Abstract:
Đặt vấn đề: Việc sàng lọc, chẩn đoán bệnh beta-thalassemia cần dựa vào đặc điểm lâm sàng và kết quả cận lâm sàng. Đồng bằng sông Cửu Long với số người bệnh hoặc mang gen betathalassemia chiếm tỷ lệ cao trong số các bệnh nhân thalassemia nhưng lại có ít các nghiên cứu về khoảng giá trị cận lâm sàng của bệnh, đặc biệt là điện di hemoglobin. Mục tiêu nghiên cứu: Mô tả đặc điểm tổng phân tích tế bào máu ngoại vi và điện di hemoglobin các thể beta-thalassemia. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 45 bệnh nhân betathalassemia đến khám và điều trị tại Bệnh viện Huyết học - Truyền máu Cần Thơ từ tháng 06/202106/2022. Kết quả: Trong 45 trường hợp được chúng tôi nghiên cứu, ghi nhận giá trị trung bình các chỉ số trên kết quả tổng phân tích tế bào máu ngoại vi là: số lượng hồng cầu (RBC) 3,07 ± 0,99x1012/L, số lượng bạch cầu (WBC) 11,75 ± 7,22x109/L, số lượng tiểu cầu (PLT) 355,16 ± 225,22x109/L, nồng độ hemoglobin (Hb) 6,51 ± 1,64 g/dL, thể tích trung bình hồng cầu (MCV) 69,64 ± 9,13 fL, lượng hemoglobin trung bình hồng cầu (MCH) 21,88 ± 3,39 pg và dải phân bố kích thước hồng cầu (RDW) 26,22 ± 6,89%. Đồng thời trên kết quả điện di Hb ghi nhận tỷ lệ HbA1 trung bình là 33,42 ± 37,61%, HbA2 và HbF trung bình lần lượt là 4,71 ± 1,82% và 37,45 ± 26,11%, HbE xuất hiện ở thể beta-thalassemia/HbE với tỷ lệ trung bình 43,86 ± 9,13%. Kết luận: Bệnh betathalassemia làm thay đổi các chỉ số trên kết quả tổng phân tích tế bào máu ngoại vi: RBC, WBC, PLT, Hb, MCV, MCH đều giảm và RDW tăng; đồng thời thay đổi tỷ lệ các thành phần Hb trên kết quả điện di hemoglobin đặc hiệu cho từng thể bệnh.
