Abstract:
Objetivo: Evaluar el efecto de la Terapia Antirretroviral Altamente Activa (HAART) y la profilaxis con cotrimoxazol sobre la morbilidad después de la elegibilidad para el HAART. Métodos: Entre 1999‐2006, se recolectaron datos de morbilidad en una cohorte basada en la comunidad de pacientes elegibles para HAART, y se compararon los pacientes que iniciaron HAART con aquellos que no lo hicieron. Los pacientes con 15 años o más visitaron la clínica cada 6 meses o cuando estaban enfermos. Se recolectaron datos de base sobre las características de los pacientes, el estadio según la OMS, hemoglobina y conteo de CD4 + . Durante el primer año, tras iniciar el HAART o haber sido calificados como elegibles para el HAART, se recolectaron datos de seguimiento sobre morbilidad (nueva, recurrente y relacionada con la medicación). Hemos estimado el efecto total del HAART sobre la morbilidad; ajustada, mediante métodos de Mantel‐Haenszel, para el efecto de la profilaxis con cotrimoxazol. Se utilizó un modelo de regresión binomial negativa para evaluar el cociente de tasas (RR – por sus siglas en ingles rate ratio ) después de ajustar para otros factores de confusión, incluido el cotrimoxazol. Resultados: 219 pacientes recibiendo HAART (edad media 37 años; 73% mujeres; 82% en profilaxis con cotrimoxazol, hemoglobina media 11.7 g/dl y conteo medio de CD4 131 cel/ul) experimentaron 94 eventos en 127 persona‐años. 616 pacientes que no recibían HAART (edad media 33 años; 70% mujeres; 26% en profilaxis con cotrimoxazol, hemoglobina media 11.2 g/dl y conteo medio de CD4 130 cel/ul) experimentaron 862 eventos en 474 persona‐años. La morbilidad total durante el primer año de HAART fue un 80% menor que entre pacientes que no recibían HAART (RR ajustado = 0.20, 95% IC: 0.12‐0.34). La profilaxis con cotrimoxazol también redujo la morbilidad (RR ajustado=0.65, 95%IC: 0.45‐0.94). Conclusión: Estos resultados confirman la reducción de morbilidad gracias al HAART, así como la protección adicional que brinda la profilaxis con cotrimoxazol.
