Abstract:
Documentar los factores que perjudican o favorecen las estrategias para reducir las primeras y segundas demoras dentro del Modelo de las Tres Demoras, en áreas rurales y ganaderas de Etiopía. Métodos Se realizó un estudio de informantes clave con 44 agentes de extensión sanitaria en la Región de Afar, Provincia de Kaffa (Región de las Naciones, Nacionalidades y Pueblos del Sur), y Adua (woreda), en la región del Tigray Se entrenó a los agentes de extensión sanitaria para que entrevistaran a las mujeres y les pidieran que explicaran historias sobre sus experiencias recientes con el parto. Entrevistamos a los agentes de extensión sanitaria sobre sus experiencias a la hora de remitir a las mujeres a una atención especializada para el parto y a cuidados obstétricos y neonatales de emergencia. Los datos se analizaron mediante un análisis temático. Resultados En los temas relativos a reducir la primera demora, como son la tradición del parto en casa, la toma de decisiones, la distancia o la falta de transporte, no se observaban variaciones entre las tres localidades. En los temas relacionados con reducir la segunda demora había diferencias sustanciales. Los agentes de extensión sanitaria en Adua (woreda) eran más propensos a llamar una ambulancia, dado el apoyo de la Health Development Army y un sistema de remisión funcional. En Kaffa, a algunos agentes de extensión sanitaria se les llamaba para asistir a las mujeres, puesto que la mayoría de ellas daban a luz en casa con la ayuda de parteras tradicionales, excepto en casos en los que era necesario viajar a un centro sanitario por requerir atención obstétrica o neonatal de emergencia. Conclusiones Las iniciativas para reducir las demoras pueden mejorar el acceso a los servicios de atención materna, especialmente cuando los agentes de extensión sanitaria reciben el apoyo de la Health Development Army y de un sistema de remisión funcional, pero las oficinas distritales (woreda) de salud, deberían asegurar que las ambulancias se utilizan para aquello para lo que están previstas.
