    Abstract: Immunoglobulin G (IgG) isolated from normal human blood plasma stabilizes the structure of perfused crosslinked fibrin and prolongs the time for its dissolution with plasmin, when the fibrin surface is exposed to 500 s-1 shear rate flow. The IgG from patients suffering in antiphospholipid syndrome with thrombotic complications exerts even stronger antifibrinolytic effect. A patient, whose IgG does not affect the fibrin dissolution with plasmin, displays a bleeding tendency. The shear stress-induced disassembly of the fibrin clots containing IgGs with antifibrinolytic potency occurs at a much more advanced stage of fibrin digestion, as evidenced by the electrophoretic pattern of the ureatreated samples. The antifibrinolytic effects are also produced under static conditions and these are caused by the variable portion of the IgG molecules (fragment Fab), whereas the constant part (fragment Fc) has no inhibitory effect. The IgGs with antifibrinolytic properties do not affect directly the plasmin activity in amidolytic assay, but the IgGs from APS patients obliterate the competition of the fibrin and the peptidyl-p-nitroanilide for the protease in the same assay system suggesting interference of the IgGs with the plasmin action on the fibrin substrate. Thus, the correlation of the clinical symptoms with the effect of the isolated IgG on the dissolution of perfused fibrin clots supports a physiological and a pathological role of IgG in the fibrinolytic process related to the variability of the cross-reactions of immunoglobulins with fibrin, fibrin degradation products or fibrin-plasmin complexes.

    Abstract: The Coronavirus Disease 2019 (COVID-19) pandemic and the measures of social distancing and national lockdown had a significant impact on everyday life. Individuals with BVI (blindness and visual impairment) are assumed to face increased barriers in numerous domains of their lives. This online survey research investigates, among Hungarian adults with BVI ( N = 132), the impact of the lockdown on their access to shopping, daily support needs, access to remote studies of higher education or work, and leisure habits. Respondents accounted for negative impacts of the lockdown on their participation and independence in all research topics. Issues of accessibility were common both concerning shopping for essential goods and access to remote study and work.

    Abstract: Háttér és célok: Cikkünkben a tájékozódás- és közlekedéstanítás egy speciális területét, a vakvezetőkutyával való közlekedéshez kapcsolódó vakvezetőkutya-kiképzést mutatjuk be, ismertetve a nemzetközi és hazai történetét is. A téma feldolgozását nagyban nehezítette, hogy a vakvezetőkutyás közlekedéssel kapcsolatosan kevés magyar nyelvű szakirodalom található, így írásunk fontos célja a szakirodalom bővítése is. Módszer: A vakvezetőkutya-kiképzés nemzetközi hátterét a témakörben publikált tudományos cikkek mellett elsősorban az International Guide Dog Federation online elérhető anyagait és történeti összefoglalóit követve mutatjuk be. A hazai kiképzés ismertetésére a Magyar Vakok és Gyengénlátók Országos Szövetsége Vakvezetőkutya-kiképző Központ és a Magyar Terápiás és Segítőkutyás Szövetség Egyesület szakanyagait használjuk. Eredmények: A szakirodalom alátámasztja, hogy a vakvezetőkutya-kiképzés egy empirikus kutatásokra támaszkodó, dinamikusan fejlődő terület, melynek szabályozása törvényi szinten is megtörtént. A magyar vakvezetőkutya-kiképzés hazai és nemzetközi tapasztalatokra épülve alakult ki. Napjainkban a hazai kiképzés módszertana és maga a szolgáltatás is nemzetközi viszonylatban is elismert. Következtetések: A vakvezetőkutya-kiképzés magasszintű műveléséhez a megfelelő szakmai háttéren és törvényi szabályozáson túl elengedhetetlen a teammunka. Mivel a központok nem képesek a valódi igényeknek megfelelő számú kutyát kiképezni, ezért nagyon fontos lenne, hogy – folyamatos anyagi háttér biztosításával – a szolgáltatás kibővítése megtörténjen.     Abstract: This study presents a comparative analysis of the HRQOL (health related quality of life) of Hungarian children and adolescents with disabilities. This empirical research was carried out during the academic years of 2016/2017, 2017/2018, and 2018/2019. Gathered via the KIDSCREEN-27 questionnaire, the data provide information on the HRQOL of 2,631 children with typical development (TD) (1,411 boys and 1,220 girls, mean age 14.88±2.68 yrs) and 1,056 children with a disability (diagnosed with special educational needs – SEN) (702 boys and 354 girls, mean age 15.72±2.6 yrs). QOL was examined from various aspects. Results in the various subsamples were first compared to Hungarian reference data. Then, an analysis of variance (ANOVA) was performed to detect differences between the subsamples in the five dimensions of QOL. When comparing the QOL of Hungarian children with SEN to Hungarian reference data, we found that only children within an autism spectrum disorder (ASD) had low HRQOL in the dimension of social support and peers. The comparison of the T-values characterising the HRQOL of the subsamples showed that children with other psychological developmental disorders (OPDD), those with ASD, and children with hearing impairment (HI) experience significantly poorer HRQOL T-values in several dimensions compared to children with TD or other disabilities. The physical well-being of children with OPDD is lower than that of students with TD or a mild intellectual disability (MID). In addition, children with OPDD show significantly lower levels of QOL in the school environment dimension compared to other subsamples (children with physical disabilities /PD/, with MID, or with low vision /LV/). Students with HI or ASD showed lower rates of QOL compared to other subsamples in the dimensions of autonomy and parent relations, social support and peers, and school environment. The lower QOL found in the different dimensions will help experts plan and determine the foci of intervention.

