Abstract:
But: Le but de cette étude était de rechercher l'efficacité de la mesure des différences de volume du tissu gingival pour enregistrer les modifications de l'état inflammatoire de la gencive. Matériaux et méthodes: Les données de cette recherche furent obtenues d'une évaluation pour un bain de bouche, conçue comme une étude en double aveugle croisée, avec contrôle placebo croisée sur 4 semaines dans laquelle une gingivite expérimentale localisée fut induite. 24 volontaires furent recrutés. Seules les données des phases placebos de chaque sujet ont été utilisées dans cette recherche. Pendant la phase d'accumulation de plaque, des systèmes de protections de plaque furent portés pendant les manœuvres habituelles d'hygiène pour empêcher l'élimination de la plaque des zones expérimentales (1 ère et 2ième prémolaires et molaires d'un quadrant supérieur). L'examen clinique avec mise en évidence de la plaque et de la gingivite fut réalisé aux jours 0, 4, 7, 14, 21, 28, et 42. Les différences de volume de la papille gingivale furent déterminées entre les jours 0, et les jours 21, 28 et 42, et entre les jours 28 et 42 par la prise de mesures sur des répliques des situations cliniques respectives à l'aide d'un scanner laser 3D et d'un logiciel de superposition 3D. Les données ont été analysées par le test de Wilcoxon en rangs assignés. Résultats: L'accumulation de plaque dans les zones expérimentales entraînait une augmentation fortement significative ( p 〈 0.001) de l'inflammation de la papille gingivale. Le GI papillaire moyen (déviation standard) initial était de 0.23 (0.34) puis de 1.22 (0.27) et 1.2 (0.31) aux jours 21 et 28 respectivement. L'augmentation moyenne de volume de toutes les papilles par rapport aux mesures initiales était de 25,478 μm 3 après 21 jours et de 24.210 μm 3 après 28 jours. Après retour à une hygiène normale, le volume moyen des papilles diminuait entre les jours 28 et 42 de 19,250 μm 3 . Conclusion: Avec cette nouvelle méthode, l'œdème papillaire gingival peut être quantifié in vivo à partir de répliques de la situation clinique.
