Abstract:
Áp xe cơ thắt lưng chậu là một tình trạng nhiễm khuẩn hiếm gặp trên lâm sàng, do đó mà bệnh thường dễ bị bỏ sót dẫn đến tăng nguy cơ tử vong và tàn phế. Chúng tôi trình bày một trường hợp viêm thân sống đĩa đệm thắt lưng do Salmonella spp ESBL(-), AmpC(-) biến chứng áp xe cơ thắt lưng chậu, là một tình trạng áp xe cơ thắt lưng chậu thứ phát từ viêm thân sống đĩa đệm cột sống thắt lưng với tác nhân vi sinh hiếm gặp là Salmonella. Người bệnh nam, 74 tuổi, nhập viện vì sốt và đau lưng, ở lần nhập viện đầu tiên được bệnh viện tuyến dưới chuyển lên trong tình trạng sốc nhiễm khuẩn đang sử dụng vận mạch. Người bệnh được cấy máu ra tác nhân Salmonella spp ESBL(-), AmpC(-) nhưng ngõ vào của nhiễm khuẩn đã không được tìm thấy, được điều trị sau 2 tuần rồi được cho xuất viện. Sau xuất viện về, tình trạng của người bệnh không khá hơn, vẫn sốt và đau lưng không giảm phải nhập viện lại. Ở lần nhập viện thứ 2, người bệnh được tình cờ phát hiện ra khối áp xe ở cơ thắt lưng chậu 2 bên do biến chứng thứ phát từ nhiễm khuẩn thân sống đĩa đệm. Người bệnh lớn tuổi, cơ địa suy giảm miễn dịch và tổng trạng suy kiệt nên chỉ định can thiệp dẫn lưu ổ áp xe đã không được đặt ra dù rằng kích thước 2 ổ áp xe khá lớn (1,5 x 12 cm bên trái, 1,5 x 6,5 cm bên phải). Người bệnh được điều trị nội khoa bằng kháng sinh và các biện pháp điều trị nâng đỡ, sau 3 tuần điều trị đã hết sốt, giảm đau lưng nhiều, bilan nhiễm khuẩn và tình trạng suy cơ quan đã cải thiện rõ rệt, được cho xuất viện theo dõi ngoại trú tại nhà sau đó.
Abstract Iliopsoas abscesses is a rare clinical infection, so it is often misdiagnosed, leading to an increased risk of mortality and disability. We present a case of lumbar disc osteomyelitis caused by Salmonella spp ESBL(-), AmpC(-) with bilateral iliopsoas abscesses complication. This case is a secondary bilateral iliopsoas abscesses which is complicated from lumbar disc osteomyelitis with Salmonella-a rare microbiological agent. A 74 - year - old male patient was admitted to the hospital with fever and back pain, at the first admission, the patient was transferred from the district hospital in a state of septic shock using vasopressors. The result of blood culture was Salmonella spp ESBL(-), AmpC(-), but the entry of infection was not found. After 2 week treatment, the patient was discharged to home. After discharge, the patient’s condition did not improve, fever and back pain did not relieve and he had to be hospitalized again. At the second admission, the patient was accidentally discovered an abscess in the bilateral iliopsoas secondary due to complication from an lumbar disc osteomyelitis. Because of the patient’s age, immunodeficiency and general debilitation, the indication for abscess’s drainage was not operated even though the size of the two abscesses was quite large (1.5 x 12 cm on the left, 1.5 x 6.5 cm on the right), the patient was only treated with antibiotic and supporting care. After 3 weeks of treatment, the patient had no fever, the back pain was relieved, the infection and organ failure had improved markedly. The patient was discharged for following-up at home.
