Abstract:
Đo hoạt độ enzyme Glucose-6-phosphat Dehydrogenase trong mẫu máu thấm khô được áp dụng trong sàng lọc thiếu hụt enzyme G6PD ở trẻ sơ sinh. Mục tiêu: Phân tích mối liên quan giữa hoạt độ enzyme G6PD trong mẫu máu thấm khô với giới tính, tuổi lấy mẫu, cân nặng khi sinh và thuốc kháng sinh sử dụng ở trẻ sơ sinh. Đối tượng và phương pháp nghiên cứu: Phương pháp mô tả cắt ngang hồi cứu. Thu thập số liệu về các biến số giới tính, tuổi lấy mẫu, cân nặng khi sinh, thuốc kháng sinh sử dụng và hoạt độ G6PD trong mẫu máu thấm khô trên 5886 trẻ sơ sinh. Nhóm nguy cơ thấp và nhóm nguy cơ cao được phân loại dựa trên ngưỡng hoạt độ G6PD lần lượt là ≤ l8 U/dL và 〉 18 U/dL. Kết quả: Giá trị trung bình của hoạt độ enzyme Glucose-6-phosphat Dehydrogenase ở trẻ nam là 54,93 ± 15,999, ở trẻ nữ là 57,37 ± 15,767 U/dL. Nguy cơ cao thiếu hụt G6PD ở trẻ nam cao hơn đáng kể so với ở trẻ nữ. Nguy cơ cao ở nhóm có sử dụng kháng sinh cũng tăng có ý nghĩa thống kê so với trẻ không sử dụng kháng sinh với p 〈 0,01 . Đối với nhóm cân nặng 〈 2500 g có tỷ lệ trẻ thuộc nhóm nguy cơ cao gấp khoảng 2,5 lần so với nhóm trẻ có cân nặng ≥ 2500g. Giữa các nhóm tuổi lấy mẫu khác nhau và thời điểm lấy mẫu ở các mùa nóng và mùa lạnh không có sự khác biệt có ý nghĩa thống kê đến hoạt độ G6PD ở trẻ sơ sinh. Kết luận: Hoạt độ G6PD ở mẫu giấy thấm máu gót chân của trẻ sơ sinh chịu ảnh hưởng của các yếu tố như giới tính, cân nặng lúc sinh và việc sử dụng thuốc kháng sinh nhưng không bị ảnh hưởng bởi tuổi lấy mẫu xét nghiệm và thời điểm lấy mẫu ở mùa nóng và mùa lạnh. Vì vậy, trong đánh giá nguy cơ thiếu enzyme G6PD cần chú ý phân tích đến các yếu tố ảnh hưởng đến hoạt độ enzym này.
