    Abstract: This study investigated the determinants and use of long‐term care (LTC) service by persons with dementia (PWDs) and their caregivers. PWDs who used LTC services had more severe neuropsychiatric symptoms (NPSs), poorer cognitive function, and inferior ability to engage in activities of daily living (ADLs) and placed a greater burden on their caregivers. More aberrant motor behaviors, greater ADL limitations, greater caregiver burden, not residing with family members, and not employing migrant caregivers were the factors most significantly associated with LTC service use. These factors should be considered in future policy amendments to provide care resources for PWDs and meet the needs of PWDs and their caregivers.

    Abstract: There is a dearth of information on male dementia caregivers in Asia and, in particular, on the factors relating to caregiver burden. We aimed to identify factors that may be associated with burden among male caregivers of people with dementia (PWD). Methods: Data were collected from a national dementia registration survey. The caregiver burden was measured with the short version of the Zarit Burden Interview (ZBI). We analyzed the correlation between ZBI scores and variables, such as demographic data of PWD and their male caregivers, caregivers’ monthly income, the relationship between PWD and caregivers, the severity of dementia, physical comorbidities and activities of daily living (ADL) of PWD, and neuropsychiatric symptoms assessed by the Neuropsychiatric Inventory (NPI). Results: A total of 509 PWD and their male caregivers were included. The majority of caregivers were sons (72.1%) and husbands (22.0%). Sons had higher ZBI scores than husbands (28.5 ± 15.2 vs 22.0 ± 17.0; p 〈 0.001). Multivariable linear regression showed that sons as caregivers (β = 7.44, p = 0.034), ADL (β = 0.52, p = 0.002) and NPI_severity subscore of apathy (β = 2.74, p = 0.001) were positively associated with ZBI scores. Conclusion: Poor ADL and apathy in PWD and being a patient's son were associated with higher levels of burden among male dementia caregivers. Effective interventions are needed to assist male caregivers in accomplishing their caregiving role and at the same time to alleviate their caregiver burden.

    Abstract: Dementia in the oldest-old is projected to increase exponentially as is the burden of their caregivers who may experience unique challenges and suffering. Thus, we aim to investigate which factors are associated with older caregivers’ burden in caring demented outpatients in a multicenter cohort. Methods Patients and their caregivers, both aged ≧65 years, in the National Dementia Registry Study in Taiwan (T-NDRS) were included in this study. Caregiver burden was measured with the short version of the Zarit Burden Interview (ZBI). The correlations between the ZBI scores and characteristics of caregivers and patients, including severity of dementia, physical comorbidities, instrumental activities of daily living (IADL), neuropsychiatric symptoms assessed by the Neuropsychiatric Inventory (NPI), and family monthly income, were analyzed. Results We recruited 328 aged informal caregiver-patient dyads. The mean age of caregivers was 73.7 ± 7.0 years, with female predominance (66.8%), and the mean age of patients was 78.8 ± 6.9 years, with male predominance (61.0%). Multivariable linear regression showed that IADLs (β = 0.83, p   〈  0.001) and NPI subscores of apathy (β = 3.83, p   〈  0.001)and irritability (β = 4.25, p   〈  0.001) were positively associated with ZBI scores. The highest family monthly income (β = − 10.92, p  = 0.001) and caregiver age (β = − 0.41, p  = 0.001) were negatively correlated with ZBI scores. Conclusions Older caregivers of older demented patients experience a higher care burden when patients had greater impaired functional autonomy and the presence of NPI symptoms of apathy and irritability. Our findings provide the direction to identify risky older caregivers, and we should pay more attention to and provide support for these exhausted caregivers.

