Abstract:
Albumina é o expansor plasmático mais importante da circulação sanguínea. Na forma de medicamento, deve ser prescrito após minuciosa avaliação dos pacientes, baseando seu uso de acordo com a literatura, pois apresenta contraindicações, como hemorragias em pacientes pós-cirúrgicos, além de ser um medicamento de alto custo. Assim, o objetivo deste trabalho foi realizar um levantamento das indicações terapêuticas contidas nas prescrições de albumina humana (AH) 20% de um Hospital Escola da cidade do Recife, Pernambuco, Brasil, a fim de avaliar se tais indicações apresentam embasamento científico, além de ser realizado um levantamento dos custos relacionados à aquisição deste medicamento. Foi feito um estudo transversal, retrospectivo, fundamentado na análise das prescrições receitadas aos pacientes, no período de maio de 2015 a maio de 2017. As indicações terapêuticas encontradas foram: paracentese de alívio (29%), síndrome hepatorrenal (SHR) (22,9%), hipovolemia (20,6%), choque séptico (8,7%), peritonite bacteriana espontânea (PBE) (6,4%), hipoalbuminemia (6,1%), ascite (4,1%), neoplasia (1,2%) e encefalopatia hepática (EH) (0,6%). Após levantamento bibliográfico, a AH 20% apresenta respaldo científico para o tratamento de paracentese, SHR e PBE, o que corresponde a 63,1% das prescrições e não apresenta respaldo científico para o tratamento da hipovolemia, choque séptico, hipoalbuminemia, câncer e EH (36,9%), podendo ser substituída por outros expansores plasmáticos. Com isso, destaca-se a importância de basear a prescrição da AH 20% de acordo com a literatura, visando sempre o uso racional de medicamentos.
