    Abstract: Background. Urgent osteosynthesis requires number of organizational, material, technical and staff resources. Aim of the study to determine advantages and disadvantages of existing strategies for osteosynthesis basing on literature data and comparative analysis of organization of osteosynthesis on the first day after injury and at a later time. Methods. Data were collected through review of medical records from first half of 2021 calendar year and consist of the patients have been treated by different types of osteosynthesis on the first day after admission to the hospital and later. Average length of hospital stay (LOS) for surgical procedures and duration of the operative time were compared. Results. In total 266 osteosynthesis of the extremities immediately after admission to the hospital were performed in the first half of 2021 in 260 patients. The most frequently performed ankle fractures fixation (20.7%) and clavicle fractures surgical repairment (13.9%). Cases of early infections complications and no revision surgeries required due to unstable fixation after urgent osteosynthesis were excluded. In the same period 659 delayed osteosynthesis were performed. Mean value of inpatient day in patients, who underwent urgent surgery, was 8.4016.67 days, while patients, who underwent delayed surgery, spent significantly greater (p0.05) amount of time in the hospital 12.986.28 days in average. Discussion. Three strategies of osteosynthesis exist: urgent surgeries, delayed surgeries in daytime in operating rooms for planned surgeries and combination of these approaches. Urgent osteosynthesis surgeries do not lead to infectious complications or unstable fixation, what makes them viable option while choosing treatment tactics in case of some injuries. Precise determination of patient groups according to fracture pattern and its localization, that can be operated on in urgent manner, is necessary. Moreover, introduction of urgent osteosynthesis requires serious organizational measures. It is also necessary to perform economical assessment of described approach. Only after solving these questions, it will be possible to make final conclusions about optimal strategy for performing osteosynthesis.

    Abstract: The effectiveness of the treatment of borderline skin burns using a gel of rare-sewn acrylic polymers with natural antimicrobial peptides FLIP-7 was evaluated. A total of 60 burned patients hospitalized in the Burn Center of Saint Petersburg Scientific Research Institute for First Aid named after I.I. Janelidze were examined during the period from 20192021. The following were evaluated: the general condition of patients, planimetric indicators, as well as the results of microscopic examination of smears-prints from the wound surface. This drug was established to be effective in the treatment of borderline skin burns from the first day of use. The burn wound areas upon use of FLIP-7 gel compared to wet-drying dressings and water-soluble ointment levomekol decreased by 6.25% and 8.6%, respectively. A complete restoration of the integrity of the skin was noted by the 7th treatment day, against the background FLIP-7 gel use. The application of a hydrogel with a complex of natural antimicrobial peptides was demonstrated to have a pronounced antibacterial effect against most pathogenic microorganisms. Hence, the number of burn wound infection decreased by 35% by the 4th day from the treatment initiation. There was a possibility of reducing the analyzed indicator to 10% by the end of the week of local treatment with FLIP-7 gel, which indicates almost complete elimination of pathogenic microflora from the burn wound. It was proven that the humid environment conditions formed by hydrogel can stimulate the reparative regeneration processes. This was confirmed by the results of microscopic examination of smear prints, indicating a statistically significant increase in the concentration of fibroblasts on the wound surface by the 4th day of observation compared with the results obtained before treatment initiation with FLIP-7 gel. Not a single case of side effect or undesirable phenomenon was recorded during course of the clinical study. The gel of rare-stitched acrylic polymers with natural antimicrobial peptides FLIP-7 is an effective means of local treatment of borderline skin burns, which provides not only optimal conditions for the course of reparative regeneration processes, but also actively eliminates pathogenic microflora, including polyresistant microorganisms strains.

    Abstract: BACKGROUND: Currently, numerous techniques or medical devices that allow complete or partial restoration of the lost skin within a short time remain as subjects of development. Many studies have shown the effectiveness of using stem cells in the treatment of full-thickness skin defects, but their use remains very limited. At present, there is no consensus among researchers about the advisability of the use of stem cells in the treatment of burns as well as about the method of their introduction. AIM: This study aimed to examine the possibility of accelerating the reparative histogenesis of tissues in the zone of deep skin burns using cultures of adipogenic mesenchymal cells, as well as to evaluate the effectiveness of various methods of introducing cultures of these cells into the defect zone. MATERIALS AND METHODS: An experimental study was carried out on small laboratory animals (rats). After simulating a grade III burn, mesenchymal stem cells were transplanted and superficially applied to the wound surface or injected under the fascia. In the control group, no wound treatment was performed. To assess the effectiveness of the wound-healing preparations, the appearance of wounds was assessed daily, the nature of the discharge and presence and type of granulations were noted, and the timing of scab rejection and wound healing was recorded. The wound area was assessed using a planimetric method. A histological examination of wound biopsies was carried out on days 7, 14, 21, and 28 of observation. RESULTS: The application of adipogenic mesenchymal stem cells demonstrated the greatest efficiency on the developed burn model. Subfascial administration was less effective, but this method achieved a significant acceleration of wound healing in comparison with the control group. An increase in the healing index by 56.6% demonstrates the highest intensity of reparative regeneration in animals applied with adipogenic mesenchymal stem cells. CONCLUSIONS: The preliminary results show that the application of adipogenic mesenchymal stem cells on the skin defect is more effective than subfascial administration based on the healing index. The use of adipogenic mesenchymal stem cells may substantially increase the effectiveness of the treatment of full-thickness skin defects.
