    Abstract: Context: One of the most important income sources of hospitals is the payment of insurance organizations, part of which is not paid as deductions in Iran. Studies have reported different values for insurance deductions, proposing various reasons. Objectives: The present study aimed to provide the published evidence of these deductions and their causes over the past 13 years. Data Sources: This systematic review and meta-analysis was conducted in 2021. Data were collected via searching in databases such as Google Scholar, Scopus, PubMed, Web of Science, SID, Magiran, and Irandoc until August 2021 using keywords such as “deduction”, “medical deduction”, “hospital”, “medical center”, and “Iran”. Data Extraction: To calculate the pooled values of hospital deductions, a random-effects meta-analysis was performed in the Stata software version 15 at 95% confidence interval. In addition, the possible heterogeneity of the studies was evaluated using the I2 statistic, and a forest plot was also used to report the results. Publication bias was assessed using a funnel plot. Results: In total, 16 articles were included in the meta-analysis. The pooled percentage of deductions (PPCD) based on random effects was 5% (95% CI: 2.88 - 7.124), and the hypothesis was rejected (PPCD = 0; Z = 31.26; P = 0.000). Based on the random-effects method, the pooled prevalence of deductions was 45.21% (95% CI: 17.601 - 72.824). Moreover, the pooled prevalence of deductions (PPVD) was considered statistically significant (PPVD = 0; Z = 251.16; P = 0.000). The variation in ES attributable to heterogeneity (I2) was estimated at 99.6%, and documentation problems were considered the most common cause of deductions (17.96%). Conclusions: According to the results, the prevalence of deductions in Iranian public hospitals is significant, and the share of deductions from the bill claimed by the hospital is manageable, albeit small.

    Abstract: It can be said that the most important function of the health system is to provide health-care services. This directly affects society’s health and justice. To the best of the authors’ knowledge, this study is the first study that has meta-analyzed the percentage of health services utilization among Iranians to answer this question “How much is the percentage of health-care services utilization in Iran?” Design/methodology/approach This study is a systematic review and meta-analysis conducted in 2020. The literature review was done by searching the databases of Google Scholar, Scopus, PubMed, Web of Science, Science Direct, SID, Magiran and Irandoc from January 2010 to August 2020. Pooled utilization percentage (UP) was estimated by STATA 15 through random-effects meta-analysis with 95% confidence interval. The I 2 statistic was used to investigate the possibility of heterogeneity of articles (I 2 ≥ 50% indicate heterogeneity). A forest plot was applied to report the results. A funnel plot exploited due to publication bias assessment. Findings Approximately 22% of the reviewed articles have been done at the national level of Iran. Most studies have examined the utilization of outpatient and inpatient services. Based on the random effects method, the pooled value of UP was obtained at 47.82%, (95% CI: 39.33–56.31). Also, the pooled percentage of utilization was statistically significant (Test of UP = 0, Z = 456.44, p = 0.000). Practical implications Policymakers can improve the level of utilization from health services by expanding universal coverage, expanding insurance coverage and removing direct and indirect barriers. Originality/value To the best of the authors’ knowledge, this is the first systematic review and meta-analysis on health services utilization in Iran that completely considers the utilization rate in all health services and states the practical solutions to cope with utilization deficiencies and barriers.

    Abstract: BACKGROUND: The prerequisite for promoting safety culture is to assess the existing safety culture level of institutes, because safety precautions without appropriate evaluation increase costs and unforeseen risks. OBJECTIVE: This study aimed to systematically review the status of patient safety culture from the perspective of clinical personnel at Iranian hospitals through a meta-analysis of studies using the Hospital Survey on Patient Safety Culture (HSOPSC) questionnaire. METHODS: The present systematic review and meta-analysis was conducted in 2018. Data were gathered by searching Google Scholar, Scopus, PubMed, and Web of Science databases up to November 2018. Search keywords were “patient”, “safety”, “culture”, “healthcare”, “hospital”, “medical center”, “HSOPSC tool”, and “Iran”. The search protocol was limited to 10 years. To estimate the PSC score, computer software CMA:2 (Comprehensive Meta-Analysis) was used. The presence of heterogeneity across the studies was assessed with the I2 statistic. A forest plot was used to report the results. Publication bias was assessed through a funnel plot. RESULTS: The meta-analysis of studies showed that the PSC score based on the random effect model was 52.7% (95% CI: 50.2%–55.2%), (Q = 522.3, df = 54, P 〈 0.05, I2 = 89.6). A mean of 12 dimensions of HSOPSC showed that the “Teamwork within units” dimension had the highest PSC score (67.2%) and “Non-punitive response to error” had the lowest score (40.4%). CONCLUSIONS: Managers and policymakers should be directed towards non-punitive responses to errors and persuade staff to report errors and execute the approach to learn from mistakes. Also, a periodic government evaluation of the patient safety culture will help further its sustainable development.

