    Abstract: Purpose: The role of the minimal residual disease (MRD) in follicular lymphoma is still debated. In this study, we assessed whether the BCL2/IGH rearrangement could have a prognostic role in patients receiving R-CHOP, R-FM, or R-CVP. Experimental Design: DNAs from 415 patients among the 504 cases enrolled in the FOLL05 trial (NCT00774826) were centralized and assessed for the BCL2/IGH at diagnosis, at the end of treatment, and after 12 and 24 months. Results: At diagnosis, the molecular marker was detected in 53% of cases. Patients without molecular marker or with a low molecular tumor burden ( & lt;1 × 10−4 copies) showed higher complete remission (CR) rate and longer progression-free survival (PFS; 3-year PFS 80% vs. 59%; P = 0.015). PFS was significantly conditioned by the PCR status at 12 and 24 months, with 3-year PFS of 66% for MRD− cases versus 41% for those MRD+ at 12 months (P = 0.015), and 84% versus 50% at 24 months (P = 0.014). The MRD negativity at 12 and 24 months resulted in an improved PFS both in CR and in partial remission (PR) patients (3-year PFS = 72% for cases CR/PCR− vs. 32% for those CR/PCR+ vs. 62% for those PR/PCR− and 25% for patients in PR/PCR+; P = 0.001). The prognostic value of MRD at 12 and 24 months of follow-up was confirmed also in multivariate analysis. Conclusions: In this study, standardized molecular techniques have been adopted and applied on bone marrow samples from a large cohort. Data reported show that the MRD detection is a powerful independent predictor of PFS in patients with follicular lymphoma receiving conventional chemoimmunotherapy.     Abstract: Abstract 827 Background and aims: We have recently shown that a consolidation therapy with bortezomib/thalidomide/dexamethasone (VTD) in multiple myeloma (MM) patients responding to autologous transplantation (ASCT) induces major tumor shrinking assessed by real time-quantitative (RQ)-PCR. Moreover we found that low levels of minimal residual disease (MRD) associated to a better progression-free survival (PFS) [GIMEMA VEL-03-096 trial, EudraCT Number 2004-000531-28: Ladetto et al, J Clin Oncol 2010]. We here present the updated results of this study at a median follow-up of 65 months. In the present analysis the following additional issues have been addressed: a) impact of MRD on PFS over time, with special interest to the role of MRD kinetics on outcome; b) impact of MRD on overall survival (OS). Patients and methods: Inclusion criteria and treatment schedule for this study have been already reported [Ladetto et al., J Clin Oncol 2010] and included: 1) a documented complete or very good partial remission following ASCT delivered as first line treatment; 2) no previous therapy with thalidomide or bortezomib; 3) presence of a molecular marker based on the immunoglobulin heavy chain rearrangement (IGH). MRD was assessed on bone marrow samples at diagnosis, study entry, after two VTD courses, at the end of treatment and then at six months intervals, up to clinical relapse. Patients underwent MRD detection using either qualitative nested PCR and RQ-PCR, employing IGH-derived patient specific primers as already described [Voena et al., Leukemia 1997; Ladetto et al., Biol Bone Marrow Transpl 2000] . For outcome analysis patients were grouped according to following definitions: a) MRD negativity on two consecutive samples by the most sensitive PCR method (nested PCR): full molecular remission (FMR); b) MRD negativity on two consecutive samples by RQ-PCR (less sensitive but currently better standardized, according to European Study Group on MRD detection guidelines [van der Vendel et al., Leukemia 2007]): standard molecular remission (SMR); c) post-treatment tumor load above the median by RQ-PCR: high tumor burden (HTB); d) post-treatment tumor load below the median by RQ-PCR: low tumor burden (LTB); e) recurrence of detectable MRD after FMR/SMR: molecular relapse (M-rel); f) increase of MRD levels of at least one log: active disease (AD). Results: Feasibility, toxicity and clinical outcome of the trial have been already reported [Ladetto et al., J Clin Oncol 2010]. Thirty-nine patients were enrolled and median clinical follow-up from start of first line treatment is 65 months. 