Abstract:
Evaluar la correlación entre la distancia total caminada durante la prueba de caminata de seis minutos ( PC 6M) y la función ventricular izquierda con la calidad de vida en pacientes con enfermedad de Chagas ( EC h) complicada con fallo cardiaco. Métodos Estudio croseccional de pacientes adultos con EC h y fallo cardiaco diagnosticado en base a los criterios de Framingham. Se realizó la PC 6M siguiendo las guías internacionales. Se determinaron: la clase funcional según la clasificación de la New York Heart Association, los niveles del péptido natriurético cerebral ( PNC ), los parámetros ecocardiográficos y la calidad de vida (cuestionarios SF ‐36 y MLHFQ ), y se evaluó la correlación de cada uno de ellos con la distancia recorrida en la PC 6M. Resultados En el estudio se incluyeron cuarenta pacientes adultos (19 hombres; 60±12 años) con EC h y fallo cardiaco. La fracción de eyección del ventrículo izquierdo era de 35±12%. Solo dos pacientes (5%) dejaron de caminar antes de que pasasen los 6 minutos. No hubo eventos cardiacos durante la prueba. La distancia media recorrida fue de 337±105 metros. La distancia cubierta presentaba una correlación negativa con PNC (r=‐0.37;P=.02), la puntuación de calidad de vida obtenida con el cuestionario MLHFQ (r=‐0.54;P=.002), la presión sistólica de la arteria pulmonar (r=‐0.42;P=.02) y el grado de disfunción diastólica (r=‐0.36; P=.03) y regurgitación mitral (r=‐0.53; P=.0006) y una correlación positiva con varios dominios del cuestionario SF ‐36. Conclusiones La distancia recorrida durante la PC 6M tiene correlación con el PNC , la calidad de vida y los parámetros de la función diastólica ventricular izquierda en pacientes con EC h y fallo cardiaco. Aquí proponemos adoptar esta prueba en áreas endémicas con recursos limitados para ayudar en la identificación de pacientes que requieren ser referidos a centros terciarios para continuar su evaluación y tratamiento.
