Abstract:
O aborto espontâneo recorrente (ABR) é definido como duas ou mais perdas espontâneas em sucessivas gestações de até 22 semanas. Afeta 1-5% dos casais que tentam conceber. Embora a etiologia das perdas gestacionais espontâneas seja multifatorial, as causas cromossômicas apresentam um papel relevante. Entre 5 e 10% dos casais com perdas reprodutivas, pelo menos um dos cônjuges, apresenta translocações balanceadas e/ou inversões, que levam à formação de gametas anormais e à perdas gestacionais. Foram avaliados, pela citogenética, 9 casais com histórico de ABR, no primeiro e segundo trimestre de gestação, do Laboratório de Reprodução Humana do Hospital das Clínicas, UFG, com o objetivo de investigar a ocorrência de anormalidades cromossômicas nos casais. Os cariótipos foram obtidos a partir da cultura de linfócitos do sangue periférico e bandamento G e C. As idades dos pacientes variaram de 28-57 anos entre os homens (média de 36,3 anos) e de 26-41 anos entre as mulheres (média de 34,2anos). O número de ABR variou de dois a cinco por casal. Em apenas um indivíduo, sexo masculino, oligozoospérmico e idade de 35 anos, foi observada a variante estrutural no cromossomo 9, 46, XY,inv9(p12q13), sendo que nos outros 17 pacientes o cariótipo foi normal, 46,XX e 46,XY para as mulheres e homens respectivamente. O casal, em que o cônjuge apresentou a inversão pericêntrica do 9 tem histórico de 4 abortos no primeiro trimestre de gestação. O resultado mostra que a inv9(p12q13) não pode ser categorizada como normal, pois tem um efeito negativo sobre a fertilidade como indicado pela literatura. Além disso, mostra a importância de estudar o cromossomo em casais inférteis. Conclui-se que para este casal com histórico de ABR, a Fertilização in vitro (FIV) com o diagnóstico pré implantacional (PGD) é importante, assim como a concepção via inseminação artificial por meio do sêmen de doadores. Os demais casais em que tiveram o seu cariótipo normal serão avaliados pela metodologia da amplificação multiplex de sondas dependente de ligação (MLPA) a fim de verificar se existem alterações que não são detectáveis pela microscopia óptica comum.