270 of the planned samples for MRD monitoring (86%) were actually received by the centralized lab. So far 17 relapses and six deaths have been reported. Following VTD consolidation, 7/38 evaluable patients achieved FMR (18%) and 15/38 achieved SMR (39%). Three M-rel were observed, two of them followed by clinical relapse within six months. Achievement of SMR proved highly predictive for PFS (5-years (y) PFS 82% vs 44%, p=0.009, figure 1A), as well as the presence of HTB and AD (5-y PFS 35% vs 87%, p 〈 0.001, figure 2). Interestingly, patients with LTB and no evidence of M-rel or AD had an excellent outcome with a 5-y PFS of 87%, (even considering that molecular follow-up was incomplete due to lack of samples in the two events observed in the low risk group, figure 2). Most notably, none of the patients achieving FMR or SMR has so far died and both SMR and AD proved to be significant predictors for OS (respectively, 5y-OS 100% vs 74%, p=0.012, figure 1B, and 5y-OS 86% vs 100%, p=0.037, data not shown). Conclusions: Our long-term results indicate that: 1) the achievement of SMR following VTD consolidation in MM patients is associated with a better outcome in terms of PFS and OS; 2) a dynamic increase in molecular tumor burden (AD), detectable by RQ-PCR, predicts late disease relapses several months before clinical recurrence. Taken together these results suggest the importance of developing tailored treatment for patients with high residual burden or showing increasing levels of MRD during follow-up, as already pursued for example in mantle cell lymphoma [Andersen et al., J Clin Oncol 2009].     Abstract: Background and aims. In CLL, TL has been associated to outcome. However a larger patient sample and an analysis on a blinded validation series are required to fully establish the independent prognostic value of TL and to define its impact on prognostic subgroups defined according to established predictors. These issues have been addressed on a CLL learning cohort and validated on an independent blinded cohort, overall accounting for 401 CLL patients. Also, we have tested TL as a risk factor of Richter’s Syndrome (RS), an extremely severe event that most of the currently available biomarkers fail to predict. Methods. The learning series (LS) included 191 patients from the university of Torino (UT), while the blinded validation series (BVS) included 210 patients from University of Eastern Piedmont (UEP). TL was assessed on PBMC collected at diagnosis by Southern blotting and no biological or clinical feature of BVS patients was available to the laboratory performing the analysis. Detailed clinical history, clinical parameters at diagnosis as well as VH-mutational status, cytogenetics, CD38 expression and Zap-70 were available for the vast majority of patients and employed together with TL as covariates for multivariate survival analysis. Results. The two series showed no differences for any clinical and biological features except age that was slightly higher in the BVS series. Also TL distribution was similar in the two series (median TL 6024 and 5959 bp respectively). By applying ROC analysis and Youden’s index to the LS we identified a cut-off point of 5000bp segregating 26% (104/401) of patients in the high-risk subgroup. TL was a powerful and independent outcome predictor for both TFS (24.6 vs 73 months p & lt;.001, Fig.1A) and OS (105.5 vs 281 months p & lt;0.001, Fig. 1B). The same cut-off was then applied to the BVS and again TL emerged as strong predictor for TFS (15.2 vs 130.8 months; p & lt;.001) and OS (79.8 vs not reached; p & lt;.001). Multivariate analysis in the BVS selected TL as an independent predictor for both TFS (p=.002) along with peripheral blood lymphocytes (p & lt;.001), CD38 expression (p & lt;.001), beta-2-microglobulin levels (p=.001), and unfavorable FISH karyotype (p=.046), and OS (p.035) (along with age & gt;65 years, p & lt;.001 and advanced Binet stage p=.015). The strict biopsy policy adopted at UEP in case of suspected RS allowed to demonstrate that TL≤5000 acts as an independent predictor also for this event (five-year