    Abstract: With the increasing number of individuals with dementia, families have hired an increasing number of live-in migrant caregivers (LIMCs). Currently, limited evidence is available regarding the influence of long-term care resource utilization on the hiring of LIMCs for caring for individuals with dementia in Taiwan. Methods: We recruited individuals with dementia who did not hire LIMCs and their primary family caregivers from nine hospitals in Taiwan as baseline. Multivariable logistic regression was used to evaluate the utilization of long-term care resources for individuals with dementia and other factors that may affect the decision to hire LIMCs. Results: The users of non-long-term care resources had the highest likelihood of hiring LIMCs (odds ratio [OR] = 4.24, 95% CI, 2.30-7.84). Compared with spouses, nonimmediate family caregivers (OR = 3.40, 95% CI, 1.16-9.90) were significantly more likely to hire LIMCs. A higher likelihood of hiring LIMCs was observed for those with Lewy body dementia compared with other individuals (OR = 2.31, 95% CI, 1.03-5.14). Compared with individuals who did not hire LIMCs, those who hired LIMCs exhibited higher scores on the Neuropsychiatric Inventory (NPI) and higher severity of individual NPI items. Conclusion: Hiring LIMCs is strongly correlated with the utilization of non-long-term care resources and is influenced by the dynamics between individuals with dementia and their primary family caregivers. A higher likelihood of hiring LIMCs was observed for individuals with Lewy body dementia and individuals with elevated NPI scores compared with their counterparts. Given these observations, various support strategies and interventions should be tailored to the specific requirements of individuals with dementia and their families.

    Abstract: Epithelial-mesenchymal transition (EMT) has been considered the mechanism underlying distant metastasis in epithelial cancers. The hallmarks of the EMT phenomenon are mainly the loss of cell-cell adhesion, acquisition of a mesenchymal phenotype, gain of mobility and invasiveness, and attainment of enhanced tumorigenicity and anchorage-independent growth. After EMT, cancer cells can enter and survive in fluid-filled spaces and subsequently colonize distant sites. Despite a huge body of in vitro and in vivo evidence recognizing EMT as an important event in tumor progression, whether EMT exists in naturally-occurring experimental settings remains a contentious debate in the field of pathology. Lack of convincing histopathological evidence of EMT in tumor subtypes without spindle sarcomatous tumor components and lack of dynamic change demonstration of EMT are to blame. In this report, we devised a naturally-occurring experimental setting to prove the existence of EMT in tumors completely lacking histological evidence of EMT. Further, we demonstrated dynamic change of EMT in in vitro and in vivo settings. We performed primary culture 37H from a monophasic metaplastic squamous cell carcinoma tissue (MMSCC), which exhibited no histologic features reminiscent of EMT, to examine whether EMT exists in tumors without sarcomatous histology. Two dominant subtypes demonstrating different morphology were derived from parental 37H: polygonal epithelial-type (HE) and epithelioid mesenchymal-type (HM) cells. The HE cells were E-cadherin+vimentin- predominant; while the HM cells were E-cadherin-vimentin+ predominant, and functionally concomitant with increased migration and invasion and anchorage-independent tumorigenicity. Taking advantage of the inherited differences between the HE and HM cells, we performed head-to-head comparison by RNA-seq and subsequently identified key EMT transcription factors (EMT-TFs) ZEB1 and ZEB2 in this naturally-occurring experimental setting. In the primary MMSCC tissue where 37H was derived, the upregulation of ZEB1 and ZEB2 was also observed in vimentin+ carcinoma cells. Further, the dynamic change of EMT was demonstrated in the HE and HM cells in in vitro and in vivo settings. Spontaneous EMT was observed specifically in the ZEB1-or ZEB2-overexpressed polygonal epithelial-type cells; the knockdown of ZEB1 and ZEB2 reverted EMT in the HM cells; and reverse EMT was found a requirement for HM cells to grow in a xenograft mouse model. In conclusion, this report backed up the existence of EMT in naturally-occurring tumor without histologic evidence of EMT. The identification of underlying regulators contributing to spontaneous EMT, and the demonstration of EMT dynamic change were also included.     Abstract: With the advent of Industry 4.0, blockchain is one of the emerging technologies that can be used in multiple fields, especially in supply chain networks, where it is considered a sustainable concept technology. Because of its decentralization, reliability, transparency, consistency, and traceability, blockchain can challenge existing conventional models in supply chain scenarios. Therefore, this study proposes a framework that integrates the technology of Blockchain 3.0 (Hyperledger Fabric) into the tea supply chain. The concept of data traceability driven by blockchain technology consists of documenting the tea supply chain system, in which the cultivation, processing, and retailing of tea leaves are documented, and optimizing the existing anti-counterfeiting mechanisms. The ECDSA algorithm is used to confirm the identity among the entities located in the Hyperledger Fabric channel, and IPFS technology is introduced to store tea traceability data to solve the problem of data increment in the tea production process. Only the hash value and traceability code of the traceability data are stored in the block. We analyze the relationship between the number of transactions and latency, and we can see the latency time of both write and read transactions. The system tested write transactions and read transactions with a minimum write speed of 0.97 s and a maximum of 6.26 s and a minimum read speed of 0.1 s and a maximum of 4.62 s via Caliper analysis. The experimental results show that the performance analysis sounds good. This study uses distributed theory and service-oriented thinking in its specific application to the tea supply chain system model and takes each entity through a B2B connection to achieve anti-counterfeit traceability in the tea supply chain of a regulatory nature.

