    Abstract: Toxic Epidermal Necrolysis (TEN) is a rare, but potentially fatal mucocutaneous reaction, that may occur due to an immunologic response to certain medications. However, TEN triggered by Trastuzumab is extremely rare. Early diagnosis, recognition, and prompt cessation of the offending drugs and initiation of steroid therapy with supportive management are the most important actions for managing TEN. Although rare, it is important to be vigilant about this potential adverse reactions associated with trastuzumab to ensure patient safety and contribute to better outcomes.

    Abstract: This longitudinal type of prospective study was carried out the clinical outcome of intra-articular Hyaluronic Acid injection in the management of mild to moderate osteoarthritis of knee joint. The sample size was 78 which was selected purposively. Partially structured questionnaire was used to collect data from the respondents by face-to-face interview. All efforts were made to collect data accurately. For open questions, the respondents were asked in such a manner way so that they could speak freely and explain their opinion in a normal and neutral way. Regarding age in group distribution of the respondents it was found that majority (47.4%) were in the age group of more than 61 years. The man monthly family income was 215431.21 ± 16898.112 Taka. About 41.0% were overweight, 14.1% had hypertension, 80.8% had multiple symptoms of osteoarthritis in knee joint, 94.87% had restricted range of motion. It was identified that 92.31% relieved pain by injections, 60.25% had satisfactory feeling after injections. The relationship between age group of the respondents was statistically significant (p 〈 0.05; p 〈 0.01) with side effect of injection, pain relief after injection, feeling of patients after injection. The relationship between BMI of the respondents was statistically significant (p 〈 0.05; p 〈 0.01) with pain relief after injection, feeling of patients after injection. The relationship between complications of injection was statistically significant (p 〈 0.05; p 〈 0.01) with systemic diseases of the respondents. TAJ 2021; 34: No-2: 48-55