risk: 18.9% vs 6.4%; p=.016 HR 2,70) (Fig 1C). Finally we analyzed how TL performed in conjunction to established prognostic indicators: TL segregated a CLL group displaying short TFS and OS despite being characterized by Binet A (p & lt;.001), IGHV-homology & lt; 98% (p & lt;.001), CD38 & lt;30% (p & lt;.001) and ZAP70 & lt;20% (p & lt;.001) and favorable FISH (p & lt;.001). Also, among CLL carrying unfavorable prognosticators, TL & lt;5000 bp identified a CLL subgroup with longer OS despite being characterized by age & gt;65 years (p=.004), Binet stage B-C (p & lt;.001), or unfavorable cytogenetics (p=.001). Conclusions. These results demonstrate that TL is a powerful independent predictor of multiple outcome events in CLL and contributes to refine the prognostic assessment of CLL when utilized in combination with other prognostic markers. We thus recommend a more widespread use of this biomarker in CLL. Figure Figure

    Abstract: Introduction. A perturbed microenvironment with secretion of inflammatory cytokines is typical of MM. Prostaglandins (pg) are implicated in inflammation and angiogenesis and play a role in the pathogenesis of several solid malignancies. Expression of COX-2, the key enzyme of pg synthesis in inflamed tissues, is common in many of these cancers and plays a major role in their development. Moreover, it often acts as a poor prognostic indicator. Despite a large amount of data concerning COX-2 expression in solid tumors, few data are currently available in hematological malignancies. In MM there are several biological, epidemiological and clinical considerations suggesting a potential involvement of the pg pathway. Aim of this study is to verify the involvement of COX-2 in MM and to assess its prognostic role. Patients and methods. COX-2 expression has been assessed by western blotting (WB) as previously described (Du Bois RN, et al, Gastroenterology, 1996). Our positive control was the COX-2 positive cell line HT-29, while bone marrows (BM) from 15 healthy donors were our negative controls. We assessed a panel of 124 samples obtained by 113 patients with plasma cell dyscrasias. Sixteen samples belonged to subjects with MGUS, 80 to patients with MM at diagnosis, and 28 to patients with relapsed/refractory MM. In 11 patients, samples taken at different treatment phases were available. To confirm WB findings and to demonstrate that COX-2 expression occurs in malignant plasma cells immunohistochemistry (IC), and flow cytometry for COX-2 were also performed in 31 and four patients, respectively. Finally, COX-2 expression has been assessed in BM cells from four COX-2 positive patients following selection for the CD138 antigen using the Miltenyi cell separation system. COX-2 expression at the mRNA level has also been assessed by real time quantitative PCR. Results. A dilution test showed that our technique is sensitive enough to detect 2% HT-29 cells in a background of COX-2 negative cells. The 15 normal BM were COX-2 negative. In contrast, COX-2 expression was noticed in 12.5% of MGUS, 34.6% of MM at diagnosis and 56% of MM at relapse. COX-2 positivity at diagnosis and relapse was unrelated to disease stage, BM plasmacytosis, creatinine, Hb levels and ß2 microglobulin. COX-2 expression appeared to be of prognostic relevance: at diagnosis the median time to progression was 14 months in COX-2 positive and 40 months in COX-2 negative subjects (p & lt;0.001). At relapse, of 14 patients showing COX-2 expression, 10 have already died of MM, and four are still alive. In contrast, of 11 COX-2 negative patients only one patient died while 10 are still alive (p & lt;0.001). IC, cell separation and flow cytometry studies indicate that COX-2 expression is related to the malignant plasma cell population. COX-2 mRNA was overexpressed in patients showing increased COX-2 protein expression. Conclusions: a) COX-2 is frequently expressed in plasma cell dyscrasias; b) COX-2 expression is more frequent in advanced disease phases; c) COX-2 expression correlates to a worse outcome. Future studies are required to verify whether COX-2 might be clinically useful as a prognostic marker and/or therapeutic target.

