Abstract:
Mục tiêu nghiên cứu: (1) Mô tả đặc điểm lâm sàng, cận lâm sàng của nhóm mạn tính và không mạn tính của bệnh giảm tiểu cầu miễn dịch (GTCMD) ở trẻ em tại Bệnh viện Nhi Đồng 1 (BVNĐ1). (2) Tìm hiểu một số yếu tố liên quan về đặc điểm lâm sàng, cận lâm sàng với diễn tiến mạn tính ở trẻ GTCMD. Phương pháp nghiên cứu: Nghiên cứu mô tả, kết hợp tiến cứu. Đối tượng: 205 bệnh nhi GTCMD tại khoa Sốt xuất huyết-Huyết học BVNĐ1, từ 01/2019 đến 01/2020. Kết quả và kết luận: Đặc điểm chung: Tỉ lệ nhóm bệnh GTCMD mạn tính là 23,4%, nhóm không mạn tính là 76,6%. Ở nhóm mạn tính (48 ca): Nữ 62,5 %, nam 37,5%, nữ/nam 1,67/1; Tuổi trung bình: 83,6 ± 53,4 tháng tuổi; Bệnh thường khởi phát với thời gian biểu hiện bệnh trung bình trước vào viện là 4,9 ± 2,1 ngày. 100% bệnh nhân có xuất huyết ở các mức độ khác nhau, đa số là xuất huyết da và niêm mạc chiếm 72,9%, xuất huyết não 4,2%; Số lương tiểu cầu (SLTC) trung bình lúc nhập viện 19,7 ± 9,6 G/L, SLTC sau điều trị 3 tuần 20,3 ± 7,4 G/L; Tỉ lệ ANA dương tính 12,5%; Tỉ lệ có đột biến gen IFNA17 16,7%. Ở nhóm không mạn tính (157 ca): Nữ 45,9%, nam 54,1%, nữ/nam 0,85/1; Tuổi trung bình: 26,0 ± 39,5 tháng tuổi; Bệnh thường khởi phát với thời gian biểu hiện bệnh trung bình trước vào viện là 2,2 ± 1,1 ngày. 100% bệnh nhân có xuất huyết ở các mức độ khác nhau, đa số là xuất huyết da và niêm mạc chiếm 82,8%, xuất huyết não 1,3%; SLTC lúc nhập viện 18,3 ± 11,7 G/L, SLTC sau điều trị 3 tuần 46,6 ± 24,1 G/L; Tỉ lệ ANA dương tính 3,8%; Tỉ lệ có đột biến gen IFNA17 84,1%. Có mối tương quan thuận (p 〈 0,05) giữa các yếu tố sau với nhóm GTCMD mạn tính: lứa tuổi khởi phát bệnh ở trẻ lớn, thời gian khởi phát xuất huyết dài ngày, SLTC sau 3 tuần điều trị không tăng cao, ANA dương tính. Có mối tương quan nghịch (p 〈 0,05) giữa tỉ lệ đột biến gen IFNA17 với nhóm GTCMD mạn tính.