    Abstract: The subscription economy was born because the relationship between creators and customers is different than it used to be. The era of the creator economy seems to be filled with boundless promise, but at the end of the day, creators are just slaves to tech giants. Neither the control of the content created, nor the money made in their pockets is in complete control of the creator. The blockchain can completely solve these injustices monopolized by enterprises. In the blockchain era, all kinds of creations from music, and video-to-text can be turned into assets that can be purchased and traded through smart contracts. In the music industry, for example, creators do not need to share profits with streaming platforms and record labels and get all the benefits directly. In addition, when the content created is on the chain, every transaction will be recorded on the blockchain, and everyone can inquire about it, avoiding opacity or causing disputes in the future. However, with the structure of the standard blockchain, as long as the registration is successful, each role in the chain will have permanent data access rights, and the flexibility of access control is poor. Therefore, this study proposes a digital media subscription mechanism based on the Hyperledger blockchain architecture combined with proxy re-encryption. We use symmetric and asymmetric cryptography, smart contracts, and algorithms to design our protocol. When the licensee violates the agreement with the creator, the creator can revoke the access rights to the digital media of the licensee at any time, to realize more secure and convenient digital media transmission. The proposed scheme meets various security requirements of blockchain architecture, and we have also applied the BAN logic proof model to evaluate the correctness of the proposed scheme. This study also proposes an arbitration mechanism when the dispute occurs, and performed well in terms of communication and computational costs.

    Abstract: Recent studies on acute mountain sickness (AMS) have used fixed-location and fixed-time measurements of environmental and physiological variable to determine the influence of AMS-associated factors in the human body. This study aims to measure, in real time, environmental conditions and physiological variables of participants in high-altitude regions to develop an AMS risk evaluation model to forecast prospective development of AMS so its onset can be prevented. Results Thirty-two participants were recruited, namely 25 men and 7 women, and they hiked from Cuifeng Mountain Forest Park parking lot (altitude: 2300 m) to Wuling (altitude: 3275 m). Regression and classification machine learning analyses were performed on physiological and environmental data, and Lake Louise Acute Mountain Sickness Scores (LLS) to establish an algorithm for AMS risk analysis. The individual R 2 coefficients of determination between the LLS and the measured altitude, ambient temperature, atmospheric pressure, relative humidity, climbing speed, heart rate, blood oxygen saturation (SpO 2 ), heart rate variability (HRV), were 0.1, 0.23, 0, 0.24, 0, 0.24, 0.27, and 0.35 respectively; incorporating all aforementioned variables, the R 2 coefficient is 0.62. The bagged trees classifier achieved favorable classification results, yielding a model sensitivity, specificity, accuracy, and area under receiver operating characteristic curve of 0.999, 0.994, 0.998, and 1, respectively. Conclusion The experiment results indicate the use of machine learning multivariate analysis have higher AMS prediction accuracies than analyses utilizing single varieties. The developed AMS evaluation model can serve as a reference for the future development of wearable devices capable of providing timely warnings of AMS risks to hikers.