    Abstract: Background. Most chronic myelogenous leukemia (CML) patients (pts) restore non-neoplastic hematopoiesis following treatment with tyrosine kinase (TK) inhibitors. However little is presently known on the functional and genetic integrity of Ph-negative hematopoietic cells (HC) repopulating the bone marrow after successful treatment. Indeed, the frequent detection of cytogenetic abnormalities (CA) reminiscent of those seen in myelodysplastic syndromes suggests the potential presence of functional and genetic defects. These issues have been addressed using short and long term HC cultures and telomere restriction fragment length (TRF-L) analysis, which is considered a reliable marker of proliferative and oxidative damage. Patients and methods. We investigated 71 CML pts in stable complete cytogenetic remission (CR) (CR had to be documented at least one year before the analysis). 62 pts were treated with Imatinib and 10 with α-interferon associated or not to ara-C. Median age was 64 (23–88), M/F ratio was 1.5, median time from diagnosis and from complete CR were 58 (7–915), and 40 months (12–150). 31 pts had low Sokal score, 27 intermediate, and 13 high. Complete and partial molecular responders were 35 and 21, respectively. 6 pts showed evidence of acquired CA in Ph-negative HC. TRF-L analysis was performed by Southern Blotting as previously described (Ladetto M et Al, Blood 2004), both on polymorphonucleates (PMN) and on monocyte-depleted PBMC (MD-PBMC) (as described by Rocci et al Exp Hematol 2007) to monitor both the myeloid and lymphoid compartment. Colony-forming unit granulocyte-macrophage (CFU-GM), burst-forming unit erythroid (BFU-E) and colony forming unit-mix (CFU-Mix) along with long-term culture-initiating cells (LTC-ICs) have been so far performed on 30 patients, using bone marrow mononuclear cells as previously described (Sutherland HJ et al Blood 1994). For both TRF-L and cell culture studies a control database of 86 healthy subjects has been used for comparison. Results. PMN from CML patients showed a striking erosion of their telomeric DNA (figure 1A). Also MD-PBMC showed a degree of telomere shortening although the finding was much less pronounced (mean telomeric loss in PMN 1683 pb p & lt;0.001; in MD-PBMC: 323 pb, p=0.04) We found no correlation between TRF-L and previously mentioned clinical parameters. Telomeric erosion is more severe in younger CML pts, resulting in loss of the association between TRF-L and age, typically seen in healthy subjects (figure 1B) Telomere shortening was observed regardless of the use of TK inhibitors. When a multivariate analysis on pts and healthy controls was performed, the presence of CML resulted a stronger predictor of telomeric damage compared to age. We found no correlation between TRF-L and previously mentioned clinical and demographic parameters. Telomeric erosion show no evidence of recovery on 40 follow-up samples taken after a median time of 10 months (range 6–13). Moreover, Ph-negative HC of CML pts were functionally impaired compared to controls with reduced numbers of CFU-Mix (median 2,62 vs 4, p=0,01), CFU-GM (median 99,5 vs 181, p & lt;0,0001) and particularly of LTC-IC (median 88 vs 198, p & lt;0,0001) (figure 1C). Discussion. Ph-negative HC repopulating the bone marrow after successful CML treatment display severe telomeric DNA erosion, roughly comparable to 34 years of physiological aging. Moreover they display major defects in their functional performances. These findings, underline the need of additional investigation and careful clinical monitoring of the Ph-negative haemopoietic compartment in these subjects. Figure Figure

    Abstract: Cyclooxygenase 2 (COX-2) is an inflammation-associated enzyme involved in the pathogenesis of many solid tumors, but little is known about its presence and role in hematologic neoplasms. Multiple myeloma (MM) is known to involve a deregulated cytokine network with secretion of inflammatory mediators. We thus decided to investigate the involvement of COX-2 in this neoplasm. Western blotting (WB) was used to evaluate 142 bone marrow (BM) specimens, including MM and monoclonal gammopathy of undetermined significance (MGUS). Selected cases under-went further evaluation by WB on purified CD138+ cells, immunohistochemistry (IC), and real-time polymerase chain reaction (PCR) for mRNA expression. COX-2 was expressed in 11% (2 of 18) of MGUS specimens, 31% (29 of 94) of MM at diagnosis, and 47% (14 of 30) of MM with relapsed/refractory disease. COX-2 positivity was associated with a poor outcome in terms of progression-free (18 vs 36 months; P & lt; .001) and overall survival (28 vs 52 months; P & lt; .05). Real-time PCR showed COX-2 mRNA overexpression. IC and cell separation studies demonstrated COX-2 expression to be restricted to malignant plasma cells. This is the first report of the presence and prognostic role of COX-2 expression in MM. Future studies will assess COX-2 involvement in other hematologic tumors and its potential use as a therapeutic or chemo-preventive target in onco-hematology. (Blood. 2005; 105:4784-4791)

