JOTS‐21‐0366.R2 Spanish Abstracts by Asociación Chilena de Estrés Traumático (ACET) Diferencias de género en los trastornos comórbidos con el trastorno de estrés postraumático entre los marineros e infantes de marina estadounidenses DIFERENCIAS DE GÉNERO EN LOS TRASTORNOS COMÓRBIDOS CON EL TEPT La comorbilidad psicológica, la coexistencia de trastornos de salud mental, es más a menudo la regla que la excepción entre los individuos con trastorno de estrés postraumático (TEPT). Las investigaciones muestran que las estimaciones de prevalencia de determinados trastornos psicológicos difieren según el género; sin embargo, se sabe poco sobre si estos patrones persisten en presencia de un diagnóstico de TEPT comórbido. Este estudio examinó las diferencias de género en las estimaciones de prevalencia de las condiciones comórbidas con TEPT utilizando los registros médicos de 523,626 marineros e infantes de marina en servicio activo que ingresaron en el ejército durante un período de 8 años. Mediante pruebas de independencia de chi‐cuadrado, detectamos diferencias de género estadísticamente significativas para condiciones comórbidas específicas en la submuestra de 9,447 miembros del servicio con un diagnóstico de TEPT. Las mujeres tenían mayor probabilidad que los hombres de tener TEPT con ajuste por comorbilidad, OR = 1.35; depresión, OR = 1.71; y ansiedad generalizada u otros trastornos de ansiedad, OR = 1.16, con mayores efectos para la alimentación, OR = 12.60, y trastornos de personalidad, OR = 2.97. Por el contrario, las mujeres tenían menos probabilidades que los hombres de tener un diagnóstico de TEPT comórbido con consumo de alcohol, OR = 0.69, y de trastornos por consumo de drogas, OR = 0.72, con mayores efectos para insomnio, OR = 0.42, y lesión cerebral traumática, OR = 0.17. No se observaron diferencias significativas entre los géneros para comorbilidad con trastornos bipolar, obsesivo‐compulsivo, pánico/fóbico, psicótico o somatomorfo/disociativo, ps = .029‐ .314. Los resultados muestran que las diferencias de género en las condiciones comórbidas con el TEPT generalmente se alinean con las dimensiones internalizantes y externalizantes. Las diferencias en las comorbilidades con TEPT entre mujeres y hombres podrían tener implicaciones para el desarrollo y la aplicación del tratamiento.