    Abstract: Modern lifestyle has contributed to serious increases in chronic diseases like cancers and tumors, hypertension, heart diseases, and diabetes mellitus; as well as debilitating diseases like rheumatoid arthritis. Currently, most medications or therapies for treatment of the above diseases have serious side-effects, which sometimes can be more life-threatening than the disease itself. It is important, therefore, to turn to medicinal plant sources for discovery of novel yet safe compounds, which has less or no side-effects to treat the above diseases. The present survey was carried out amongst the traditional health practitioners in the Munshiganj district of Bangladesh to collect information on medicinal plants used by them to treat the above diseases. Information was collected through a series of interviews with traditional health practitioners, rural and urban people. Field notes were recorded on the medicinal plants and their uses; following the methodology of Bhat et al. (1990) and Martin (1995). The identified medicinal plant specimens were stored at the Bangladesh National Herbarium; under the first author's collector series. The following medicinal plants or plant parts were found to be used as remedy for cancers & tumors: Achyranthes aspera (L), Morinda citrifolia (L), Linum usitatissimum (L), Aegle marmelos (L.) Corr. Serr., Derris indica (Lam.) Bennet, Randia dumetorum (Retz.) Poir., Ficus racemosa (L), Ocimum tenuiflorum (L), Polygonum persicaria (L), Abrus precatorius (L), Cyrtandra cupulata Ridl., Myristica fragrans Houtt., and Nigella sativa (L). Medicinal plants used for treatment of hypertension included Bacopa monnieri(L) Pennell, Tinospora cordifolia (Thunb.) Miers, Plantago ovata Forssk., Cocos nucifera (L.), Allium sativum (L), and Manguera indica (L.). Medicinal plants used to treat heart diseases were Ocimum gratissimum (L), Terminalia arjuna (Roxb.) W. & A., Cicer arietinum (L), and Swertia chirata Buch.-Hams. ex Wall. Anti-diabetes mellitus medicinal plants included Mentha spicata (L), Lepidagathis hyalina Nees, Citrus maxima Merr., Syzygium cumini(L.) Skeels., Tamarindus indica (L), Coccinia grandis (L.) Voigt, Aloe vera (L.) Burm. f., Momordica charantia Descourt., Carica papaya (L), Withania somnifera (L.) Dunal, and Emblica officinalis Gaertn. Plants used as remedy for rheumatoid arthritis included Datura metel(L), Achyranthes aspera (L), Ricinus communis (L), Piper betle (L), Calotropis gigantea (L.) W. TAiton, Basella alba (L), Musa sapientum (L), Nigella sativa (L), Aconitum napellus (L), Santalum album (L), Brassica napus (L), Curcuma longa (L), and Boerhavia diffusa (L). A survey of the scientific literature revealed that preliminary studies conducted on some of the above medicinal plants justify their use to treat specific ailments as practiced by the traditional health practitioners. Other medicinal plants need to be scientifically studied towards obtaining new and safer medicines for treatment of diseases like cancers and tumors, hypertension, heart diseases, and diabetes mellitus; as well as debilitating diseases like rheumatoid arthritis, which affect a large portion of the world's population and have become the foremost chronic diseases in modern times.     Abstract: Introduction: In this series, we used five parameters according to the Juhana Leppilahti Modified scoring system to assess final outcome of surgical repair of open achilles tendon repair : the ability to stand on affected tiptoe unsupported, range of motion of affected side of the ankle, power of plantar flexion, calf muscle wasting. Methods: The study was designed as a prospective experimental study conducted at Rajshahi Medical College Hospital and one private hospital of Rajshahi from January 2006 to December 2019. The study was carried out on 50 patients irrespective of age and sex. Pulling down the proximal tendon was the operative procedure. Follow-up was undertaken at the private chamber, the outpatient department and in the orthopedic ward. Result: Among 50 patients, 38(76%) were male, and 12(24%) were female. The mean delay of repair was 6 hours 7 minutes, with a standard deviation ± 2.362 minutes. The mean age was 30.02 years with SD± 7.7. In our study, median level of cut was 3.0 cm proximal to the insertion of tendon Achilles and where the interquartile range was 2.5 to 3.5 cm. The mean follow-up period was six months with a standard deviation of 2.37. Common complications were superficial skin infection in four cases (37%), mild swelling in four cases (36%), ugly scar in two cases (18%), and wound gap in one case (9%). In our study, no major complications occurred, but minor complications like superficial skin infection and delayed wound healing were effectively dealt with an oral antibiotic with no long-term sequelae.  At 16 weeks of follow-up, among 50 patients, 90% (45) can stand on affected tip toes unsupported, around 4 cases (8%) cannot stand on tiptoes of affected foot can't support but able to stand on affected foot with unsupported. Around 2% (1 case) failed to stand on the affected foot without support due to severe infection and wound gap. The eventual outcomes were excellent in 28 cases (56%), good in 16 cases (32%), fair in 4 cases (8%), and poor in 2 cases (4%). In this study, satisfactory Result was obtained after immediate repair of acute open tendon Achilles tendon injury cases. According to Juhana Leppilahti Modified Score at 95% confidence interval, satisfactory outcome was 78.99% to 97.00%. Conclusion: Early repair of open tendo Achilles injuries, complete or partial, provided an effective outcome, which was comparable with the Result described in others. TAJ 2021; 34: No-2: 15-23

    Abstract: Colorectal cancer (CRC) is one of the most common cancers with a high mortality rate. Early diagnosis and therapies for CRC may reduce the mortality rate. However, so far, no researchers have yet investigated core genes (CGs) rigorously for early diagnosis, prognosis, and therapies of CRC. Therefore, an attempt was made in this study to explore CRC-related CGs for early diagnosis, prognosis, and therapies. At first, we identified 252 common differentially expressed genes (cDEGs) between CRC and control samples based on three gene-expression datasets. Then, we identified ten cDEGs (AURKA, TOP2A, CDK1, PTTG1, CDKN3, CDC20, MAD2L1, CKS2, MELK, and TPX2) as the CGs, highlighting their mechanisms in CRC progression. The enrichment analysis of CGs with GO terms and KEGG pathways revealed some crucial biological processes, molecular functions, and signaling pathways that are associated with CRC progression. The survival probability curves and box-plot analyses with the expressions of CGs in different stages of CRC indicated their strong prognostic performance from the earlier stage of the disease. Then, we detected CGs-guided seven candidate drugs (Manzamine A, Cardidigin, Staurosporine, Sitosterol, Benzo[a]pyrene, Nocardiopsis sp., and Riccardin D) by molecular docking. Finally, the binding stability of four top-ranked complexes (TPX2 vs. Manzamine A, CDC20 vs. Cardidigin, MELK vs. Staurosporine, and CDK1 vs. Riccardin D) was investigated by using 100 ns molecular dynamics simulation studies, and their stable performance was observed. Therefore, the output of this study may play a vital role in developing a proper treatment plan at the earlier stages of CRC.

