Abstract:
Die intrathekale IgG Synthese kann zum einen quantitativ und zum anderen qualitativ nachgewiesen werden und ist oftmals ein wichtiges Kriterium zur endgültigen Diagnosestellung vor allem bei demyelinisierenden Erkrankungen des zentralen Nervensystems (z. B. der Multiplen Sklerose). Die qualitative IgG Bestimmung mittels isoelektrischer Fokussierung ist die Methode mit der höchsten Sensitivität und ist daher als erste Wahl zu empfehlen. Die quantitative Bestimmung hat den Vorteil der wesentlich rascheren Verfügbarkeit und kann im Falle eines positiven Ergebnisses als Nachweis einer intrathekalen IgG Synthese gewertet werden. In dieser Arbeit werden die gängigsten Methoden und ihre Wertigkeit in der neurologischen Labordiagnostik besprochen.
