Abstract:
Uvod. Pseudo-Barterov (Bartter) sindrom cini heterogena grupa poremecaja koja
je klinicki i laboratorijski slicna Barterovom sindromu. Prikazujemo odojce
sa pseudo-Barterovim sindromom uzrokovanim kongenitalnom hlornom dijarejom.
Prikaz bolesnika. Musko dete rodjeno je u 37. gestacionoj nedelji (GN) od
mladih, zdravih i nekonsangvinih roditelja. Ultrazvucnim pregledom je u 35.
GN potvrdjen polihidramnion sa dilatacijom creva. Po rodjenju se manifestovalo
nekoliko pojava hiponatrijemijske dehidratacije s hipohloremijom,
hipokalijemijom i metabolickom alkalozom, te je, zbog sumnje na Barterov
sindrom, zapoceto lecenje novorodjenceta indometacinom, spirinolaktonom i
dodatnim unosom soli. Medjutim, ova terapija nije dala rezultate, pa je dete u
uzrastu od sest meseci, zbog uporne vodene dijareje, povracanja,
dehidratacije i akutne insuficijencije bubrega (nivo kreatinina u serumu bio
je 123 ?mol/l), ponovo hospitalizovano. Rezultati laboratorijskih analiza su
ukazali na hiponatrijemiju (123 mmol/l), hipokalijemiju (3,1 mmol/l), tesku
hipohloremiju (43 mmol/l), alkalozu (pH krvi 7,64, bikarbonati 50,6 mmol/l),
visok nivo renina (20,6 ng/ml) i aldosterona (232,9 ng/ml) u plazmi i malu
koncentraciju hlora u urinu (2,1 mmol/l). Na osnovu ovih nalaza, kao i
visokog nivoa hlora u stolici (110 mmol/l), postavljena je dijagnoza
kongenitalne hlorne dijareje i zapoceta intenzivna nadoknada fekalnog gubitka
vode, natrijuma i kalijuma, koja je tokom deset meseci klinickog pracenja
dovela do normalizacije nivoa elektrolita u serumu i funkcije bubrega, te
optimalnog mentalnog i fizickog razvoja deteta. Zakljucak. Uporna vodena
dijareja s visokom koncentracijom hlora u stolici kljucni je nalaz u
diferenciranju kongenitalne hlorne dijareje od Barterovog sindroma. Terapija
hlorne dijareje je prilicno jednostavna i prevashodno zasnovana na
odgovarajucoj nadoknadi vode i elektrolita.