    Abstract: Background: The Oxford–AstraZeneca vaccine (Covishield) was the first to be introduced in Bangladesh to fight the ongoing global COVID-19 pandemic. As this vaccine had shown some side-effects in its clinical trial, we aimed to conduct a study assessing short-term adverse events following immunization (AEFIs) in Bangladesh. Method: A cross-sectional study was conducted on social and electronic media platforms by delivering an online questionnaire among people who had taken at least one dose of the COVID-19 vaccine. The collected data were then analysed to evaluate various parameters related to the AEFIs of the respondents. Results: A total of 626 responses were collected. Of these, 623 were selected based on complete answers and used for the analysis. Most of the respondents were between 30–60 years of age, and 40.4% were female. We found that a total of 8.5% of the total respondents had been infected with the SARS-CoV-2 virus. Our survey revealed that out of 623 volunteers, 317 reported various side-effects after taking the vaccine, which is about 50.88% of the total participants. The majority of participants (37.07%, 231/623) reported swelling and pain at the injection site and fever (25.84%, 162/623); these were some of the common localized and generalized symptoms after the COVID-19 vaccine administration. Conclusion: The side-effects reported after receiving the Oxford–AstraZeneca vaccine (Covishield) are similar to those reported in clinical trials, demonstrating that the vaccines have a safe therapeutic window. Moreover, further research is needed to determine the efficacy of existing vaccines in preventing SARS-CoV-2 infections or after-infection hospitalization.

    Abstract: Healthcare workers (HCWs), such as doctors, nurses, and support staffs involved in direct or indirect patient care, are at increased risk of influenza virus infections due to occupational exposures. Vaccination is the most effective way to prevent influenza. Despite the World Health Organization (WHO) recommendations, Bangladesh lacks a seasonal influenza vaccination policy for HCWs, and thus vaccination rates remain low. The current project aims to investigate the effect of interventions on influenza vaccine awareness and availability of vaccine supply, explore HCWs’ knowledge and perceptions about influenza vaccination, understand the barriers and motivators for influenza vaccine uptake, and understand policymakers' views on the practicality of influenza vaccination among HCWs. Method We will conduct the study at four tertiary care teaching hospitals in Bangladesh, using a cluster randomized controlled trial approach, with the hospital as the unit of randomization and intervention. The study population will include all types of HCWs.The four different types of intervention will be randomly allocated and implemented in four study hospitals separately. The four interventions will be: i) ensuring the availability of influenza vaccine supply; ii) developing influenza vaccine awareness; iii) both ensuring influenza vaccine supply and developing influenza vaccine awareness and iv) control arm with no intervention. Both quantitative and qualitative approaches will be applied to assess the intervention effect. We will estimate the Difference in Differences (DID) with 95% CI of the proportion of vaccine uptake between each intervention and control (non-intervention) arm, adjusting for the clustering effect. The qualitative data will be summarised using a framework matrix method. Discussion The results of this study will inform the development and implementation of a context-specific strategy to enhance influenza vaccination rates among Bangladeshi HCWs.     Abstract: A study was conducted to know the problems and prospects fish nursery operators in Jessore district. A total of 50 nursery operators (Jessore sadar, Jikhorgasha and Avynagor) were interviewed using a structured questionnaire during March-May 2016. The objective of the survey was to study different aspects on pond area, fingerling rearing status, nursery layer, number of employees, sources of fingerlings, survival rate, fingerlings prices, market system, capital sources, fingerling production, prospects and problems for nursery operators. In survey areas, most of the nursery operators were continued their business in two to five ponds with an average area of 1.25±0.04 ha. In the present study, it was found that 4.5±3.25 persons were associated in each nursery but the permanent manpower was minimal. The total fingerlings production in the selected nurseries were 1685 laks of where silver carp 23.74%, Rohu 20.77%, Catla 19%, Mrigal 14.84%, Common carp 10.68%,  Koi 2.97%, Thai Puti 5.93%, Grass Carp 1.19% and Pangus 0.89%.The survival rate were 70-80%. Average price of Indian major carps- were Tk. 350 to 500, Chinese carp were Tk. 200 to 400, Thai sharpunti were 350-400 and Pungus were Tk. 400-500 fingerlings/ kg. In the study area, nursery operators were faced various  problems such as lack of capital (21.25%), diseases problem (19.75%), high lease value (15.00%), shortage of water (5.00%), high price of production inputs (11.00%), flooding (3.00%), terrorism (11.00%), high temperature (3.00%), intense market competition (6.00%) and the lack of policy support (5.00%).