    Abstract: In mature lymphoid disorders, minimal residual disease (MRD) detection based on real time quantitative PCR (RQ-PCR) of immunoglobulin heavy chain gene rearrangement (IgH) has a well-established role in prognostic assessment, particularly in Mantle cell Lymphoma (MCL) and Multiple Myeloma (MM). RQ-PCR has excellent sensitivity and specificity but has a major limitation in its relative quantification nature, as it requires a reference standard curve usually built with dilutions of diagnostic tumor DNA or on plasmids containing the target rearrangement. Droplet Digital PCR (DD-PCR), applying the principle of limiting dilution of DNA and single molecule detection allows a reliable absolute quantification of target. In this study we compared IgH-based MRD detection by RQ-PCR and DD-PCR, to assess whether DD-PCR could achieve the same performances of RQ-PCR in the absence of the limitation mentioned above. Methods Bone marrow (BM) and peripheral blood (PB) samples were collected from patients affected by MCL and MM in which RQ-PCR based MRD analysis was already performed in the context of prospective clinical trials. In all trials patients gave the informed consent for MRD determination. IgH-based MRD detection by RQ-PCR was carried out as previously described [Ladetto et al. BBMT 2000] and results were interpreted according to the Euro-MRD guidelines [van der Velden et al. Leukemia 2007] . DD-PCR was performed by the QX100 Droplet Digital PCR system (Bio-RAD Inc.) on 500 ng of genomic DNA combined with the same Allele Specific Oligonucleotides (ASO)-primers and TaqMan-probes used in the RQ-PCR. Droplets were generated by QX100 droplet generator. End-point PCR (40 cycles) was performed on a T100 Thermal cycler (Bio-RAD Inc). The PCR product was loaded in the QX100 droplet reader and analyzed by QuantaSoft 1.2 (Bio-Rad Inc). For data interpretation RQ-PCR and DD-PCR results were expressed as amount of target copies per 1E+05 cells. Comparability of MRD results by DD-PCR and RQ-PCR was assessed by means of bivariate correlations between methods analysis (R2.15.1 package irr). Discordances were classified as follows: a positive/negative discordance was defined as major when the positive result was 〉 1E-04 and minor when ≤1E-04; a quantitative discordance was defined as the presence of two positive results with a quantitative discrepancy 〉 1 log. Results Overall, 161 samples belonging to 35 patients (18 MCL and 17 MM), 66 MCL and 95 MM were analyzed. 35 samples were taken at diagnosis and 126 at follow-up. 118 were BM while 43 were PB. A significant correlation was found between DD-PCR and RQ-PCR (R2=0.89, p 〈 0.0001) (fig). DD-PCR and RQ-PCR showed superimposable sensitivity (10-5). Specificity in terms of appearance of non-specific amplifications signals in no-template samples (tested for all patients) and reproducibility on 30 replicates (4 samples) were superimposable. 128 out of 161 samples were fully concordant (Choen's K=0.80). MRD detection was concordantly positive in 106/161 (65.8%) samples and concordantly negative in 22/161 samples (13.7%). Only 5/161 (3.1%) samples showed major qualitative discordance. 28/161 (17.4%) samples showed minor qualitative discordance (which might be related to Poisson's statistics). Quantitative discordances were observed in 5/161 (3.1%) of cases (positive non quantifiable (PNQ) cases were conventionally placed to a value intermediate between sensitivity and quantitative range). Interestingly, 17 samples negative by RQ-PCR were scored positive by DD-PCR (median 6 copies, range 2-74) while 16 samples positive by RQ-PCR (median 5 copies, range 2-44) were negative by DD-PCR. Conclusions Here we report for the first time the use of DD-PCR in the context of IgH-based MRD evaluation in lymphoproliferative disorders. DD-PCR is a feasible tool for IGH-based MRD monitoring in MCL and MM, reaching similar sensitivities compared to standardized RQ-PCR. Moreover DD-PCR allows bypassing the need of building a standard curve thus considerably reducing the complexity of IgH-based RQ-PCR (need of purified diagnostic tissue or Flow Cytometry-based quantification of tumor load or diagnosis, or building of a plasmid-derived standard curve). Finally DD-PCR might potentially overcome the problem of positive non-quantifiable samples. These features make DD-PCR a feasible and attractive alternative method for IgH-based MRD assessment.