    Abstract: BACKGROUND: Reducing interpersonal contact has been one of the least expensive and most widely used COVID-19 control strategies. OBJECTIVE: This systematic review has been conducted with the aim of identifying social distancing strategies and policies and their impact on the COVID-19 pandemic. METHODS: In order to compile this systematic review, Google Scholar, PubMed, Scopus, Web of Science, Science Direct, Magiran, SID, and Irandoc databases were searched from the COVID-19 outbreak until March 2021. Keywords included “social”, “physical”, “distance”, “outbreak”, “incidence”, “prevalence”, “spread”, “new case”, “death*”, “mortality*”, “morbidity*” , “covid-19”, “coronavirus”, “sars-cov-2” and “time series*”. The articles were qualitatively evaluated by two researchers using the STROBE tool. Finally, the study data were divided into three conceptual categories by three researchers, who then agreed on one category. The practical suggestions were also categorized in the same way. RESULTS: The policies and strategies adopted to implement social distancing were included in five categories of restrictions, prohibitions, closures, incentives, and punishments. Transportation and travel restrictions, crowded places and schools closure, use of telecommunications and virtual communications, and financial and psychological support to society members were the main policies in this area. CONCLUSION: Rapid and complete vaccination of all people around the world is out of reach, therefore social distancing and the implementation of physical restraints, especially in crowded and densely populated environments, should be done extensively until COVID-19 is eradicated.

    Abstract: We designed a forecasting model to determine which frontline health workers are most likely to be infected by COVID-19 among 220 nurses. We used multivariate regression analysis and different classification algorithms to assess the effect of several covariates, including exposure to COVID-19 patients, access to personal protective equipment, proper use of personal protective equipment, adherence to hand hygiene principles, stressfulness, and training on the risk of a nurse being infected. Access to personal protective equipment and training were associated with a 0.19- and 1.66-point lower score in being infected by COVID-19. Exposure to COVID-19 cases and being stressed of COVID-19 infection were associated with a 0.016- and 9.3-point higher probability of being infected by COVID-19. Furthermore, an artificial neural network with 75.8% (95% confidence interval, 72.1-78.9) validation accuracy and 76.6% (95% confidence interval, 73.1-78.6) overall accuracy could classify normal and infected nurses. The neural network can help managers and policymakers determine which frontline health workers are most likely to be infected by COVID-19.

    Abstract: Background: Medical equipment maintenance plays an important role in improving the equipment's function, increasing its effectiveness and efficiency, providing continuous health services, and improving the quality of services. This study is conducted to investigate the factors affecting medical equipment maintenance management. Methods: The present study was a qualitative investigation based on content analysis that was conducted in 2021. Data collection method was an open ended interview. The interviewees included all medical equipment operators and technicians of educational and medical centers affiliated with Qazvin University of Medical Sciences. The semi-structured interview guide was used as a data collection tool. Qualitative data obtained from interviews were analyzed by the content analysis method. The obtained coded were classified into dimensions and components using MAXQDA Software12 . Results: According to the findings, categories of time constraints, access to information, instructions and programs, type and number of equipment, financial constraints, user, patients, training programs, and equipment repair agencies were the most important factors affecting medical equipment maintenance management. Conclusion: Observing purchasing standards and guaranteed equipment, training all the staff involved, developing and explaining instructions and programs are essential for optimal equipment maintenance. These measurements will reduce the cost of repairing. g medical equipment and the need to purchase equipment; improve equipment, service quality performance, patient and user safety; and increase the device life.

    Abstract: This study aims to investigate the effect of the Diamond Justice model on self-efficiency with the mediating role of job stress among the staff of Qazvin hospitals affiliated with Qazvin University of Medical Sciences. This study is a cross-sectional descriptive-analytical study conducted among the staff of Qazvin hospitals affiliated with Qazvin University of Medical Sciences in 2020. Design/methodology/approach Sampling was performed using the structural equation method. Data collection tools included three sections: demographic information, justice and self-efficiency questionnaire and job stress questionnaire. Data were finally analyzed using SPSS software version 26 and AMOS version 23 at a significance level of 0.05. Findings The structural equation model’s standard estimation coefficients show that all existing paths are at a significant level. Finally, the regression analysis showed that justice is inversely related to stress level ( ß = −0.185, p = 0.015). Justice is directly related with self-efficiency ( ß = 0.282, p 〈 0.001). Originality/value Justice, stress and self-efficacy have been measured in various studies among health workers. However, a fitting model showing these three variables’ interaction was necessary. Therefore, this study tries to conceptualize the multifaceted relationships of the components of these concepts by presenting a model.
