HR]: 3.62, 95% [CI] , 1.20-10.98, p=0.023). Conclusion: The predictive factors of pelvic LNM in cervical cancer patients were parametrial involvement and LVSI and for paraaortic LNM was parametrial involvement.

    Abstract: Amaç: İnfertil hastalarda soğuk ve sıcak kontrast madde kullanılarak çekimi yapılan histerosalpingografi (HSG) işlemi sırasında ve sonrasında vizüel analog skalası (VAS) kullanarak ağrı skorlarını karşılaştırmak ve sıcak kontrast kullanımının spontan gebelik oranını değiştirip değiştiremeyeceğini araştırmak. Gereçler ve Yöntem: Çalışma prospektif randomize kontrollü tasarlandı ve Mart-Eylül 2017 tarihleri arasında HSG prosedürü uygulanan primer infertil hastalar dahil edildi. Hastaların yaşı, vücut kitle indeksi (VKİ), geçirilmiş cerrahi öyküsü, infertilite süresi ve en ağrılı adet dönemindeki ağrı düzeyleri kaydedildi. İşlem sırasında ve işlem sonrası 30. dakikada ağrı şiddeti VAS ile sorgulandı. Hastaların yaşı, vücut kitle indeksi (VKİ), geçirilmiş cerrahi öyküsü, infertilite süresi ve en ağrılı adet dönemindeki ağrı düzeyleri kaydedildi. İşlem sırasında ve işlem sonrası 30. dakikada ağrı şiddeti VAS ile sorgulandı. Bulgular: Gruplar arasında yaş, VKİ, infertilite süresi, geçirilmiş cerrahi ve menstrüasyon sırasındaki VAS açısından istatistiksel olarak anlamlı fark yoktu. Sıcak gruptaki hastaların ortalama HSG süresi anlamlı olarak daha düşüktü (p=0,001). Sıcak gruptaki hastaların HSG sırasındaki ve 30 dakika sonraki ortalama VAS skoru, kontrol grubuna göre (sırasıyla 5,3±4,2 vs 7,5±2,0) (1.1±0,9 ve 2,4±1,4) anlamlı derecede düşüktü. HSG sırasında şiddetli ağrı yüzdesi sıcak grupta %25.5 iken kontrol grubunda %66.7 idi.     Abstract: Amaç: Enfekte epizyotomi (EE) vajinal doğumun nadir bir komplikasyonudur. Ancak, EE’nin ortaya çıkmasında rol oynayan risk faktörlerinin pratikte kullanımı belirsizdir. Bu çalışmanın amacı, EE gelişimini tahmin etmek için belirleyeceğimiz risk faktörleri ile klinikte uygulanabilecek bir risk skorlama modeli oluşturmaktır. Gereç ve Yöntem: Bu retrospektif çalışmaya 1 yıllık süreç içerisinde mediolateral epizyotomi ile vajinal yolla doğum yapan kadınlar dahil edildi. EE tanılı çalışmaya alınmaya uygun olan 42 kadın vaka grubunu oluştururken, rastgele seçilen ve EE saptanmayan 168 sağlıklı kadın kontrol grubunu oluşturdu. EE gelişimi için olası risk faktörleri çok değişkenli regresyon analizi ile değerlendirildi. Bulgular: Yapılan analizde EE ile ilişkili 5 önemli bağımsız faktör bulundu. Bu faktörlerden 5 puan mevcut sigara içme durumuna, 4 puan gebelikte ≥11.5 kg kilo alımına, 3 puan postpartum nötrofil-lenfosit oranı ≥10.4’a verildi. Diyabetes mellitus varlığına 2 puan ve doğumun aktif evresinden önce hastaneye yatış varlığına 1 puan verildi. 5.5 cut-off değeri EE’yi tahmin etmede orta derecede etkili bulunmuştur. Sonuç: Bu beş değişkenli model, yüksek bir etkinlikle EE varlığını predikte edebilir. Bu değişkenlerin varlığında, klinisyenler EE riski taşıyan hastaları belirleyebilir. Bu sayede bireyselleştirilmiş hasta risk değerlendirilmesi ve danışmanlık yapılabilir.

    Abstract: This study aimed to investigate the relationship between the degree of prolapse and bone mineral density (BMD) in postmenopausal women with pelvic organ prolapse (POP). Materials and Methods: This retrospective study includes 40 postmenopausal women who were operated for POP. While patients who had grade 〈 3 vaginal cuff prolapse were accepted as low grade prolapse, patients with grade ≥ 3 vaginal cuff prolapse were accepted as high grade prolapse. Patients' demographic characteristics, obstetric and gynecological history, laboratory findings and dual- energy X-ray absorptiometry results (lumbar spine BMD, lumbar spine T and Z scores, femur neck BMD, femur neck T and Z scores) were recorded as study parameters. Results: There were no significant differences between the groups in terms of age, body mass index, gravida, parity, follicle-stimulating hormone levels. BMD of the femur neck and lumbar spine were significantly higher in the low grade prolapse group than in the high grade prolapse group. T score of the femur neck were significantly higher in the low grade prolapse group than in the high grade prolapse group. In addition, estradiol levels of the high grade prolapse group was lower than the low grade prolapse group. BMD and T score of the femur neck and BMD of the lumbar spine were found to be significantly lower in the high grade prolapse group, suggesting that, there may be an association between the severity of POP and the risk of osteoporosis in postmenopausal women.

    Abstract: Introduction: This investigation’s purpose was to determine the incidence of acute kidney injury, the associated risk factors, and the 90-day outcomes and kidney function of geriatric patients who underwent major open abdominal gynecologic oncologic surgery. Materials and Method: This retrospective cohort study involved patients aged ≥ 65 years who underwent major open abdominal gynecologic oncologic surgery. Results: The postoperative incidence of acute kidney injury in gynecologic oncologic surgical procedures was 22.1%, with a transient nature observed in 72% of patients. The in-hospital mortality rate was 4 %. Kidney function on the 90th day after acute kidney injury development revealed that estimated glomerular filtration rate regressed by over 25% in 6 patients (24%). Acute kidney injury development was associated with surgical time, intraoperative bleeding volume, bowel procedures, the presence of ascites, intraoperative hypotension, vasopressor use, postoperative diuretic use, postoperative hypoalbuminemia, prolonged post-anesthesia care unit, and hospital stay (p 〈 0.05). A logistic regression analysis of the risk factors for acute kidney injury revealed that surgery duration was a significant one (p 〈 0.05). Conclusion: Postoperative acute kidney injury is an important postoperative complication associated with the development or progression of chronic kidney disease. This leads to a prolonged stay in the post-anesthesia care unit and in the hospital. Although acute kidney injury is frequently transient in geriatric patients following major open abdominal gynecologic oncologic surgery, developing preventive measures, encouraging team collaboration, and monitoring serum creatinine concentration in the early postoperative period are critical in complex surgical procedures.