    Abstract: Introduction: Body dissatisfaction is a general concern from teenage years onwards, and is especially characteristic in the population of young women all over the world. Objectives: This qualitative research sought to find answers to how young (aged 18-30) women with congenital or acquired blindness perceive their body image (BI), and how they get information about the appearance of others. Methods: Semi-structured interviews were conducted with twelve blind women, aged 18-30 years. Verbatim interview transcripts were analysed independently by both first and second author, following guidelines of constructivist grounded theory (CGT). Coding was conducted in accordance with Strauss and Corbin’s (1998) procedure: transcripts were analysed with open, axial and finally with selective coding. Results: Findings confirm that feedback from important others, especially negative feedback, greatly influences young women with blindness in developing their BI, and that for many it is a concern to what extent they should internalise negative remarks which are related to their appearance. Conclusion: Since feedback and verbal information from others are essential for women with blindness in trying to understand what they and others look like, it is important for professionals, parents and for the women themselves to understand how vulnerable they may be to the criticism of others.

    Abstract: A fejlett országokban a látássérült gyermekek körében vezető kórok az agyi eredetű látássérülés. Az angol nyelvű szakirodalom erre leggyakrabban a CVI (cortical, cognitive, esetleg cerebral visual impairment) rövidítéssel utal, ami a hazai szaknyelvben is széles körben használatos. Így a továbbiakban mi is e betűszóval utalunk az állapotra.A CVI kutatása viszonylag újkeletű, az 1970-es években indult meg az Egyesült Államokban. Bár nemzetközi szakirodalma igen átfogó, magyar nyelven alig-alig találni a CVI tüneteit és a (gyógy)pedagógiai megsegítés módszereit leíró munkákat. Írásunkban röviden ismertetjük a gyermekkori CVI fogalmát, valamint nemzetközi és hazai prevalenciáját. Ezt követően összefoglaljuk kialakulásának lehetséges okait, majd ismertetjük a legjellemzőbb tüneteket.A CVI okai épp annyira változatosak, mint a gyermekek tünetei és speciális szükségletei. Így egyetlen írás nem nyújthat átfogó útmutatást. E „review” célja közérthető nyelven ismertetni a CVI hátterében meghúzódó agyi elváltozásokat, és hogy ezek miként befolyásolják a vizuális feldolgozást. Olyan ismereteket osztunk meg, amelyek – a gyógypedagógus kollégákon túl – segítik a szülőket, befogadó pedagógusokat és más, CVI-vel diagnosztizált gyermekekkel dolgozó szakembereket az állapot megértésében.     Abstract: Myosin modulates the fibrinolytic process as a cofactor of the tissue plasminogen activator and as a substrate of plasmin. We report now that myosin is present in arterial thrombi and it forms reversible noncovalent complexes with fibrinogen and fibrin with equilibrium dissociation constants in the micromolar range (1.70 and 0.94 μM, respectively). Competition studies using a peptide inhibitor of fibrin polymerization (glycl-prolyl-arginyl-proline [GPRP]) indicate that myosin interacts with domains common in fibrinogen and fibrin and this interaction is independent of the GPRP-binding polymerization site in the fibrinogen molecule. An association rate constant of 1.81 × 102 M–1 · s–1 and a dissociation rate constant of 3.07 × 10–4 s–1 are determined for the fibrinogen-myosin interaction. Surface plasmon resonance studies indicate that fibrin serves as a matrix core for myosin aggregation. The fibrin clots equilibrated with myosin are stabilized against dissolution initiated by plasminogen and tissue-type plasminogen activator (tPA) or urokinase (at fibrin monomer-myosin molar ratio as high as 30) and by plasmin under static and flow conditions (at fibrin monomer-myosin molar ratio lower than 15). Myosin exerts similar effects on the tPA-induced dissolution of blood plasma clots. Covalent modification involving factor XIIIa does not contribute to this stabilizing effect; myosin is not covalently attached to the clot by the time of complete cross-linking of fibrin. Thus, our in vitro data suggest that myosin detected in arterial thrombi binds to the polymerized fibrin, in the bound form its tPA-cofactor properties are masked, and the myosinfibrin clot is relatively resistant to plasmin.
