Abstract:
Während die Effektivität störungsspezifischer, multimodaler stationärer Verhaltenstherapie mit Exposition gut belegt ist, wurde bislang noch nicht systematisch untersucht, wie dieses Verfahren von Patienten mit Zwangserkrankung selbst erlebt und bezüglich seiner Wirksamkeit eingeschätzt wird. Die vorliegende Untersuchung widmete sich der Frage, welche Behandlungselemente von Patientenseite als wichtigste Bausteine betrachtet werden. Insbesondere interessierte das Ausmaß der erlebten Belastung durch Exposition im Verhältnis zu ihrem Nutzen sowie die Einschätzung der therapeutischen Beziehung. 124 stationär behandelte Patienten mit Zwangsstörung gaben mit Hilfe eines anonymisierten Fragebogens Auskunft über die Effektivität der stationären Behandlungsbausteine sowie der Elemente im Rahmen des einzelpsychotherapeutischen Prozesses. Die Ergebnisse zeigen, dass Einzelpsychotherapie und Medikamente aus Patientensicht ca. ein Drittel des Behandlungserfolges ausmachen. Innerhalb der Einzelpsychotherapie wird Expositionsbehandlung als wichtigstes Element erlebt, wobei der subjektive Nutzen gegenüber der wahrgenommenen Belastung überwiegt. Die Resultate verdeutlichen den Wert eines multimodalen Behandlungskonzeptes und ermutigen zur Durchführung von Exposition in Begleitung und Eigenmanagement.
